<br />I <br /> <br />t_COR ZO <br />_CASTELLA <br />CARBAllO <br />r.rm THOMPSON <br />UJUSAlMAN <br /> <br />I <br /> <br />EXPERIENCE <br /> <br />I <br /> <br />Mr. Crews has been design and field engineer on various projects in the field of struclural and civil <br />engineering. Mr. Crews' bridge experience includes both the designs of new structures as well as <br />rehabilitation of existing structures. This experience has included cast-in-place, AASHTO girders, Repair <br />and inspection of steel bridges, box culverts, and rehabilitation of Bascule bridges and tender house <br />facilities. Mr. Crews has also inspected and rehabilitated historic bridges such as the bridges along the <br />Venetian Causeway in Biscayne Bay. Mr. Crews' civil experience has included the preparation of utility <br />relocation plans, drainage analysis, and development of traffic control and stormwater pollution <br />prevention plans on various FOOT projects throughout South Florida. Mr. Crews' experience has <br />included the design of numerous projects with utility coordination issues. Mr. Crews also has had <br />extensive design, construction inspection and administration experience in all types of building structures. <br />Recent relevant experience includes: <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />· Lauderdale Lakes Park Pedestrian Bridge, Lauderdale Lakes, Florida - Project Manager for the <br />redevelopment of an existing park for the City of Lauderdale Lakes. The project was being performed <br />as collaboration with the Broward County School Board to provide a student drop off for a new <br />Elementary school on an adjacent property, The project featured a 110' long fully enclosed pedestrian <br />bridge to provide access for the students across the South Florida Water Management ~istrict C-13 <br />Canal. The project also included approximately 1500' of new roadway with additional parking, as <br />well as several hundred feet of covered walkway for student protection. The project also included the <br />development of a concession and restroom building for the park, as well as renovation of an existing <br />press box structure. New sports field lighting was also provided for all of the play fields, <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />· 1-95 over SR-40 Bridge Replacements, Vol usia County, FDOT - District V - QC Officer and <br />Roadway Engineer responsible for the design of the replacement of the existing twin three-span <br />structures over SR-40 with a new twin single span structure. The new structures provide a 40-meter <br />(131.25') clear opening beneath 1-95. The typical section consists of six lanes divided with three- <br />meter (10 foot) shoulders. The superstructure is supported by pre-stressed piling and MSE walls for <br />the embankment. <br /> <br />I <br /> <br />I <br /> <br />· SR- lOA (US-90 Cervantes Street) Bridge Replacement over Bayou Texar, Escambia County, <br />FDOT - District III - QC Officer and Roadway Engineer responsible for the preparation of full <br />roadway and bridge plans for the replacement of a 155.8 meter (511 L.F.) four-lane bridge over <br />coastal waters and reconstruction of 509.1 meters (1670 L.F.) of approach roadway. The bridge is to <br />be constructed using phased construetion and maintaining four lanes of traffic operational at all times. <br />This is being accomplished through the use of a two-lane acrow bridge built over the Bayou with sheet <br />pile abutments into the Bayou. The project has architectural features, as well as bridge and roadway <br />lighting. <br /> <br />I <br /> <br />I <br /> <br />I <br /> <br />· Sunset Drive Bridge Emergency Repairs, Miami Beach, FDOT - District VI - QC Officer and <br />Roadway Engineer responsible for the inspection, load rating and emergency repairs of a ten-span, <br />250 bridge. Superstructure type consisted of steel girders supporting a cast-in-place concrete deck, <br />Steel girders were nearly eorroded and bearing supports required replacement/rehabilitation. Repairs <br />were performed so that the bridge could be replaced at a later date. <br /> <br />I <br /> <br />· Venetian Causeway Bridges Rehabilitation / Restoration Study, Miami Beach, FDOT - District <br />VI - QC Officer and Roadway Engineer assigned to the bridge inspection team for inspecting ten <br />concrete girder fixed span bridge and two bascule bridges. The in-depth bridge inspection performed <br />on these historic bridges was for the purpose of developing our in-depth condition survey to develop <br />preservation / rehabilitation schemes. Inspected components included structural steel on bascule <br />leaves, superstructure and substructure elements of concrete girders, utility supports and bridge <br />railings. Upon completion of the condition survey a recommendations report was prepared for FOOT. <br /> <br />I <br /> <br />I <br /> <br />· Sunset Drive Bridge Replacement between Sunset Islands No.1 & 2, Miami Beach, FDOT _ <br />District VI - QC Officer and Roadway Engineer responsible for replacement of an existing ten span, <br />250 foot bridge with a two-lane, seven-span, cast-in-place continuous concrete slab bridge supported <br />on concrete bents and 24 inch piling. Project consists of providing architectural treatment, <br />reinforcement of bulkheads, major utility adjustments and a critical M.OT due to condition of <br />existing structure. Project also includes approximately 400 linear feet of adjacent roadway requiring <br />gravity wells for drainage. <br /> <br />I <br /> <br />I <br /> <br />JEFFREY S.
