A) 1 indicates ………the base contract for the <br />lump sum prices with schedule of values which includes the entire project except <br />for the following items: Install asphalt pavement, sidewalks and stormwater <br />drainage. These items are included in the schedule of values. Please clarify this <br />is confusing. <br /> <br />A6: Please see attached revised Table 1, MAJOR ITEM SCHEDULE OF <br />VALUES. <br /> <br /> <br />Q7: Please clarify the requirements for vibration monitoring for this project. <br /> <br />A7: The purpose of the vibration monitoring is to protect the contractor from claims <br />of inadvertent damage to nearby private properties resulting from construction <br />activities. The extent of monitoring during appropriate phases or elements of <br />construction is to be determined by the contractor and shall conform to FDOT <br />requirements. See Note 10 on Sheet B-11 of the Bridge Drawings. <br /> <br /> <br />Q8: Will the contractor be required to supply a Builders Risk insurance policy? <br /> <br />A8: Please refer to RFP Section 2.10 Insurance. <br /> <br /> <br />Q9. Section 6, Bid Submittal Forms, page 22 of the RFP, refers to an excel form <br />to be inserted, we have not received this form, when will it be issued? <br /> <br />A9: See attached Table 1 MAJOR ITEM SCHEDULE OF VALUES. <br /> <br /> <br />Q10: Does the concrete for the structure have to be FDOT certified? <br /> <br />A10: Yes. <br /> <br /> <br />Q11: There is no requirement for an Engineer’s Field Office or Trailer, please <br />confirm that we are not to provide one as part of our bid price. <br /> <br />A11: The contractor will be allowed to occupy a portion of the North Bay Road right <br />of way from Station 26+00 south to the canal, outside the limits of the 174th St <br />drainage improvements referenced in Q2). This area can be used for staging, <br />storage of non-hazardous materials, and for a required construction trailer to be <br />used for maintaining record drawings and documentation as well as hosting <br />periodic site meetings during construction.
