<br />o <br /> <br />substantially blocked or cribbed before personnel are permitted to work underneath or between <br />them. End-loader buckets shall be fully lowered or blocked when not in use. All controls shall <br />be in neutral positions with the engines stopped and brakes set, unless work being performed on <br />the machinery requires otherwise. Stationary machinery and equipment shall be placed on a firm <br />foundation and secured before being operated. All work areas in which heavy machinery is <br />being operated shall be illuminated. All vehicles which will be parked or moving slower than <br />normal traffic on haul roads shall have a yellow flashing light or four-way flashers from all <br />directions. Equipment shall be shut down prior to and during fueling operations. <br /> <br />Whenever equipment is parked, the parking brake shall be set. Equipment parked on an incline <br />shall have the wheels chocked or track mechanism blocked and the parking set. All equipment <br />left unattended at night, adjacent to a highway in normal use or adjacent to construction areas <br />where work is in progress, shall have lights or reflectors, or barricades equipped with lights or <br />reflectors, to identify the location of the equipment. <br /> <br />No modifications or additions which affect the capacity or safe operation of the machinery or <br />equipment shall be made without the manufacturers' written approval. If such modifications are <br />made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be <br />changed accordingly. In no case shall the original safety factor of the equipment be reduced. <br />Steering or spinner knobs shall not be attached to the steering wheel. All industrial trucks shall <br />meet the requirements of design, construction, stability, inspection, testing, maintenance, and <br />operation, defined in ANSI/ASME B56.1 Safety Standards for Low Lift and High Lift Trucks. <br /> <br />C The controls of loaders, excavators, or similar equipment with folding booms or lift arms shall <br />not be operated from a ground position unless so designed. Personnel shall not work or pass <br />under or ride in the buckets or booms or loaders in operation. <br /> <br />All machinery and construction equipment will be equipped with a reverse signal alarm. <br />Reverse signal alanns shall be audible and sufficiently distinct to be heard under prevailing <br />conditions. Reverse signal alarms shall in addition to requirements for signal persons. <br /> <br />All belts, gears, shafts, pulleys, sprockets, spindles, drums, flywheels, chains, or other <br />reciprocating, rotating, or moving parts of equipment shall be guarded when exposed to contact <br />by persons or when they otherwise create a hazard. All hot surfaces of equipment, including <br />exhausts pipes or other lines, shall be guarded or insulated to prevent injury and fire. Substantial <br />overhead protection shall be provided for operators of construction equipment in the form of <br />FOPs and lor Raps. <br /> <br />() <br /> <br />Motor Vehicles <br />Every person operating a motor vehicle shall possess, at all times while operating such a vehicle, <br />a permit valid for the equipment being operated. No vehicle shall be placed in service until it has <br />been inspected on a scheduled maintenance program. Weekly inspections include the following. <br />.
