The percentage of the construction work self -performed by Lunacon with its labor crews was 25%. Actual start and completion dates (month/year): 6/1112019 — 4/30/2021. The opening ceremony was held in Oct 2021. The original completion date was projected for the end of August 2020. Due to COVID 19, schedule impacts began in March 2020. The project encountered major schedule impacts and delays which Miami Dade County authorized and approved an extension for the project to be completed by April 2021. Major impacts and delays were caused by equipment not available by the manufacturers and needed for the building. i.e., so, when states implemented lockdowns and quarantine protocols, many factories and mills shut down because they were not considered "essential" in most states. Items like HVAC equipment and roofing materials remained severely impacted because the manufacturing companies did not have employees to build for timely delivery to job sites. Labor shortages became relevant in the construction industry which impeded daily production rates. Many workers, when laid off at the beginning of COV1D due to shutdowns, never returned. JL Lu N Fpco CONSTRUCTION GROUP r• ,)L Ir Lu N ACON YOUR ONSTK4 TION Wr"ER Approach/ Methodology Lunacon's Project approach is based on one very basic concept — COMMUNICATION — therefore, Lunacon will ensure that the Project begins with a very clear understanding of the scope of work and objectives the team set forth early in the process. Once the project is awarded, we will schedule a series of meetings/processes that will focus on planning, risk mitigation, client satisfaction and contract documents compliance. Some of these include: • Team Review prior to the commencement of the physical work of the plans, specifications and contract documents with Lunacon pre -construction and construction team. This will take into consideration valuable input from Trade Partners which will facilitate valuable information such as: equipment procurement, delivery and installation timeframes. • Subsequent quality review meetings are conducted by the Project Manager, General Superintendent and in-house Quality Director / Coordinator. We encourage the participation of the A/E in these meetings and to confirm the schedule for periodic monitoring and inspections. This process contributes to Lunacon's reputation for on-time and on budget delivery. • When appropriate, Lunacon will add value to the A/E in the event conflicts arise, to ensure timely resolution of design concerns that could affect the schedule. • At every stage of the project, Lunacon will keep these questions in mind: Are we meeting the Owner's requirements and expectations?" "Are we honoring LUNACON's values: Integrity, Courage, Resourcefulness, Excellence, Work Ethics. Pre -Award Meetings: The Team will conduct pre -award meetings with all subcontractors prior to issuing their contracts. We will follow the LEAN Construction methodology to take into consideration, scope, phasing, equipment delivery, shutdown, etc. Project Partnering Meeting: There will be a partnering session, initiated by the Lunacon with the owner, with all selected subcontractors as soon as selection is complete. The Critical Path schedule will be presented at this meeting, and the process of issue and conflict resolution will begin. Close coordination with the Owner's Commissioning agentwill be required since the submittal process up until close out and training. Project Kick-off Meeting: We will conduct a project kick-off meeting with the owner, end user and subcontractors prior to commencing construction. Quality ControUTask Analysis Meetings: Prior to each major task commencing on-site, a series of quality control/task analysis meetings will be held to confirm that the subcontractor is compliant with the scope and that all necessary shop drawings and submittals are approved and on file. During these meetings, we will also clarify how the task will affect factors, such as project safety, other subcontractors' activities, maintenance of traffic plans and overall project schedule. Our team's quality control plan has three phases that encompass pre -work, work and post -work for each major task. Activity Hazard Analysis: Every task is subject to activity hazard analysis. This analysis is a component of our risk management plan. Potential hazards created by the task and to the task are identified and analyzed for potential impact. A weighted average will be assigned to each identified Lunacon Engineering Group Corp dba. Risks will be prioritized and planned for appropriately. Weekly Subcontractor and Progress Meetings: Each week these meetings will be held with all subcontractor and owner representatives present. The Project Manager and, Superintendent, will coordinate the meetings and prepare and distribute meeting minutes. One-week, two-week and Long-term schedules will be reviewed, and any issues addressed in the meetings. Schedule