<br />· General construction work <br />· Possible preparation of complete as-built drawings <br />· Possible public involvement <br /> <br />The City contemplates the construction firm team to include (but not be limited to) the minimum <br />following disciplines. <br /> <br />· Structural Concrete Specialists <br />· Parking Facility Specialists <br />· Traffic Management Specialists <br />· Landscape Professionals <br />· Park Development Specialists <br /> <br />The intent is to have the contractors provide a viable holistic approach to the construction and <br />transition process. The final product must be economically feasible, it must have community and <br />business buy-in, and it must be aesthetically pleasing. The estimated construction budget is <br />$12,5 million dollars, <br /> <br />1,7 SUBMITTAL REOUIREl\1ENTS <br /> <br />All submittals by contractors shall contain no more than fifteen (15) pages specifically addressing <br />the following issues. All other required documentation (except for 5 and 6 below) will not be <br />considered a part of the fifteen (15) pages. Any contractor wishing to provide the services <br />described in Section 1,6 above must submit one (1) original and four (4) additional complete <br />copies that include, at a minimum, the following documentation: <br /> <br />1. Name, address and company, including, but not limited to, a business overview, financial <br />state of the business, annual revenue for past two years, and names and addresses of <br />persons having financial interest in the firm. <br /> <br />2, A list of all projects (including government projects) completed or in process for the past <br />five (5) years, and a corresponding list of client references with contact names and <br />telephone numbers. <br /> <br />3, Proof of authorization to transact business in Florida from the Florida Secretary of State, <br />from the prime as well as supporting firms. <br /> <br />4, The name, title, qualifications, experience, and all contact information of the proposed <br />contact person. <br /> <br />5, A listing of the proposed personnel to be assigned to the City's various potential projects, <br />including their qualifications and experience. <br /> <br />6, Sworn statement pursuant to Section 287. 133(3)(A), Florida Statutes, on Public Entity <br />Crime, a copy of which is attached hereto. <br /> <br />8 <br />
