SCHEDULE OF INSURANCE (CONTINUED) <br />DEPENDENTSINSURANCE <br />(For Class 1) <br />�ependent Amount of Life Insurance <br />Spouse $10,000 <br />Dependent Child 250 <br />(age 14 days to 6 months) <br />Dependent Child 5,000 <br />(age 6 months to 19 years, <br />23 years if a full -time student) <br />Spouse Life Insurance will terminate when the Spouse attains age 70. <br />Dependent's Life Insurance is subject to a maximum of 50% of the Insured Employee's Life Insurance Benefit. <br />Insured Persons are not required to make contributions for Personal Life Insurance and AD &D Insurance.
