<br />"SEALED BID" <br />RFP # 10-08-01 PROFESSIONAL CONSTRUCTION SERVICES <br />FOR THE CENTRAL ISLAND EMERGENCY OUTF ALLS IMPROVEMENTS <br />OPENING DATE AND TIME: WEDNESDAY, SEPTEMBER 15,2010 AT 10:00 A.M. <br /> <br />1.4 SUBMITT AL REQUIREMENTS <br /> <br />All submittals by contractors shall contain no more than fifteen (15) pages specifically <br />addressing the following issues. Item numbers 9 and 10 below will not be considered a part of <br />the fifteen (15) pages. Any contractor wishing to provide the services described in Section 1.2 <br />above must submit one (1) original and four (4) additional complete copies that include, at a <br />minimum, the following documentation: <br /> <br />1. Name, address and company, including, but not limited to, a business overview, financial <br />state of the business, annual revenue for past two years, and names and addresses of <br />persons having financial interest in the firm. <br /> <br />2. Provide a narrative description of current and previous experience on a project similar to <br />the Central Island Emergency Outfalls Improvements project described in this RFP. <br /> <br />3. A list of all projects (including government projects) completed or in process for the past <br />five (5) years, and a corresponding list of client references with contact names and <br />telephone numbers. <br /> <br />4. Proof of authorization to transact business in Florida from the Florida Secretary of State, <br />from the prime as well as supporting firms. <br /> <br />5. The name, title, qualifications, experience, and all contact information of the proposed <br />contact person and proposed personnel to be assigned to this project. <br /> <br />6. Provide a detailed description of the Proposer's procedures for coordinating the Central <br />Island Emergency Outfalls Improvements project, including the project implementation <br />schedule, identifying events and the estimated number of calendar days to achieve <br />completion. <br /> <br />7. Provide a detailed financial cost breakdown of the total cost involved in completing the <br />Central Island Emergency Outfalls Improvements project described in this RFP. <br /> <br />8. Provide documentation demonstrating Proposer's [mancial strength and [mancial capability to <br />provide the construction services in this RFP. Such documentation should include Proposer's <br />most recent certified [mancial statements as of a date not earlier than the end of the Proposer's <br />preceding official tax accounting period, with a statement in writing, signed by a duly authorized <br />representative, stating that the present [mancial condition is materially the same as that shown on <br />the balance sheet and income statement submitted, or with an explanation for a material change <br /> <br />6 <br />