updates will be made as required or at least once per month Closeout Meetings: Within days of each subcontractor's mobilization, the first closeout meeting will be held. The purpose of these meetings is to ensure the subcontractor has a clear understanding of its role and responsibilities in the closeout/as-built process and to ensure the subcontractor is properly maintaining documentation for turnover to the owner. These meetings are in addition to meetings planned and coordinated by the owner. QUALITY ASSURANCE The purpose of Lunacon's Quality Assurance Plan is to provide a tangible means to monitor, evaluate and manage Quality Assurance throughout the entire design/build process. The goal throughout is to exceed the clients' quality expectations while maintaining the schedule and budget as planned. As the lead on this project, Lunacon will oversee the Trade Partner's quality control efforts during construction using a three-phase Construction Quality Assurance System utilized by the US Army Carps of Engineers. Quality Assurance Plan Objective: establish, implement, and maintain an effective Quality Assurance Program dedicated to achieving the highest attainable level of quality, workmanship, and safety. To accomplish this mission, we propose the following basic concepts: • To document and verify compliance with the Contract Documents. • To prevent the construction of deficient, defective, or nonconforming work. • To minimize pre -final and final punch lists. To ensure that Quality Assurance objectives are attained, the Quality Assurance Staff will: • Verify that all submittals are compliant with Contract Documents. • Establish an effective and efficient Document Control System in accordance with reporting and record requirements, record Quality Assurance Daily Reports, coordinate changes, and ensure any Non -Conformance Work is corrected in a timely manner. • implement proper Inspection Procedures through Preparatory, Initial, and Follow-up Inspections. • Verify compliance and inspections through appropriate testing. Transition Planning Lunacon's philosophy is that transition planning is probablythe most important phase of the project; it is when the client commences to use the building and to evaluate the return of the investment. Lunacon will organize and furnish the Owner with all close-out documents. The services offered by Lunacon include operational and maintenance programming, move -in and start-up assistance, the development of comprehensive record drawings of the project and the acquisition and turnover of all operations and maintenance manuals as well as all required warranties. These records are Lunacon Engineering Group Corp dba. Lunacon Construction Group ITB No. 25-01-01 City Annex Building Construction Renovations Due Date: 05 March 2025,4:30 PM 41 LUNACON. YOUR CONSTRUCTION PARTNER collected, kept and organized during the construction period to systematically submit them to the Owner shortly after the project is occupied. This is a process that will require close coordinating with A/E and the Commissioning Agent. Immediately upon completion Lunacon will strive at completing all project punch list in the allotted contract time. The punch list will be verified for completion and compliance with the Architect and owner. Luriacon's creates an Operations and Maintenance Manuals log to identify a program for the continuing operations and maintenance of the physical plant and equipment, a program that identifies labor and materials requirements for building operation and maintenance (cleaning, routine repair, calibration and balancing of HVAC systems, replacement of materials and components, etc.). As part of the close out phase, Lunacon will coordinate with the A/E the completion and accuracy of the project records or "As -Built Drawings". The As -Built Drawings will be provided to the Owner electronically in a cad data base. Final payment will be processed/submitted immediately thereafter. Lunacon will look forward to a superb letter of recommendation from the owner. The Home (Corporate) Office has the overall responsibility for the pre -qualification of subcontractors, establishment of credit relations with local suppliers, prompt payment and resolution of payment disputes. Part of Lunacon's mission is to build the community. Our "business as usual practice" includes hiring, supporting and training small business. Thus, our procurement strategy intentionally focuses hiring small and disadvantaged small business. Currently we hire between approximately 20% to 50% small business based on the risk and contractual requirements. Moreover, we have established specific policies to assist them with technical and administrative support thereby providing them with an opportunity to succeed. Lunacon is focused in maximizing the potential use of minority local subcontractors. Lunacon, also a Small Woman Owned Business, can self -perform some minor miscellaneous scopes