A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF SUNNY
ISLES BEACH, FLORIDA, APPROVING A SIXTH AMENDMENT TO
THE AGREEMENT WITH PERKINS+WILL ARCHITECTS, INC. FOR
ARCHITECTURAL DESIGN SERVICES FOR THE PEDESTRIAN
OVERPASS BRIDGE AT 18080 COLLINS AVENUE, IN AN AMOUNT
NOT TO EXCEED FIFTEEN THOUSAND SEVEN HUNDRED EIGHTY-
TWO DOLLARS ($15,782.00), ATTACHED HERETO AS EXHIBIT "A";
AUTHORIZING THE MAYOR TO EXECUTE SAID AMENDMENT;
AUTHORIZING THE CITY MANAGER TO DO ALL THINGS
NECESSARY TO EFFECTUATE THIS RESOLUTION; PROVIDING FOR
AN EFFECTIVE DATE.
WHEREAS, on April 30, 3015 the City issued Request for Qualifications No. 15-04-01
for Architectural Design Services for a New Facility at 18080 Collins Avenue, of which eight (8)
responses were received; and
WHEREAS, on September 17, 2017 via Resolution No. 2015-2459 the City Commission
ratified an agreement with Perkins+Will Architects, Inc. to provide Architectural Design Services
for the Government Annex Building at 18080 Collins Avenue ("Agreement"), in an amount not to
exceed $2,222,000.00; and
WHEREAS, on December 17, 2015 via Resolution No. 2015-2504, the City Commission
approved a First Amendment to the Agreement to include interior design and consultant design
fees for the 250-seat Community Theater integrated within the design of the Government Annex
Building, in an amount not to exceed $229,300.00; and
WHEREAS, on April 19, 2018 via Resolution No. 2018-2812, the City Commission
approved a Second Amendment to the Agreement to include the design of a pedestrian overpass
bridge integrated within the design of the Government Annex Building, in an amount not to exceed
$370,000.00; and
WHEREAS, on October 17th, 2019 via Resolution No. 2019-3002, the City Commission
approved a Third Amendment to the Agreement to include coordination with the Florida
Department of Transportation ("FDOT") and other regulatory agencies having jurisdiction over
Collins Avenue relating to the pedestrian overpass bridge, in an amount not to exceed $50,000.00;
and
WHEREAS, on February 20, 2020 via Resolution No. 2020-3044, the City Commission
approved a Fourth Amendment to the Agreement to include coordination with FDOT civil
engineering and architecture, cost estimate and formatting relating to schematic designs, and wind
modeling, in an amount not to exceed $183,850.00, bringing the total contract amount not to
exceed $3,055,150.00; and
WHEREAS, on July 16th, 2020 via Resolution No. 2020-3083, the City Commission
approved a Fifth Amendment to the Agreement to provide structural analysis and supporting
documentation for Construction Sequence Engineering, as requested by FDOT, provide the precast
concrete attachment details as part of the construction documents, engage GFRC manufacture,
Building Blocks, for design assist to develop the basis of design for the precast panel attachment
details and performance specifications, and to provide design assist to further define the GFRC

design to optimize fabrication, constructability and cost efficiencies, as requested by FDOT, in an
amount not to exceed $99,900.00, bringing the total contract amount not to exceed $3, 155,050.00;
and
WHEREAS, the City Commission now wishes to approve a Sixth Amendment to the
Agreement with Perkins+Will Architects, Inc. for the development of a Maintenance of Traffic
plan, as required by FDOT, in an amount not to exceed Fifteen Thousand Seven Hundred Eighty -
Two Dollars ($15,782.00), bringing the total contract amount not to exceed Three Million One
Hundred Seventy Thousand Eight Hundred Thirty -Two Dollars ($3,170,832.00), attached hereto
as Exhibit "A".
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Approval of Sixth Amendment. The City Commission hereby approves a Sixth
Amendment to the Agreement with Perkins+Will Architects, Inc. for the development of a
Maintenance of Traffic plan, as required by FDOT, in an amount not to exceed Fifteen Thousand
Seven Hundred Eighty -Two Dollars ($15,782.00), bringing the total contract amount not to exceed
Three Million One Hundred Seventy Thousand Eight Hundred Thirty -Two Dollars
($3,170,832.00), attached hereto as Exhibit "A".
Section 2. Authorization of Mayor. The Mayor is hereby authorized to execute said
Amendment.
Section 3. Authorization of City Manager. The City Manager is hereby authorized to do all
things necessary to effectuate this Resolution.
Section 4. Effective Date. This resolution shall become effective upon adoption.

