Contractor shall closely evaluate the City’s minimum <br />manpower requirements defined in the Performance Standards and make recommendation if <br />minimum specified manpower is sufficient and, if not, how many personnel do they recommend. <br />The Contractor’s recommendation will be taken into consideration in Bid evaluation. <br /> <br />G. Financial Status <br /> <br />1. Attach a professionally prepared financial statement with the City’s ability to audit upon request, <br />including Contractor’s latest balance sheet and income statement showing the following items: <br />a. Current assets, i.e. cash, joint venture account, accounts receivable, notes receivable, <br />accrued income, deposits, materials inventory, and prepaid expenses. <br />b. Net fixed assets. <br />c. Other assets. <br />d. Current liabilities, i.e. accounts payable, notes payable, accrued expenses, provision for <br />income taxes, advances, accrued salaries, and accrued payroll taxes, <br />e. Other liabilities, i.e., capital, capital stock, authorized and outstanding shares per values, <br />earned surplus, and retained earnings, and <br />f. Name of firm preparing financial statement and date thereof <br /> <br />2. Bankruptcies: <br />a. Has the Contractor, or any of its parents or subsidiaries, ever had a Bankruptcy <br />Petition filed in its name, voluntarily or involuntarily? (If yes, specify date, <br />circumstances, and resolution). <br /> <br /> <br /> <br /> <br /> <br /> b. Has any Majority Shareholder ever had a Bankruptcy Petition filed in his/her <br />name, voluntarily or involuntarily? (If yes, specify date, circumstances, and <br />resolution). <br /> <br /> <br /> <br /> <br /> <br />c. Loans: Is this Contractor in default on any loan agreement or financing <br />agreement with any bank, financial institution, or other entity?
