<br />o <br /> <br />.~ .~ <br />...., TAG...., <br />GRINDING SERVICES, INC. <br /> <br />The attached are copies of the following: <br /> <br />1) TAILGATE FENCING DETAIL - Describes in detail the appropriate truck tailgate <br />requirements. <br />2) TRUCK CERTIFICATION FORM - This form is completed when truck is registered <br />and measured. Copies of this form are distributed to appropriate personnel with one <br />copy staying with the truck at all times. <br />3) TRUCK PLACARD - This large sticker is placed on both sides of the truck after it is <br />registered for easily identifying the truck and truck capacity. <br />4) DEBRIS LOAD TICKET - This ticket, in five copies, is completed and disbursed to <br />the appropriate personnel. This ticket lists load quantities and provides easily <br />accessible information. We are currently in development of a program that will <br />enable us to use "smart cards" to electronically store and access truck and load <br />information, as requested by FEMA. <br /> <br />I) <br /> <br />o <br />
