Enclose a copy of your Florida Public Service Commission operating certificate. <br /> <br />PART B: DESCRIPTION OF PROPOSED SYSTEM AND SERVICES <br /> <br />1. Describe in detail: <br /> <br />a. The geographic area within the City of Sunny Isle Beach where the Applicant proposes to <br />offer Telecommunications Services and a schedule for buildout to the entire area. <br /> <br />b. Provide the City with written documentation that; <br /> <br />(i) The Applicant made diligent, but unsuccessful efforts to install or co-locate the <br />Applicant's telecommunications facilities within ducts or conduit owned by other <br />persons and present at the proposed location of the new facilities. <br /> <br />(ii) The Applicant made diligent, but unsuccessful efforts to install or co-locate the <br />Applicant's telecommunications facilities overhead on existing utility poles at the <br />Proposed location of the new facilities. <br /> <br />2. Where, if at all, the Applicant intends to interconnect its system with other Telecommunications <br />Systems, Open Video Systems or cable systems in Sunny Isle Beach. <br /> <br />3.