of work, outside the scopes on this project. Our Team will be capable of self -performing all the Demolition and Concrete for the project. All remaining trades will be subcontracted to pre- qualified subcontractors by Lunacon. All products and services provided by the subcontractor will be subject to the acceptance of the owner, Lunacon, and Architect. Each subcontract will contain appropriate terms and conditions. Adequate facilities will be provided to meet the needs of the subcontractors, and Lunacon will support subcontractors in processing invoices and payments, subject to the invoices being delivered to Company ABC in an acceptable format for consolidation and remittance to XYZ. To this end, Company ABC will establish format requirements for invoices. Our approach to subcontractor selection, management and support is summarized below. Each bulleted clause defines a primary control in our subcontractor management plan. ■ Prescreening: Subcontractor candidates shall be checked for licensing, insurance, creditworthiness, client references, technical skills, response time, and willingness to provide emergency response if needed. We shall confirm their capabilities by reference checks and personal interviews. • Competitive Bidding: We shall pre -qualify those subcontractors wishingto participate in this bidding process. Only pre -qualified contractors shall be allowed to bid on work under this Contract. Bid requests and submissions will be formal and in writing, and awards based on price, completeness of scopes, responsiveness and technical competence. • Master Agreements: Each subcontractor shall sign a master subcontract agreement, which contains response time criteria and payment conditions. Specifications and quality control standards required for the project shall be clearly stated and made a part of the subcontract. • Rapid Response: As part of a master subcontract, selected subcontractors agree to provide rapid (24-hour or less) response when needed. This clause ensures that we have subcontractors that we can quickly contact during emergencies (storm or accident damage) or when the owner's priorities change (a project attracts general officer interest, for example). • Close Coordination: Subcontractors' work shall be carefully integrated into each element of work on the progress schedule. Performance dates, specifications and QC requirements stated in a subcontract will be discussed at a pre -construction conference and reemphasized after work begins. Lunacon Construction Group iib No. 25-01-01 City Annex Building Construction Renovations Due Date: 05 March 2025, 4:30 PM 4C LUNACQN YOUR C OH9'RUCTI0H PARTNER Safety Program Overview. Lunacon has implemented extensive policies, procedures, and incentives designed to provide a safe and healthy work environment for all our employees, subcontractors, and customers. Here is a general discussion and minimum details on Lunacon's Safety Program. Our goal on everyjob is to ensure that allwork is completed safely, with appropriate tools, equipment, and methods as implemented in complete compliance with the US Army Corps of Engineers Safety and Health Requirements (USACE) Manual (EM385-1-1, Revised November 2003). This goal takes priority over scheduling concerns. Any task that the Safety Manager or his designee observes being performed in an unsafe manner or without appropriate safety precautions will be stopped until proper safety procedures can be instituted. Potential hazards are identified and systematically reduced or eliminated as much as possible. Environmental hazards are given extra consideration to verify that all dangerous procedures and materials are handled by Occupational Safety and Health Administration (OSHA) and Environmental Protection Agency (EPA) guidelines. Every effort is made to substitute safe alternatives for potentially hazardous materials and methods. Safety compliance is a primary condition of continued employment and an integral component of our annual employee performance evaluations, which inform decisions regarding income and position advancements within the organization. Christopher McKenize is the Lunacon Safety Manager. He is responsible for certifying that all workers know proper safety procedures. He will conduct weekly safety training sessions to ensure that everyone understands the dangers and hazards of the work in progress and that all site workers are familiar with the safe operation and handling of equipment and materials. We require that subcontractors maintain the same level of education as all employees in all jobs. Health and safety inspections are held regularlywhile a project is in progress. We publish and maintain a Safety Plan for all our employees and subcontractors. The goals of the Safety Plan are to minimize the risk of injury on the job, reduce property damage, and enforce compliance with all laws, codes, and guidelines. Our Safety Plan includes education in specific