A large portion of these projects have been roadways and bridges, including a wide variety of bridges throughout Florida. With differing sizes, types and structure, many of these bridges have involved working in and over water. From protecting manatees to working effectively and safely from barges and trestles, we understand the specific challenges involved with working near water. Many of our bridges also have involved a pedestrian portion. For instance, an important part of the scope of work on the recent Bear Cut and West Bridges Emergency Rehabilitation project involved widening and adding a shared use path for pedestrians and cyclists. On this project, we were able to maintain heavy cyclist and pedestrian traffic throughout the construction period. In addition, many of our bridges were constructed in close proximity to businesses and structures. On our 5th Street Bascule Bridge project, we held public town hall meetings to discuss the impacts of construction and worked with area businesses to lessen those impacts. This technical and local experience is embodied in key personnel such as Project Director Frank DiGilio and Project Manager Nick Sosa. Together, these residents of southern Florida have 40 years of industry experience and have been involved with building more than a dozen projects featuring bridges. Frank, Nick and the other key team members will utilize our methodologies built on years of project experience and strategies, innovative construction solutions, and in -house expertise to provide a cost - effective, and a construction phasing plan that will deliver the project successfully — safely, on time, and within budget. Two of our primary focuses are safety and quality, which are highlighted in the overviews below. Team members — including subcontractors — follow the site - specific practices detailed in the project's safety 2014 Experience Modification Ratings One of the most important indicators of our safety performance is our current EMR, providing ample proof of our care and commitment to sending our employees home SAFE each and every day. Average / At Risk A SAFE Good plan and participate in training and on -site safety meetings to enhance their understanding of safety issues and procedures. Our safety programs also implement best practices to ensure the local community is kept safe. From pedestrians and motorists to working crews and inspectors, we work hard to make sure "Nobody Gets Hurt." Our outstanding safety record shows the effectiveness of the safety culture and approach adopted by all team members. We focus on the belief that accidents happen because someone did not understand he was working in an unsafe manner, or was not concerned whether or not he worked safe. Our approach to safety focuses on addressing these two root causes of accidents by placing substantial emphasis on training all personnel to know what hazards were present and what precautionary steps needed to be implemented; and by caring about every team member and focusing on the importance of safety and the personal loss when someone gets hurt. The following programs are the core of our safety approach and motivating everyone to work safe: Creating a "Zero Tolerance" Safety Culture We have moved beyond standard industry safety practices based on OSHA recordable incidents. Our project teams embrace a "zero tolerance" policy that records all hurts (not just OSHA recordable incidents) and focuses on eliminating the behaviors that can lead to them. All new employees are trained in this "Nobody Gets Hurt" safety program and informed about site - specific hazards before being allowed to begin work. They are then monitored for adherence to safety practices and general safety attitude during a 30 -day period. Staff members are also expected to never to walk by an unsafe act or situation without stopping and taking action to fix it. These safety issues and appropriate best practices are discussed at weekly safety meetings and at the "Play of the Day" meetings that plan the next day's work. Hazard Recognition and Avoidance Historically, 50% of accidents are caused by hazards not identified by supervisors or the crew. We have enhanced our existing Job Hazard Analysis program by increasing the focus and training on recognizing and communicating potential hazards to all team members. These training programs also focus on increasing crew participation and improving the supervisors' presentation of the JHA to their crews. Our site - specific safety plans are tailored to each project to address all aspects of safety goals At the core of our daily tools that support our safety program is the Job Hazard Analysis, which is prepared for every project task or work package. It serves as a useful tool to help ensure the safety and health of personnel and the public, maintain compliance with OSHA hazards assessment requirements and the Personal Protective Equipment Standard. The JHA is not just an administrative exercise; it is a practice implemented by safety managers and team leaders. They are used to brief project personnel on the type and degree of hazards expected before each operation. In return, personnel understand the means they will use to safeguard themselves from any potential hazards. We hold our subcontractors to the same level of accountability; they are contractually required to comply with the same safety program requirements as our Kiewit employees. Supervisor Training