Many of DBPR's services are available online at <br />www.MyFloridaLicense.com. We encourage you to utilize these services to make address changes, licensing changes or <br />to renew your license. <br /> <br />Name changes require legal documentation verifying the name change, which must be mailed to DBPR. An original, a certified <br />copy, or a duplicate copy of an original or certified copy of a document that shows the legal name change will be accepted, <br />unless DBPR has a question about the authenticity of the document. <br /> <br />If applicable, DBPR will send a renewal notice to your last known address prior to the expiration date on your license.
