<br />City of Sunny Isles Beach |Request for Qualifications No. 17-12-01 31 <br /> <br />requirements for the services. Concentrate only on those services rendered by the Firm within the <br />past five (5) years, which mostly match the scope of services. <br /> <br />Please indicate which team members were part of the referenced service and will be a part of the <br />City’s team. Indicate whether the experience was with the current Firm or a past employer. In all <br />illustrations of experience, indicate specifically the nature and extent of the work performed by the <br />individuals or firms on prior similar services. The title and a brief description of each project and/or <br />service shall include: <br /> <br />a. Client Name, Address, Contact, Phone and valid e-mail address; <br />b. Project Title; <br />c. Description of work performed, including the project location; and <br />d. Year completed. <br />e. Past 5 years copies of executed agreements made between your firm and local <br />municipalities, with a scope similar to this RFQ. <br />f. Please explain, have you ever defaulted to complete any work awarded to you? If so, note <br />when, where and why. <br /> <br />If sub-consultants are a part of the proposed work plan, include items A through E for each firm, <br />including an organizational chart showing the relationship between the responding firm and the <br />sub firm(s). <br /> <br />F. Corporate Standing and Authorized Signatory <br />Respondent must demonstrate that the company is in good standing and that the person signing <br />this submittal is an Authorized Signatory on behalf of the respondent to sign bids, proposals, <br />negotiate and/or sign contracts, agreements, amendments and related documents to which the <br />respondent will be duly bound. The respondent must provide a copy of the State Certificate of good <br />standing listing the officers of the company. If the signatory is not one of the officers listed on the <br />State Certificate, the respondent must provide one of the following forms of evidence of Signatory <br />Authority with its response: <br /> <br />a. A copy of firm’s Articles of Incorporation listing the approved signatories of the corporation; <br />b. A copy of a resolution listing the members of staff as authorized signatories for the firm; <br />and <br />c. A letter from a corporate officer listing the members of staff that are authorized signatories <br />for the firm. <br /> <br /> G. Litigation History <br /> <br />Consultant shall provide a summary of any litigation or arbitration that the CONSULTANT, its parent <br />company or its subsidiaries have been engaged in or are currently engaged in, during the past five <br />(5) years against or involving (1) any public entity in Florida for any amount (2) any private entity <br />for an amount greater than $100,000. The summary shall state the n ature of the litigation or <br />arbitration, a brief description of the case, the outcome or projected outcome, and the monetary <br />amounts involved. The CITY may disqualify any CONSULTANT it determines to be excessively <br />litigious.
