I Documentation & Accounting I o Through its experience working with local governments in response to natural disasters, Crowder has realized the importance of excellent documentation in obtaining full FEMA reimbursement for our client. In order to maximize the ability of our clients to receive reimbursement, we have implemented a multifaceted documentation system designed to track debris from initial pick up all the way through reduction and final disposal. Both Crowder and CITY OF SUNNY ISLES BEACH representatives will maintain a copy of the form for each truck. Crowder will create a database of the capacity of all of its trucks in employment on the project, and create a capacity log for both CITY OF SUNNY ISLES BEACH'S inspector at the TDSRS and for Crowder. An equipment log will also be created and given to CITY OF SUNNY ISLES BEACH and Crowder, listing all working equipment by a unique ID number and location. o TNdlo...._ . -: TNdt Ortww -. Rf:preHnI.civ.; .. ,- ""'..............-.../, '--(11-- ..:..::;,':f:.': In addition to certifying the capacity of each truck, Crowder also ensures proper documentation of its debris removal activities by requiring a load ticket to be filled out each time a Crowder or subcontractor truck hauls a load. This ticket describes each load that is hauled by Crowder, requiring such information as the type of load, where hauled from, where taken, truck number, etc. The load tickets are completed in duplicate, with CITY OF SUNNY ISLES BEACH and Crowder receiving copies. A daily log given to both CITY OF SUNNY ISLES BEACH and Crowder will certify work done by crews or by equipment. Each day, load tickets will be entered into a database by Crowder's diligent and detail oriented office staff. Once computed, the data will be transformed into reports. Crowder will provide daily reports to CITY OF SUNNY ISLES BEACH as required. Crowder is committed to working with CITY OF SUNNY ISLES BEACH and adhering to its documentation requirements. Using these two sources of information, Crowder ensures transparency in documentation and provides reconciliation between CITY OF SUNNY ISLES BEACH and itself. By distributing information between our clients and ourselves, Crowder aims to conduct a thorough and accurate accounting program. Crowder offers the services of its full-time accounting staff in order to achieve this goal. o All reports will be given to the reporting official's superior, with duplicates given to the Project Manager. Reports will be available to our client upon request. All of the reports will be reviewed in a weekly Quality 50 I P age
