Federal and State Awards <br />The City of Sunny Isles Beach anticipates receiving the following during FY 2010: <br />Florida Department of Transportation $457,000 <br />Safe Neighborhood Parks $312,500 <br /> <br />1.5. PROPOSAL REQUIREMENTS <br /> <br />A. General Requirements <br /> <br />Submission of Proposals <br /> <br />Proposals shall be as thorough and detailed as possible so that the City may <br />properly evaluate the capabilities of respective firms to provide the required <br />services. All submittals by Proposers shall contain no more than fifteen (15) <br />pages specifically addressing the following issues. All other required <br />documentation (except for the Public Entity Crime and Non-Collusive <br />Affidavits) will not be considered a part of the fifteen (15) pages. <br /> <br />All Proposals shall be submitted in the form required and shall include all <br />requested documentation and a completed Public Entity Crime Form along <br />with a completed Non-Collusive Affidavit. <br /> <br />a. Any Proposer wishing to provide the Services described in Sections <br />1.1 through 1.4 above must submit one (1) original and four (4) <br />additional complete copies that include, at a minimum, the following <br />documentation: : <br /> <br />1. Title Page <br /> <br />Title page showing the request for proposals subject; the <br />firm's name; the name, address and telephone number of the <br />contact person; and the date of the proposal. <br /> <br />H. Table of Contents <br /> <br />HI. Transmittal Letter <br /> <br />A signed letter of transmittal briefly stating the proposer's <br />understanding of the work to be done, the commitment to <br />perform the work within the time period, a statement why the <br />firm believes itself to be best qualified to perform the <br />engagement. <br />