job-related tasks as well as general, continuing safety training. The plan outlines methods to detect, correct, and prevent unsafe practices and hazardous conditions. The plan also provides information on supplying and adequately using protective equipment and safety mechanisms and establishes a regular tool and equipment maintenance schedule. All subcontractors are also shown the location of alt emergency equipment and Material Safety Data Sheets (MSDS), telephones, and evacuation routes. Standards in housekeeping, protective safety equipment, weekly safety meetings, fire protection equipment, and treating, reporting, and investigating injuries are also outlined in Lunacon's Safety Plan. Also included are safe procedures for all construction activities by CSI category. Lunacon's Safety Plan is included as a compliance document with our Employee Handbook and as a referenced specification in all subcontracts. A copy of his resume is included in this section. The assigned onsite staff will be responsible for our Corporate Safety Program and the safety of all field activities. The Site Safety and Health Officer wiLL be responsible for managing safety for all field operations on the site (s) with support from the Site Superintendent. They will conduct preparatory Activity Hazard Analysis (AHA) with all subcontractors and conduct daily safety inspections. They can shut down any work activities if any safety standards are violated. They wiLL be responsible to the and Lunacon for the overall safety of the project(s) and have complete authority and responsibility to shut the job down if OSHA Safety Standards are not being met. In summary, on all projects, our corporate principles require full Lunacon Engineering Group Corp dba. Please note that all our Project Managers and Superintendents are cross trained in Safety. How Lunacon Promotes Construction Safety. To promote Safety on all our project job sites and keep our customers as involved as possible in our safety activities, Lunacon has developed the following procedures for use with all our government and private customers. We are currently using these programs in all our current projects: • Monthly Safety Briefing: The safety manager sets the attendance of all local Lunacon employees for the monthly government/contractor safety meeting as a priority. In addition, government quality control and safety personnel are invited (and encouraged) to attend these meetings. Maximum employee participation is always achieved. • Monthly "Onsite" Safety and Quality Control Audit: The Lunacon Site Safety and Health Officer and Quality Control Managers physically conduct a monthly "on-site" safety and quality control audit at all construction sites. The audit consists of a safety and quality control standard inspection of all sites, performed under a checklist of critical safety and quality control items, and conducted by a competently trained and responsible professional. All construction work sites are validated and scored according to Lunacon safety and quality control standards. The results are posted for aLL the employees (superintendents and supervisors) to "learn from the mistakes of others" and "to learn from good ideas of others." • Monthly Safety Bulletin: The Lunacon safety and quality control manager publishes and distributes a monthly Safety Bulletin, which identifies safety and quality control trends over the last month and identifies noteworthy safety and quality control issues pertinent to the current construction activities. • Local Weekly Safety Administrative and Equipment Reviews: The Site Safety and Health Officer/Safety Manager physically conducts a weekly safety administrative and safety equipment review of all construction sites. This review uses a standard checklist of administrative safety items of concern, and the validation of safety equipment maintained at the construction job site. The primary focus for the onsite safety review is to ensure all "onsite" safety administration requirements meet identified safety standards and that aLL "onsite" safety equipment meets the identified safety standards. • Project Site Visitors: All persons visiting the project site shall sign in at the site office, be accompanied by an authorized escort while onsite, wear a hard hat, comply with all project safety rules, and sign out upon leaving the project facility. This policy promotes the safety and security of all visitors. • Mandatory PPE: On the project site, all workers and site personnel must wear mandatory Personal Protective Equipment (PPE): safety glasses, hard hats, and leather safety -toe shoes. This policy has reduced personnel injuries for employees and subcontractors. Lunacon Engineering Group Corp dba. Lunacon Construction Group ITB No. 25-01-01 City Annex Building Construction Renovations Due Date: 05 March 2025, 4:30 PM 50 LUNRCO • COVID-19 Safety and Reporting Procedures. Lunacon developed a Corporate COVID-19 response