<br /> <br />Addendum No. 3 <br /> <br />CITY OF SUNNY ISLES BEACH <br />Professional Audit Services <br />CITY RFP NO. 10-06-01 <br /> <br />To All Bidders: <br /> <br />Bidders for the above-referenced project shall take note of the following changes, <br />additions, deletions, clarifications, etc. to the Plans and Specifications, which in <br />accordance with the Contract Documents shall become a part of and have precedence <br />over anything shown or described otherwise. <br /> <br />~ Please note the following deletions/additions to the RFP Specifications. This <br />addendum must be signed and included in your firm's response: <br /> <br />1.5. PROPOSAL REQUIREMENTS <br /> <br />A. General Requirements <br /> <br />Submission of Proposals <br /> <br />Proposals shall be as thorough and detailed as possible so that the City may <br />properly evaluate the capabilities of respective firms to provide the required <br />services. All submittals by Proposers shall contain no more than fifteen (15) <br />pages specifically addressing the following issues. * All other required <br />documentation (except for the Public Entity Crime ::md Non Collusi\(e <br />Affidavits) will not be considered a part of the fifteen (15) pages. <br /> <br />a., *ReQuired documentation requested such as Quality control reviews,! <br />resumes. sample manaaement letters. Public Entity Crime Affidavit. the <br />Non-collusive Affidavit. and the Schedule of Professional Fees will not be <br />considered as part of the fifteen paaesJ <br /> <br />All Proposals shall be submitted in the form required and shall include all <br />requested documentation and a completed Public Entity Crime Form along <br />with a completed Non-Collusive Affidavit. <br />
