QIUFIHI Eer rises, Inc. <br /> p <br /> DISASTER RESPONSE DIVISION <br /> • A photograph of the Measured Truck w/driver will either be, taken by Polaroid <br /> Camera and attached to the Certification Sheet; or by Digital and Stored. <br /> • Each Truck Dump Bed will be assigned the required TFR issued side signs on each <br /> dump body, all to be weather durable, tamperproof and non-removable and will <br /> include (in large and clear writing): <br /> o Company Name <br /> o Truck Number <br /> o Maximum Volume in Cubic Yards <br /> o Inspector's Name and Date <br /> o These signs will be placed, one each on each side of the dump bed body and <br /> be maintained throughout the duration of the project to ensure readability. <br /> • Additional Truck dump trailers (pup trailers) will each have a separate truck measure <br /> certification including all of the.information outlined above along with a notation as <br /> to the truck that it is coupled with. <br /> • The truck certification will include: • <br /> o Inspector's Name and Date <br /> Before equipment is dispatched to the loading sites, it will have already undergone all the <br /> necessary safety inspection, measurement and hauling procedures at the staging area as <br /> outlined in the section above. All loading and hauling crews will have received a copy of the <br /> scope of work, accident prevention plan, safety indoctrination, and assigned a crew foreman. <br /> All crews and foremen will be instructed by the Project Manager that they are to work in <br /> ® areas designated by a client Debris Manager and are not to relocate or move from one area <br /> to another without prior approval of the Operations Manager. No employees, or <br /> subcontractors of TFR will be allowed to work for private or other public entities while <br /> employed or contracted under this project. <br /> Hazardous Tree Removal <br /> Unstable and leaning trees along a public ROW or within a naturalized area, such as public <br /> parks or golf courses, may be eligible for removal. The Sub-Grantee may choose to attempt <br /> to save the tree through straightening and bracing, if the cost of repair is less than the <br /> removal and disposal. A tree is deemed hazardous and eligible for removal if: <br /> • The tree is an immediate threat to public health and safety or improved property <br /> • It has a DBH of 6" or greater when measured 4.5 feet above ground level <br /> AND one or more of these criteria: <br /> • 50% or more of the crown is damaged or destroyed <br /> • A split trunk or broken branches that expose the heart wood <br /> • Fallen, leaning or uprooted within a public use area <br /> • Leaning at an angle greater then 30 degrees <br /> After a tree has been deemed eligible and scheduled for removal, TFR tree crews will <br /> discuss a tree specific removal plan to ensure a safe, proper felling operation, considering: <br /> ® • Surrounding area for anything that may cause trouble when the tree falls <br /> • The shape of the tree, the lean of the tree and decayed or weak spots <br /> 37 <br />
