C1112-068 <br /> This First Amendment to the Agreemijnt between the City of Sunny Isles Beach and Calvin, <br /> Giordano & Associates, Inc., executed this lb day of Otto Bt - , 2012, is made a part of the original <br /> Agreement between the parties dated December 15, 2011 attached hereto as Attachment "B" and <br /> incorporated herein by reference, (the "Agreement") between the City of Sunny Isles Beach ("City") and <br /> Calvin, Giordano & Associates, Inc., ("Consultant") a business corporation licensed in the State of <br /> Florida, whose Federal Identification # is 650013869. The City and Consultant hereby agree as follows: <br /> 1. ADDITIONAL SERVICES TO BE PERFORMED. Section 2 of the Agreement between the <br /> parties is hereby amended to include additional services to be performed by the Consultant related to the <br /> North Bay Road bridge project, to include 1) professional engineering services, and 2) professional <br /> government consulting services, as more thoroughly described in Attachment "A". <br /> 2. COSTS. The total compensation to the Consultant, as set forth in Section 4 of the Agreement, is <br /> hereby amended to include additional compensation in an amount not to exceed I) Twenty Four <br /> Thousand Nine Hundred Ninety Dollars ($24,990.00) for professional engineering services, and 2) Nine <br /> Thousand Six Hundred Fifty Four Dollars and Seventy Five Cents ($9,654.75) for professional <br /> government consulting services, bringing the total amount not to exceed Thirty Four Thousand Six <br /> Hundred Forty Four Dollars and Seventy Five Cents ($34,644.75). <br /> 3.