plan to protect workers, which follows OSHA, CDC Interim Guidance for Businesses and Employers, and the Department of Defense (DoD) guidelines. All job site boards include this important information. COVID-19 is addressed in all APPS submitted to the government. During daily safety meetings, the safety officer addresses COVID-19, and all workers on site must answer questions on how they feel It is stressed to everyone during this meeting that if someone feels ill, they must leave and stay home. Each site has hand sanitizer and antibacterial soap to support all personnel. If required, wash stations are also provided with port -a -johns on the project site. * First Aid Treatment Plan. Regardless of how detailed our construction company's safety plan is and despite how in-depth and meticulous ourtraining program is, accidents involving injuries can still occur. When a worker suffers an injury, it is vital to have a first aid program in place. Because no two construction sites are the same, Lunacon designs a job site- specific first aid program for each project so that our workers can receive the best possible care should they suffer an injury. The procedures for developing a job site-specific first aid program is part of our overall safety plan. Our first aid program is available to workers who keep them on hand for reference. On each project, the assigned SSHO and Superintendent, at a minimum, are the onsite qualified and certified First Aid providers. The assigned first aid providers receive training that includes basic first aid, CPR, and an automated external defibrillator (AED). Additional training may be required depending on the type of work being performed at the job site. Lunacon ensures all retraining is done yearlyfor CPR, using an AED, and every 2— 3 years for first aid training, depending on how long the certification is good for. In addition, the site safety plan includes all emergency numbers, local hospital information, and directions. All projects have extensive safety kits that are checked daily and restocked. • Workplace incident reporting procedures. The Site Safety and Health Officer will report all accidents (up to 24 hours afterward) to the Lunacon executive team and . The Site Safety and Health Officer will also perform thorough accident investigations and complete accident report forms using Contractor Significant Injury Reports to be transmitted to the as soon as possible (but at most five working days following the incident). The Site Safety and Health Officer will coordinate accident investigations and reporting activities to maintain the OSHA recordable and other applicable injury logs and implement corrective actions as soon as reasonably possible. For immediate notification of major accidents, the Site Safety and Health Officer will immediately notify the Lunacon Executive Team and and follow up this notification with written reports within four (4) hours of the occurrence. Major accidents include: o A fatal injury or permanent partial or total disability o The hospitalization of three or more people results from a single occurrence. o Property damage more than $100,000 • Subcontractor Safety Program Compliance Requirements. Lunacon considers our subcontractors valuable members of our project teams and encourages active involvement and communication from our subcontractors at every step. Our subcontractors are fully integrated into our management and quality control approach. As part of our management Lunacon Engineering Group Corp dba. Lunacon Construction Group ITB No. 25-01-01 City Annex Building Construction Renovations Due Date: 05 March 2025,4:30 PM 51 LUNACON YOUR CDN57RLICTION PPRTRER and quality control approach, a preparatory safety assessment of the project specifications, conceptual design, training requirements, and governing regulations is performed to identify project safety requirements accurately. During the project planning stage, site-specific documentation, such as the Fall Protection Plan, Weight Handling Equipment Plan, APP, AHA, Health and Safety Plan, etc., are developed. During project execution, but as a function of the quality control program, independent, frequent, and targeted safety inspections are performed to ensure that project plans are being adhered to and applicable sections of OSHA and EM 385-1-1 are followed. The Project Team continuously collaborates to assess overall project safety, identify areas of weakness, and implement corrective measures. Lunacon has established a proven strategy for successful subcontract administration based on this approach. o Before site work begins, Lunacon initiates a Subcontractor Coordination Meeting to develop a mutual understanding of safety objectives, identify critical control measures, discuss implementation and inspection, identify the lines of authority and communication, and establish quality and safety expectations. Attendance and participation from subcontractor Lead