<br />o <br /> <br />10 <br /> <br />All disposal trucks shall be pre-measured and accepted by the government before being utilized <br />in debris removal operations. <br /> <br />MAINTENANCE/FUEL VEHICLES AND PERSONNEL - Maintenance/fuel vehicles will be <br />assigned and manned as needed to provide an adequate supply of fuel to maintain equipment <br />operations. Maintenance/fuel vehicles will be assigned and manned as needed to provide all <br />required field maintenance to ensure equipment operations. <br /> <br />SAFETY - All supervisors and/or foremen will utilize the check sheet provided by the Company <br />Safety Officer to ensure all safety equipment is maintained and operable on all debris disposal <br />hauling equipment and to ensure compliance with the Corporate Accident Prevention Plan as <br />part of the Corporate Safety Plan. <br /> <br />OPERA TIONS - All field supervisors shall ensure that all debris disposal-hauling operators are <br />licensed and/or certified to operate required equipment. All debris disposal operators will be <br />given area maps designating assignment/authorized areas of operations as well as transport <br />routes designated and/or approved by the Government. All debris disposal haul operators shall <br />visibly display colored signs provided by the Company and, if applicable, the Government. The <br />Company signs are magnetic signs to be placed on the driver and passenger doors of the vehicle <br />cab. Any signs provided by the government will be displayed on both sides of the forward most <br />section of the vehicle bed, unless otherwise directed by the government task order. All signs will <br />be removed from the exterior of the vehicle, at close of business each day and secured by the <br />driver to prevent theft or loss. Signs will be replaced on the vehicle at the beginning of the <br />workday. Colored paper signs/passes will be displayed in the driver's side windshield of each <br />vehicle. The color of the sign/pass is subject to change, without notice, to ensure quality control <br />measures regarding authority to exit work sites and enter disposal site(s). All debris disposal <br />haul operators shall maintain the numbered debris hauling/transportation documentation/ <br />verification form ORC-DM-12. Each form contains directions, which should be followed. All <br />supervisors will be responsible to ensure that all employees utilizing and/or inputting infonnation <br />on the form are procedurally trained. It will be each supervisor's responsibility to maintain a <br />supply of the required number of fonns. Forms will be distributed by supervisors/foremen to <br />debris disposal haul operators during loading operations and after completing Section 1 of form <br />DRC-OM-12. All debris disposal operators will maintain daily ticketlhaul records to be turned <br />into field supervisors, with copies ofload tickets at close of business each day. <br /> <br />DEBRIS STAGING SITE RESTORATION <br /> <br />Restoration is conducted during the closing of each TOSR site. The scope of remediation is <br />determined during operation and closure, by terms of the land lease (if any), or government <br />directive or task order. Remediation consists of final removal of all debris (including residual <br />debris), removal and remediation of HTW, abatement of any safety and/or environmental <br />concerns (to include environmental testing and/or monitoring, if required), the removal of <br />temporary structures (including any inspection towers), grading and leveling, removal of roads <br />and fencing, if appropriate, and potentially grassing or seeding of the site, if required, to <br />f' documented pre-use condition. <br />.., <br /> <br />38 <br />
