0 IL REPORTING INJURIES OR PROPERTY DATNIAGE <br />• <br />No matter how careful everyone may be, accidents do happen. Therefore, no matter how minor, <br />accidents must be reported to the supervisor immediately after the accident occurs (but no later <br />than at the end of the shift). <br />Late reporting (after 24 hours) will result in disciplinary action. <br />Reporting the exact location and nature of the injury or damage is extremely important to the site <br />supervisor. He or she is responsible for the follow -up actions to ensure other employees and site <br />personnel are aware of the accident and the actions taken to prevent furthur injury or damage. <br />• The supervisor must get the following information for the Accident Report: <br />• The employee's name <br />• Date and time of the accident <br />• Description of injury or damage <br />• The cause of the accident <br />• Witnesses <br />SUPERVISOR MUST REPORT ALL ACCIDENTS TO THE CSI OFFICE WITHIN 24 <br />HOURS AFTER THE ACCIDENT. <br />7 <br />