Resolution Approving a Sixth Amendment to the Perkins
& Will Architects, Inc. Agreement
RECOMMENDATION:
Staff is recommending the City Commission approve the attached
Resolution.
REASONS:
The City of Sunny Isles Beach wishes to approve a Sixth Amendment
to the Agreement with Perkins & Will Architects, Inc. for architectural
design services for the pedestrian overpass bridge located at 18080
Collins Avenue.
Based on the feedback from both Florida Department of
Transportation ('FDOT') and Peer Review Engineer, a 'Maintenance of
Traffic Plan' must be developed prior to selection of contractor.
Therefore, additional services, pursuant to Section 1.3 of the
Agreement, Staff wishes amend Article 3 of the Agreement
('Architect's Basic Services') to include the development of a
Maintenance and protection of Traffic Plans ("MOT") as requested by
FDOT. The total amount of this amendment is not to exceed
$15,782.00.

SIXTH AMENDMENT TO THE AGREEMENT BETWEEN
THE CITY OF SUNNY ISLES BEACH AND
PERKINS & WILL ARCHITECTS, INC.
CONTRACT NO. C1415-087
This Sixth Amendment to the Agreement between the CITY OF SUNNY ISLES BEACH
("Ci ") anjPELZKjNS & WILL ARCHITECTS, INC. ("Architect"), executed tliis day
of 2020 is made a part of the original Agreement between the parties dated
August 21, 2015, as amended, Contract No. C1415-087 (the "Agreement"), and the City and the
Architect hereby agree to amend the Agreement as follows:
1. ADDITIONAL SERVICES. Pursuant to Section 1.3 of the Agreement, the City
hereby wishes to amend Article 3 of the Agreement ("Architect's Basic Services") to include the
development of a Maintenance and Protection of Traffic Plans ("MOT") as requested by FDOT;
all as more particularly described in composite Exhibit "A", which is attached hereto and
incorporated herein by reference.
2. ADDITIONAL COMPENSATION. Article 2 of the Agreement is hereby
amended to include additional compensation for the Additional Services in an amount not to
exceed Fifteen Thousand Seven Hundred Eighty -Two Dollars ($15,782.00), bringing the total
contract amount not to exceed Three Million One Hundred Seventy Thousand Eight Hundred
Thirty -Two Dollars ($3,170,832.00).
3. OTHER PROVISIONS REMAIN IN EFFECT. Except as specifically
modified herein, all terms and conditions of the Agreement, as amended, shall remain in full force
and effect.
4. CONFLICTING PROVISIONS. The terms, requirements, and provisions
contained in this Sixth Amendment shall prevail and be given superior effect and priority over any
conflicting or inconsistent term, statement, requirement, or provision contained in any other
document or attachment, including but not limited to the Agreement.
IN WITNESS WHEREOF, the parties have executed this Sixth Amendment to the
Agreement as of the date referenced above.

ASR BACKGROUND & SCOPE
Based on preliminary feedback from both the FDOT and the peer review engineer related to FDOT expectations, we
have concluded that the design team must develop the Maintenance and Protection of Traffic Plans (MPT) prior to
selection of a contractor. Thornton Tomasetti, Inc. (TT) is pleased to submit this Additional Service Request (ASR) to
provide expanded Engineering Services for Sunny Isles Beach pedestrian bridge (the project). We propose to
complete the additional scope on a time and materials basis. We understand the scope of services to include:
Single -Lane. Double -Lane and Total Roadway Closure
The project crosses over Florida State Road Al (Collins Avenue) which is a major thoroughfare in southern Florida.
It is our anticipation that the construction of the project will involve single -lane, double -lane and total closure of Al in
order to facilitate construction. In close coordination with the sequencing drawings TT is producing, we anticipate the
following lane closure drawings (working from left to right facing north):
• SB left lane single -lane closure for support tower work adjacent to the west curb line and cladding of the
bridge over the left lane. We anticipate this work during normal daytime hours.
• SB center and right lane double -lane closures for temporary shoring tower work in the median and cladding
of the bridge over the center and right lanes. We anticipate this work during nighttime hours.
• SB right lane single -lane closure for ancillary work in the median. We anticipate this work during normal
daytime hours.
• NB left lane single -lane closure for ancillary work in the median. We anticipate this work during normal
daytime hours.
• NB left and center lane double -lane closures for temporary shoring tower work in the median and cladding of
the bridge over the left and center lanes. We anticipate this work during nighttime hours.
• NB right lane single -lane closure for support tower work adjacent to the east curb line and cladding of the
bridge over the right lane. We anticipate this work during normal daytime hours.
• Total closure of the NB and SB roadways for the delivery and installation of the precast structure. We
anticipate this work during nighttime hours, along with a detour in place to utilize 178th Street, Atlantic
Boulevard and 183rd Street.
• Partial and total closure of the sidewalk for ancillary construction as needed. Pedestrian detour plans will be
included.
• Temporary closure of the crosswalk at 179th Street as needed to facilitate construction. Pedestrian detour
plans will be included.

ASR DESCRIPTION MPT Plan
Lane closures will follow the guidelines expression in the Manual of Uniform Traffic Control Devices (MUTCD) and any
FDOT requirements that may pertain. General Notes will accompany the drawings.
EXCLUSION
The ASR excludes any traffic study required to determine exact work hours for the contractor. We anticipate daytime
hours for single -lane closure, and nighttime hours for double -lane and total closures.
