C4775-1516-118 <br /> THIS AGREEMENT ("Agreement") entered into this (511± day of St i.MBER- 2016; <br /> by and between the CITY OF SUNNY ISLES BEACH (hereinafter referred as to "City"), a municipal <br /> corporation of the State of Florida, and ACTION LABOR MANAGEMENT, LLC. D/B/A STAFFING <br /> CONNECTION a Florida corporation authorized to do business in the State of Florida (hereinafter <br /> referred to as "Contractor`); whose Federal Identification # is 65-1038617. The City and Contractor <br /> hereby agree as follows: <br /> RECITALS <br /> WHEREAS, the City of Sunny Isles Beach is in need of a contractor to provide school crossing <br /> guards at several locations located within the City of Sunny Isles Beach ("Services"); and <br /> WHEREAS, the City issued Invitation to Bid No. 16-04-05 for School Crossing Guard Services, <br /> and Contractor submitted a response thereto, a copy of which is attached hereto and incorporated herein <br /> as Attachment "A": and <br /> WHEREAS, Contractor was selected as the lowest responsible, responsive bidder in response to <br /> Invitation to Bid No. 16-04-05: and <br /> WHEREAS; the City wishes to enter into an Agreement with Contractor to provide the desired <br /> Services as more particularly described in Attachment "A", for a total amount not to exceed Two Hundred <br /> Twenty Nine Thousand Nine Hundred Twenty Four Dollars ($229,924.00) during the initial two (2) year <br /> term. <br /> NOW THEREFORE, in consideration of the premises and the mutual covenants herein names; <br /> the panics agree as follows: <br /> TERMS <br /> 1. RECITALS. The recitals set fonh above are hereby incorporated into this Agreement and made <br /> a pan hereof. <br /> 2.
