C09l0-050 <br /> <br />THIS THIRD AMENDMENT TO THE TENEX ENTERPRISES, INC., AGREEMENT <br />FOR SR AlA/COLLINS A VENUE AT l83RD STREET STREETS CAPE IMPROVEMENTS <br />PROJECT, executed this ~~ day of .JVl,.....h' 2010, is attached hereto and made a part of the <br />original Agreement between the parties approve via ResolutIOn No. 2009-1495, between the CIty of <br />Sunny Isles Beach ("City") and Tenex Enterprises, Inc. ("Contractor") a business corporation licensed in <br />the State of Florida, whose Federal Identification # is !S)..... 'Zo~~ (p t C\ The City and <br />Contractor hereby agree as follows: <br /> <br />1. INCORPORATION OF FEDERALLY MANDATED CONTRACT DOCUMENTS: The <br />City wishes to amend the original Agreement with the Contractor for SR AlA/Collins Avenue at 183rd <br />Street Streetscape Improvements Project to include updated form "FL325", effective 10/09/2009, <br />published by the Department of Labor, and updated form "275-030-11B", Equal Opportunity Office, <br />effective 02/08. In addition, the City wishes to incorporate the following federally mandated contract <br />documents into the original Agreement between the parties, including (1) Form FHW A-1273 (Rev. 3-94), <br />required contract provisions federal aid construction contracts, and (2) Executive Order 11246 - equal <br />employment opportunity. These forms are attached hereto as composite Exhibit "A" and made a part of <br />the original Agreement between the parties. <br /> <br />2. EXTENSION OF TIME: The parties agree that Contractor shall have an extension oftime of 60 <br />(sixty) days to complete the Work described in the Contract Documents. <br /> <br />3.
