�NNYi .; • Li, 4 o FIRST AMENDMENT TO THE AGREEMENT BETWEEN THE CITY OF SUNNY ISLES BEACH AND A&A DRAINAGE AND VAC SERVICES, INC. CONTRACT NO. C1112-033 This First Amendment to the Agreement between the City of Sunny Isles Beach and A &A Drainage And Vac Services, Inc. executed this el day of , 2012, is made a part of the original Agreement, C1011- 053, approved by Resolution No. 2011-1726 ("Agre ment"), between the parties dated June 15, 2011 incorporated herein,between the City of Sunny Isles Beach ("City") and A&A Drainage And Vac Services, Inc, ("Contractor") a business corporation licensed in the State of Florida, whose Federal Identification # is (,5-Oclg 1 530. The City and Contractor hereby agree as follows: 1. ADDITIONAL SERVICES TO BE PERFORMED. The scope of services of the Agreement is hereby amended to include the cleaning of 539 storm water catch basins and line jetting services and adding video services for 2,600 feet of storm drain line to determine if the pipe is suffering from any damage. 2. COST. The payments to the Contractor, as set forth in Section 3 of the original Agreement, is hereby amended to include compensation for the additional services to be performed, in an amount not to exceed Forty Eight Thousand Five Hundred Ten Dollars($48,510.00)for the cleaning of the 539 storm water catch basins and an additional amount not to exceed Nine Thousand Seven Hundred Fifty Dollars ($9,750.00) for the video services of 2,600 feet of storm drain line, for a total amount not to exceed Fifty Eight Thousand Two Hundred Sixty Dollars ($58,260.00). 3. OPTION TO RENEWAL: The City hereby exercises its first optional one (1) year renewal term, as provided in Section 2 of the original Agreement. 4. OTHER PROVISIONS REMAIN IN EFFECT: Except as specifically modified herein, all terms and conditions of the original Agreement between the parties, dated June 15, 2011, shall remain in full force and effect.
