Q: On page 4 of RFQ 10-04-01, the City of Sunny Isles Beach requests "A list of all <br />projects (including government projects) completed or in process for the past five <br />years, and a corresponding list of client references with contact names and telephone <br />numbers." - Is this item excluded from the 10-page limit? <br /> <br />A: Yes. <br /> <br />Q: On page 4 of RFQ 10-04-01, the City of Sunny Isles Beach requests "Proof of <br />authorization to transact business in Florida from the Florida Secretary of State, from <br />the prime as well as supporting firms." - Is this item also excluded from the 10- <br />page limit? <br /> <br />A: No. <br /> <br />Q: Should we include tax documents to indicate financial state of business? Or what <br />other document should we include to show financial state of business? <br /> <br />A: Tax documents are not necessary, just a brief summary will be fine. <br /> <br />Q: Do these count as part of the ten-page proposal? <br /> <br />A: Yes. <br /> <br />Q: Do the sworn statement and affidavit count as part of the ten-page proposal? <br /> <br />END OF SECTION <br /> <br /> <br />A: No. <br />