personnel is mandatory. o After the subcontract award, subcontractor site personnel attend a mandatory "Safety Orientation Course" conducted by Lunacon to define the rules and requirements about safety and the subcontractor's responsibilities and authorities to promote safety on site. n Subcontractor site personnel also attend daily tailgate safety briefings. Particular emphasis is placed on AHAs, the specific DFOWs to be executed by each crew, governing regulations, emergency response actions, communication, and the hazards associated with multi-employer work sites. o Lead subcontractor personnel are included in Preparatory and Initial Phase Meetings for each DFOW as part of our USACE-derived quality control program. Safe inspection objectives, frequencies, and features are identified and initiated; AHAs are reviewed and updated as necessary; training records are reviewed; competent persons are identified; required testing parameters are confirmed; and permits are issued. o Weekly progress meetings are held to discuss safety inspection and testing results, current and potential trends affecting safety, corrective actions taken and implemented, lessons Learned, and upcoming requirements. Safety issues directly affectingsite personnel are addressed and corrected immediately. o Since multi -task execution potentially affects all site personnel, any person (including subcontractors) has stop work authority upon observation of a potentially unsafe activity on site. c Enforcement Procedures (i.e., disciplinary actions implemented afterviolations). Per Lunacon's Safety DisciplinaryActions and Imminent Dangers policy, at the discretion of the site superintendent and SSHO with concurrence from the Project Manager, Lunacon Engineering Group Corp dba. Lunacon Construction Group ITB No. 25-01-01 City Annex Building Construction Renovations Due Date: 05 March 2025, 4:30 PM 52 1 LUNRCO serious or imminent danger violations warrant immediate action, up to and including termination and removal from the project to include subcontractors Any supervisor or manager who knowingly exposes workers to imminent danger situations is also subject to immediate termination. This policy is also discussed with each subcontractor and is included in the subcontract issued for the work to be done on the project. Lunacon may disqualify any subcontractor for future work at its sole discretion for safety concerns such as, but not limited to, poor safety performance, repeated safety violations, or other unsafe acts. Protection of the workforce, subcontractors, buildings, and equipment. As the prime contractor, Lunacon is fully responsible for the safe execution of work for this contract, which involves protecting the workforce, including our subcontractors, buildings, and equipment during construction. Our approach to integrating our Safety Program with our subcontractors promotes a safe working environment for the project team and an incident -free attitude and commitment from all onsite personnel. Employees and subcontractors must always comply with the safety requirements of EM 385-1-1 and Lunacon's approved corporate and project plans, Stop work authority on the grounds of safety is granted to everyone. An accident prevention plan and health and safety plan will be developed for each project, specific to the Location and complexity of the work to be completed. Lunacon ensures that all workers, including subcontractors, comply with EM 385-1-1 and all OSHA and state standards that apply to the work performed. We train employees in relevant hazards and hazard control methods, including PPE and fire/emergency procedures, and require our employees to work safely, follow applicable safety rules, standards, regulations, and contract provisions, and take meaningful and effective action to hold employees accountable for noncompliance. This flows down to the subcontract agreements, where we expect all contractors to adhere to Lunacon's safety culture. Lunacon requires that all project worksites be reviewed daily to identify existing workplace hazards and ensure employees and subcontractors follow prescribed safety procedures. Where hazards are found, either promptly correct them or implement interim controls to protect employees until they are corrected. Lunacon stresses the importance of recording and reporting all employee injuries as required by 29 CER Part 1904, "Recording and Reporting Occupational Injuries and Illnesses," or applicable state OSHA requirements. We act immediately when notified of serious/recurring safety problems, including removing subcontractors, employee(s), and equipment from a project site or suspending work (including subcontractor activities) pending satisfactory resolution of said safety problems. Since multi -task execution potentially affects all site personnel, any person (including subcontractors) has stop work authority upon observation of a potentially unsafe activity on site.
