SV,iNi,J,Fj <br /> City of Sunny Isles Beach <br /> i;;., Personnel Policy and Procedure <br /> F. <br /> Policy Name WORKERS' COMPENSATION LEAVE <br /> Policy Number 703:2 (NEW) <br /> Effective Date 5/18/17 <br /> Revision Date(s) <br /> Prepared By (Title) Human Resources Director <br /> Approved By (Title) City Manager <br /> POLICY: <br /> It is the policy of the City of Sunny Isles Beach to ensure the availability of all reasonable <br /> and necessary authorized medical care and payment of all related medical costs to <br /> employees who are injured or develop an occupational disease as a result of the <br /> conditions of their job. Additionally, employees may be entitled to receive partial <br /> compensation for lost wages due to a qualified job-related injury or illness. This policy is <br /> in accordance with Chapter 440 of the Florida Statues. Where provisions of this policy <br /> conflict with a collective bargaining agreement, the collective bargaining agreement shall <br /> prevail. <br /> PURPOSE: This policy sets forth guidelines and procedures to safely reintegrate <br /> employees who sustain a work-related injury or illness to full-duty. <br /> PROCEDURES: <br /> A. General <br /> 1. Employees who sustain an on-the-job work-related injury or illness related to work <br /> should immediately inform their supervisor. Failure to notify the supervisor <br /> immediately may affect benefits under the Workers' Compensation Law and may <br /> result in discipline up to and including termination. <br /> 2. The supervisor is responsible for reporting all Workers' Compensation injuries to <br /> the Human Resources Department within 24 hours of the injury. <br /> 3. In non-emergency situations, prior to medical attention being sought for any work- <br /> related injury or illness, employees shall consult with the Human Resources <br /> Department for treatment or referral to an authorized health care provider. Failure <br /> to follow this procedure may jeopardize the employee's benefits under the <br /> Workers' Compensation Law. This provision does not apply to life-threatening <br /> emergencies when immediate care must be sought through "9-1-1" dispatch. <br /> However, the Human Resources Department must be informed as soon as <br /> possible of the injury. <br /> Policy 703:2—Workers' Compensation Leve Page 1 of 4 <br /> 100 <br />
