The affidavit must <br /> <br />be executed in the presence of a notary, and include: <br /> <br />(a) If known to the Vehicle Owner, the name, address, and the driver <br /> <br />license number of the person 'tyho leased, rented or otherwise had cure, <br /> <br />custody, or control of the motor vehicle at the time of the alleged Red <br /> <br />Zone Infraction; or <br /> <br />(b) If the ';ehicle Vias stolen, the police report indicating the ';ehicle <br /> <br />'.vas stolen at the time of the alleged Red Zone Infraction. <br /> <br />Red Light Camera Ord.
