The bidder's quoted price <br /> must include all labor cost and total cost of providing services including, but not limited to, the <br /> following: overhead, set-up and tear down, profit, travel time, service charges, gratuity, delivery, <br /> potable water, fuel, propane, generators, food products and transportation, beverages, ice, all <br /> necessary catering equipment including refrigeration, warming units, serving items, carry out food <br /> containers, utensils, napkins, and fringe benefits. The awarded proposer agrees to keep this offer <br /> open for acceptance for One Hundred Twenty Days (120) after date of opening the proposals. <br /> 2.7 PERFORMANCE BOND <br /> Intentionally Omitted <br /> 2.8 DELIVERY <br /> F.O.B. Destination. Town Center Park 17200 Collins Ave, Sunny Isles Beach, FL 33160, including <br /> but not limited to any other designated park within City limits. <br /> 2.9 INSURANCE <br /> Upon City's notification, the Firm shall furnish to the City Manager or his designee, <br /> Certificates of Insurance that indicate that insurance coverage has been obtained, which meet the <br /> requirements as outlined below: <br /> 2.9.1 Comprehensive General Liability Insurance <br /> Contractor shall be required to purchase, maintain, and keep in full force, effect, and good <br /> standing, Comprehensive General Liability with primary limits of One Million Dollars <br /> ($1,000,000) per occurrence and Two Million Dollars ($2,000,000) aggregate during the <br /> initial and any renewal term of this Agreement. Coverage must be afforded on a form no <br /> more restrictive than the latest edition of the Comprehensive General Liability policy, <br /> without restrictive endorsements, as filed by the Insurance Services Office, and must <br /> include: <br /> • Premises and Ongoing Completed Operations—on a primary and non-contributory <br /> basis including waiver of subrogation on behalf of the City of Sunny Isles Beach. <br /> • Independent Contractors <br /> • Broad Form Property Damage <br /> • Broad Form Contractual Coverage applicable to this specific Contract, including <br /> any hold <br /> • Harmless and/or indemnification agreement. <br /> • Personal Injury Coverage with Employee and Contractual Exclusions removed,with <br /> minimum limits of coverage equal to those required for Bodily Injury Liability and <br /> Property Damage Liability. <br /> City of Sunny Isles Beach I Request for Proposal No. 17-02-01 11 <br />
