The following relationships are the only potential, <br />actual, or perceived conflicts of interest in connection with this proposal: (If none, <br />state none.) <br /> <br />7. Public Disclosure. In order to determine whether the Contractor or its <br />principals have any association or relationships with the City which would <br />constitute a conflict of interest, either actual or percieved, and in an attempt to <br />ensure full and complete disclosure regarding this contract, all Proposers are <br />required to disclose all persons and entities who may be involved with this Proposal. <br />This list shall include public relation firms, lawyers and lobbyists. The City <br />Manager or his designee shall be notified in writing if any person or entity is added <br />to this list after receipt of proposals. <br /> <br />8. Are there any pending discussions relative to mergers, acquisitions, <br />partnerships, or assignment of contract? <br /> <br />The Proposer understands that information contained in this Questionaire will <br />relied upon by the City in awarding the proposed contract, and such information is <br />warranted by the Proposer to be true and accurate. The Proposer agrees to furnish <br />such additioanl information, pior to acceptance of any proposal relating to the <br />qualifications of the Proposer, as may be required by the City Manager or his <br />designee. <br /> <br />18 <br />
