RESOLUTION NO. 2012 - I °C b 7 <br />A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF <br />SUNNY ISLES BEACH, FLORIDA, APPROVING THE FIRST <br />AMENDMENT TO AGREEMENT WITH A & A DRAINAGE & VAC <br />SERVICES, INC. FOR STORM WATER CATCH BASIN CLEANING <br />AND LINE JETTING SERVICES, IN AN AMOUNT NOT TO <br />EXCEED FIFTY -EIGHT THOUSAND TWO HUNDRED SIXTY <br />DOLLARS ($58,260.00), ATTACHED HERETO AS EXHIBIT "A "; <br />AUTHORIZING THE MAYOR TO EXECUTE SAID AGREEMENT; <br />AUTHORIZING THE CITY MANAGER TO DO ALL THINGS <br />NECESSARY TO EFFECTUATE THIS RESOLUION; PROVIDING <br />FOR AN EFFECTIVE DATE. <br />WHEREAS, the City of Sunny Isles Beach was in need of cleaning its storm water drains <br />and catch basins and jetting the lines throughout the City to prepare for hurricane season, and to meet <br />State Department of Environmental Protection (DEP) requirements, and issued Request for <br />Proposals (RFP) No. 11- 04 -03; and <br />WHEREAS, on June 15, 2011 via Resolution No. 2011 -1726, the City Commission awarded <br />RFP No. 11 -04 -03 and entered into an agreement with A & A Drainage & Vac Services, Inc., to clean <br />448 stormwater catch basins and line jetting services , in an amount not to exceed Forty Thousand <br />Three Hundred Twenty Dollars ($40,320.00), with option to renew the agreement for up to four (4) <br />consecutive one (1) year renewal terms; and <br />WHEREAS, the City being satisfied with the work completed by A & A Drainage & Vac <br />Services, Inc., wishes to approve the First Amendment to Agreement to clean 539 stormwater catch <br />basins and line jetting services at a cost of $48,510.00, and additionally adding video services for <br />2,600 feet of storm drain line to determine if the pipe is suffering from any damage at a cost of <br />$9,750.00, in a total amount not to exceed Fifty -Eight Thousand Two Hundred Sixty Dollars <br />($58,260.00), attached hereto as Exhibit "A ". <br />NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE <br />CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS: <br />Section 1. Approval of First Amendment to Agreement. The City Commission hereby approves <br />the First Amendment to Agreement with A & A Drainage & Vac Services, Inc. for storm water catch <br />basin cleaning, line jetting, and video services, in an amount not to exceed Fifty -Eight Thousand <br />Two Hundred Sixty Dollars ($58,260.00), attached hereto as Exhibit "A ". <br />Section 2. Authorization of Mayor. The Mayor is hereby authorized to execute said Agreement. <br />Section 3. Authorization of City Manager.
