IIIJ I L DIN f, f, Il 0 IJ I) Executive Summary Project Overview Ocean Building Group, Inc. is please to provide this proposal to the city of Sunny Isles for the demolition of structures locat- ed at 151 and 215 Sunny Isles Boulevard and 16050 and 16100 Collins Avenue, Sunny Isles Beach Florida 33160. This proposal is a culmination of a comprehensive cost evaluation to the city of Sunny Isles for the demolition of the aforemen- tioned sites. The study included extensive site examination and analysis of historical building data. To the greatest extent feasible, we have attempted to respond to the city's desire for a competitive proposal for the demoli- tion of structures located at 151 and 215 Sunny Isles Boulevard and 16050 and 16100 Collins Avenue, Sunny Isles Beach Florida. Our proposal and implementation of this project encompasses all obligatory documentation, property surveys and plans, acquiring all permits required by the City and County agencies and any additional information necessary for the suc- cessful demolition of the existing structures described in this proposal. The implementation and completion of this project is proposed at $146.250 for the demolition of the existing structures described in this proposal. Proposal cost details and itemized costs per site are furnished in section 6 (Price Proposal). This proposal is categorized into eight (8) main sections: Section 1- This section will consist of all Company Information to include: Name of Agency/Company Company Locations Internet Web Site Address Details of Entity Business Structure Date Founded Home office address and telephone number, and local address and phone List of any outstanding litigation Proof of insurance Indication of how long it would take to implement service after authorized to begin Section 2- This section will illustrate or qualifications, relevant experience, and past performance to include the following: An explanation of why Ocean Building Group is the best qualified to perform the contract by solidifying its qualifica- tions including an item-by-item disclosure outlining how our firm meets or exceeds the requirements of this RFP. A schedule of proposed services with a clear understanding of the issues and tasks of the project at hand. Section 3-Staffing will consist of a list of experience and qualified key personnel, including key personnel of subcontractors that will be assigned to this project and experience and qualifications of subcontractors Section 4- Approach and Methodology to the proposed demolition work. Section -5 Price Proposal illustrating cost details associated with the services described in this RFP and a itemized price breakdown. Section 6 - References furnished for three (3) Current and Past Customers of which we have provided services similar in scope and size of those described in the RFP. Each Reference is furnished on the Clients letterhead, signed by t he specified contact. Section 7- Schedule Provide a schedule and work plan to complete the Services within the 60 (sixty) day completion dead line. Section 8- Contract Forms: All completed contract forms
