Any questions regarding these <br />features of the RHS program can be directed to your ICMA-RC Retirement Plans Specialist. <br /> <br />SAMPLE WELFARE BENEFIT PLAN <br /> <br />You will need to execute a welfare benefit plan if you do not already have one in place. You may execute <br />the sample welfare benefit plan provided herein or you may execute your own welfare benefit plan. It can <br />be a simple document, but it must be in writing in order for your employees to enjoy tax-free treatment of <br />the benefits they receive from this plan or any other welfare benefit plan you provide. <br /> <br />FOR ASSISTANCE <br /> <br />Please contact your VantageCare Retirement Health Savings Plan New Business Analyst at 1-800-326-7272. <br /> <br />Please note that the information in this booklet and the documents herein take into account only the <br />federal tax rules related to ICMA-RC's VantageCare Retirement Health Savings Plan. Prior to implementing <br />an RHS plan. the employer is responsible for determining that there are no state or local laws that would <br />prohibit it from offering the plan to its employees. The employer must also determine that the options it <br />selects in the VantageCare Retirement Health Savings Plan Adoption Agreement fall within statellocal <br />requirements. <br /> <br />2 <br />
