5.1.2.2 Independent Contractors <br />5.1.2.3 Products and/or Completed Operations Hazard. <br />5.1.2.4 Explosion, Collapse and Underground Hazard Coverage. <br />5.1.2.5 Broad Form Property Damaged. <br />5.1.2.6 Broad Form Contractual Coverage applicable to this specific Contract, <br />including any hold harmless and /or indemnification agreement. <br />5.1.2.7 Personal Injury Coverage with Employee and Contractual Exclusions <br />removed, with minimum limits of coverage equal to those required for <br />Bodily Injury Liability and Property Damage Liability. <br />5.1.3 Business Automobile Liability with minimum limits of One Million Dollars (1,000,000.00) per <br />occurrence, combined single limit for Bodily Injury Liability and Property Damage Liability. Coverage <br />must be afforded on a form no more restrictive than the latest edition of the Business Automobile <br />Liability policy, without restrictive endorsements, as filed by the Insurance Services Office, and must <br />include: <br />5.13.1 Owned Vehicles. <br />5.1.3.2 Hired and Non -Owned Vehicles. <br />5.1.3.3 Employers' Non - Ownership. <br />5.2 Before starting the Work, the CONTRACTOR will file with the CITY and CONSULTANT certificates of such <br />insurance, acceptable to the CITY; these certificates shall contain a provision that the coverage afforded under the policies <br />will not be canceled or materially changed until at least thirty (30) days prior written notice has been given to the CITY <br />and CONSULTANT by certified mail. The CITY shall be named as an additional insured on the above - referenced <br />policies. <br />5.3 The CONTRACTOR agrees that if any part of the Work under the Contract is sublet, they will require the <br />Subcontractor(s) to carry insurance as required, and that they will require the Subcontractor(s) to furnish to them <br />insurance certificates similar to those required by the CITY in 5.1 above. <br />Fire and Extended Coveraue Insurance (Builders' Risk): <br />5.4 The CONTRACTOR shall maintain, as applicable, in an Insurance Company or Insurance Companies acceptable <br />to the CITY, Fire, Extended Coverage and Vandalism & Malicious Mischief Insurance on buildings and structures, while <br />in the course of construction, including foundations, additions, attachments and all permanent fixtures belonging to and <br />constituting a part of said buildings or structures. The policy or policies shall also cover machinery, if the cost of <br />machinery is included in the Contract. The amount of insurance must at all times be at least equal to the actual cash value <br />of the insured property. <br />The policy shall be in the name of the CITY and the CONTRACTOR, as their interest may appear, and shall also cover <br />the interests of all Subcontractors performing Work. <br />5.5 The CONTRACTOR shall provide the CITY with satisfactory evidence certifying that the foregoing insurance is <br />in force; and such evidence shall include provisions that the insurance shall not be canceled, allowed to expire or be <br />materially changed without giving the CITY advance notice by registered mail. <br />00710-8 <br />
