Trash Receptacles: <br /> <br />Each <br />Each <br />Each <br />Each <br />Each <br />Each <br />Each <br />Each <br />Each <br />Each <br /> <br />$ <br />$ <br />$ <br />$ <br />$ <br />$ <br />$ <br />$ <br />$ <br />$ <br /> <br />The following documents are attached to and made a condition of this Bid: <br /> <br />a) Required Bid Security in form of AlA Document A310 Bid Bond (5%) <br /> <br />b) Required Contractor's Qualification Statement with supporting data per AlA A305 <br /> <br />The stipulated sum bid items include, but are not limited to: <br /> <br />c) All payments to be made by Contractor to its independent contractors, subcontractors, <br />suppliers, materialmen and employees; <br /> <br />d) The costs and expense of application for obtaining all necessary governmental permits, <br />licenses, insurances, and bonds which may be required in connection with the performance of <br />the Work; <br /> <br />e) All federal, state, county, municipal, and other taxes imposed by law and based upon labor, <br />services, materials, equipment, or other items acquired, performed, furnished or used for or <br />in connection with the Work, including but not limited to sales, use and personal property <br />taxes payable by or levied or assessed against the Owner, Contractor or Subcontractors. <br />Whether the law requires any such taxes to be stated and charged separately, the total price <br />of all items included in the Work plus the amount of such taxes shall not exceed the <br />stipulated lump sum bid items. <br /> <br />Before commencing with any work, the Bidder shall comply with all requirements of insurance and bonds <br />as defined in the contract documents. Delays in complying with the requirements will not result in an <br />extension of the completion date. <br /> <br />Failure to satisfy the Bond and Insurance provisions of the Contract Documents are deemed a material <br />breech of this contract. <br /> <br />BID FORM-Page 6 <br />
