BRIEF DESCRIPTION OF PROJECT AND RELEVANCE TO THIS CONTRACT (Include scope, size, and cost) <br />Craven Thompson and Associates was contracted with the City of Lake Worth to design, permit and provide <br />construction services for Projects 1, 3, 5, 6 & 8 of Year 1 of the Lake Worth 2020 Master Plan. <br />Project 1 - North M Street and North K Street from 2nd Avenue North to 6th <br />Avenue North included new watermain and hydrants. North K Street from <br />2nd Avenue North to 7th Avenue North was in poor condition and required <br />roadway and sidewalk reconstruction. A small segment at the southern end <br />of each road fell within Problem Area 8 of the City’s “Stormwater Master <br />Plan”, and therefore the storm sewer was upgraded. <br />Project 3 - North F Street from 5th Avenue North to 10th Avenue North <br />involved the construction of both storm sewer and watermain. No pavement <br />was identified with a PCI at or below 55 and therefore road reconstruction <br />was necessary; however, utility trenching necessitated pavement <br />replacement in trench areas and asphalt overlay. <br />Project 5 - North M Street from 9th Avenue North to 18th Avenue North <br />involved the reconstruction of roadway. The existing section of road was <br />identified as in a failed condition with a PCI less than or equal to 55. The <br />entire road and base was reconstructed. The majority of the sidewalk also <br />required reconstruction. The existing storm sewer pipe and structures from <br />16th Avenue North to 18th Avenue North was evaluated as to the condition <br />of the materials. From 9th Avenue North to 16th Avenue North storm sewer <br />was added. <br />Project 6 - on 11th Avenue North from Dixie Highway to North M Street <br />involved the reconstruction of roadway and storm sewer. Between North M <br />Street and North L Street the existing road was rated as being in a failed or <br />poor condition with a PCI of less than or equal to 55. In these areas the <br />entire road and base was reconstructed. The structural condition of the <br />existing storm sewer pipe and structures along the roadway were evaluated. Piping and/or structures identified as in <br />poor structural condition were replaced. <br />Project 8 - North J Street from 13th Avenue North to 16th Avenue North, and 15th Avenue North from North J Street to <br />the railroad tracks, and 14th Avenue North from North J Street to Dixie Highway was identified as a part of Problem <br />Area 3 in the “Stormwater Master Plan”. This project included the installation of 700 linear feet of 24-inch diameter <br />exfiltration trench with a nutrient separating baffle box at the end of the trench, the replacement of 950 linear feet of <br />existing 18 and 24-inch diameter storm sewer with 30-inch diameter RCP storm sewer, and the replacement of 70 <br />linear feet of existing 12-inch diameter storm sewer with 24-inch diameter RCP storm sewer. <br /> <br />25.
