<br />. The composition of the staff team should include. <br />. The names of the employees in the area responsible for this contract <br />. Their function in the company. <br />. The name of the person who will be responsible for the coordination of work. <br /> <br />4. Approach / Methodology <br />Proposers approach methodology to providing the services requested in this solicitation <br /> <br />. Suitability of the methodologies and approaches used in achieving tasks <br />· Overall organization to completing the project <br />· Ability to meet desired timelines and deadlines <br /> <br />5. Price Proposal <br />. Each firm shall submit in their proposal a detailed cost proposal which shall <br />include all costs associated with the services described in this RFP. <br /> <br />6. References <br />Each Proposer must submit a list of three (3) references of Current and Past Customers <br />of which they have provided services similar in scope and size of those described herein. <br />. Each Reference must be supplied on Clients Letterhead <br />. Each Reference letter must me signed with contact person and phone number. <br /> <br />The City retains the right to request any additional information pertaining to the <br />Proposer's ability, qualifications, and procedures used to accomplish all work under the <br />contract as it deems necessary to ensure safe and satisfactory work. <br /> <br />7. Project Information: <br /> <br />. Provide a schedule and work plan to complete the Services requested as part of <br />this RFP. <br />. Provide a rendering of the proposed design. <br />Proposer MUST submit a sketch or simple rendering of what is proposed. A <br />listing of all the beneficial elements related to the proposal must be clearly <br />defined for consideration. Failure to provide sufficient data, information, <br />descriptions, and/or material may result in proposal rejection. Once an <br />agreement has been executed with the successful proposer, such rendering, <br />submitted at the time of the Proposal, shall be modified and revised as necessary <br />to reflect the desires of the City. Such modifications shall be provided by the <br />Proposer at no extra cost to the City. <br />. Provide sufficient information on the type of material proposed and the type of <br />connections. <br />. Provide the type of services from third party vendors that will be sought to fulfill <br />the terms of the RFP. <br />. LIST of ALL Sub vendors, Sub consultants, material men, and suppliers that may <br />be used by the Proposer MUST be provided for the City's approval or rejection at <br />the time of PROPOSAL SUBMISSION. The Proposer shall hold prices as stated in <br />the proposal regardless of any requested modification provided by the City. <br /> <br />8.
