Morphine Drug or Metabolites <br /> . 6-AM (Heroin) <br />~ Phencyclidine (PCP) <br /> <br />The use and consumption of the five illegal drugs is prohibited at all times and employees can <br />be tested for drugs anytime while on duty. <br /> <br />A covered employee that has a confirmed positive drug test result on a required test will be <br />relieved from duty immediately. A covered employee that refuses a drug test will be removed <br />from duty immediately. (See Employer Action on Positive Test Result and Refusal to Test.) <br /> <br />If a drug test is canceled on a pre-employment, return to duty, or follow-up test, the employee or <br />applicant must re-take the drug test and produce a negative drug test result. <br /> <br />An immediate observed collection will occur when an employee produces a sample out of the <br />temperature range, shows evidence of tampering/adulterant, and will be required on all return to <br />duty and follow up tests. <br /> <br />See Prohibited Drug and Alcohol Testing Procedures section for new collection procedures. <br /> <br />The City shall require an immediate retest upon being notified by the MRO that an employee's <br />test result is negative dilute. This policy applies to all required tests. (See required tests.) <br /> <br />Required Tests <br /> <br />Pre-Employment Testing <br /> <br />Requires: <br /> <br />~ DOT drug and alcohol tests. <br />~ Request For Drug and Alcohol Information From Previous Employer Form. <br /> <br />All applicants must produce a negative drug test and alcohol test result prior to being hired. If a <br />test is cancelled by the MRO, the prospective employee must retake the test as soon as <br />applicable. <br /> <br />An employee may not transfer from a non-safety sensitive position to a safety-sensitive position <br />until they take a pre-employment drug and alcohol test with verified negative results. <br /> <br />SIB _Drug-Free_ Workplace_Program_-_ W-CDL <br /> <br />Page 27 of 41 <br /> <br />12/15/2011 <br />
