Actions taken to mitigate future occurrences of the deficiencies <br /> Contractor must provide Project Manager(s) with a hard copy and/or electronic copies of <br /> all forms and documents prepared as part of the Quality Control Plan monitoring. <br /> 2.20.2 FIELD SUPERVISOR <br /> The Contractor must provide 1 qualified on-site full-time working Field Supervisor_to <br /> manage Contractor's personnel at the Work sites. One (1) Field Supervisor shall be <br /> assigned to the Public Works Department. These people (and their substitutes) must have <br /> full authority to act for the Contractor on all matters relating to the daily performance of <br /> the Work at the Work site(s). The Field Supervisor must be the central point of contact in <br /> the field for the City; and, must effectively communicate in English. The Field Supervisor <br /> must understand and be able to fulfill, completely and clearly, the Performance Standards <br /> and Technical Specifications and reporting requirements of the Contract. A resume of the <br /> Field Supervisor must be submitted with the Contractor's Qualifications as specified in <br /> Section 7, which must include all contact information (i.e. telephone, email address, cell <br /> phone, etc.). The field supervisor must have a minimum of five (5) years - combined <br /> horticultural education and experience as a landscape field maintenance supervisor on <br /> contracts of similar size, scope, and complexity, and must remain on-site at all times while <br /> Work is being performed under the Contract. When the on-site working Field Supervisor <br /> is absent for the.day or for an extended period(more than 4 hours),the Project Manager(s) <br /> must be notified and the Contractor must appoint a qualified substitute. Within five (5) <br /> days of execution of the Contract,the Contractor must provide the Project Manager(s)with <br /> a cellular phone number for the Field Supervisor where he/she can be reached at all times. <br /> 2.20.3 The Contractor must provide a minimum of six (6) full time staff, one (1) on-site Field <br /> Supervisor Monday— Friday and two (2) fulltime staff during weekends with 1 one onsite <br /> Field Supervisor (or additional staff as needed to meet scope of service, as determined by <br /> the City). All Work shall be accomplished anytime between the hours of 7:00 A.M. to 5:00 <br /> P.M. Monday through Sunday. Weekend schedule should include no less than 2 full time <br /> staff and one field supervisor for debris and litter patrol in all City ROW's. Collins Avenue <br /> and Sunny Isles Blvd. shall have debris and litter removed twice daily as per section A2. <br /> The minimum required staff is for routine landscape maintenance services only and does <br /> not include services such as annual flower change-outs, shrub replacement, special <br /> projects, irrigation repair,tree and palm pruning.
