<br />If the empployee is already eligible for health benefits through the RHS plan, his/her account will not be consid- <br />ered de minimis regardless of the balance. Likewise, if the employee dies or is disabled, no de minimis payment <br />will be made. <br /> <br />If you wish to name a lower de minimis account value, or if you do not wish to include a de minimis feature <br />from our RHS program, check the appropriate box in Section XII. <br /> <br />o The de minimis account value shall be $5,000 or less. <br /> <br />o The de minimis account value shall be $ <br />less. <br /> <br />(insert dollar amount between $0 and $5,000) or <br /> <br />o The Plan shall not allow de minimis account distributions. <br /> <br />XIII.