and Involvement Supervisors are the key to safe operations. Without them, none of the safety goals can be realized. Due to their critical role, we provide training to all supervisors on the key elements of supervising safe operations, including increased involvement in safety planning and effective communication to their crews. Craft Participation and Involvement Our safety approach also includes having craft personnel actively involved in the program. Safety awareness, training, and empowerment assist the craft to help each other work safely. Through their participation and implementation of the best safety practices, craft have a voice in the safety plan. Performance Measurement Many construction companies measure their safety performance solely on the number of accidents or accident frequencies. This sole focus on accidents fails to measure how safe a crew member is working. Instead of measuring accidents, we measure certain safety leading indicators and focus on crews practicing the right safety behaviors. When possible, crews receive safety incentive awards for consistently "doing the right thing." MKiewit Construction of the North Bay Road Pedestrian Emergency Bridge I No. 14 -09 -01 The Human Side of Safety Many construction workers believe their employers are more concerned about the statistics than the individuals who are injured. A company's focus on the number of accidents or accident frequency rates reinforces this common belief, sending the wrong message and hindering the effectiveness of a safety program. Our safety training and communication highlights the "why" we all work accident free. The message emphasizes that non -work reasons such as family and friends are the most important motivators for safe work behavior. When these true reasons become part of the project's safety focus, a new awareness and success usually follow. Quality Focus: Building it Right the First Time Kiewit has been a primary participant in pioneering what is now the industry standard for quality programs. Our strong corporate quality culture places a premium on delivering high quality, long- lasting projects that reduce the overall life cycle costs for our owners. We recognize that our project owners expect and deserve a high - quality product, and our reputation is built on delivering on that expectation. Our philosophy for achieving that quality expectation in all we do is to simply "Build it right the first time." The QA /QC plan is the basis of the entire program and starts with the development of a project specific plan. As part of this plan, we define each quality and testing requirement, testing and frequency of inspection for field operations, as well as identifying specific hold points for self- inspection before proceeding to the next step. We also identify each task or component requiring specific QA or QC analysis for risk management and contract compliance. The procedures, which are specific to the project, define the purpose, scope and steps to follow to complete each process and are reviewed regularly to ensure effectiveness. Our Quality Manager will prepare a project- specific Contractors Quality Control Plan (CQC Plan) as part of our project start -up. The CQC Plan will set forth various specific procedures to ensure the quality of construction. However, our overall process can be summed up in these three steps: "Plan it. Build it. Check it." Plan It. The only way to achieve success on every operation is to carefully plan the work. Prior to the start of any construction activity, our superintendents and engineers will be involved in detailed planning to ensure the craft understand exactly what they are building, the requirements that must be followed, and the critical quality elements. We will make certain that the work is executed in a controlled manner by requiring that all operations be compliant with an approved, written work plan. The work plans will be researched, planned, reviewed, approved, executed, monitored and revised as necessary. To obtain the desired product, the work plans will be extremely detailed and prescriptive in the procedures to be followed, specifying: 4 A summary of the materials, equipment, labor, permits, traffic control, and other key elements that are needed for the operation. MKiewit1 Construction of the North Bay Road Pedestrian Emergency Bridge � No. 74 -09 -07 A step -by -step guide to constructing the work including applicable sketches and drawings. Each step will include the specified tolerances, quality risks and hold points for the operation. An in- depth, operation- specific quality analysis, which will identify potential quality risks and include a summary of pertinent specifications, allowable tolerances, hold points and any submittals or certifications that are required. Work plans will be reviewed to ensure that applicable codes, standards, specifications or other contractual requirements are identified. The various members of the quality organization will ensure that work is f conducted in accordance with these work plans through inspection, testing and audits. Prior to construction of each identifiable feature of work, or after a major change in construction operations, a pre- activity meeting will be held with the City to ensure that all necessary planning has taken place, to communicate the plan to all parties involved and to receive and incorporate feedback. Build It. Our superintendents, field engineers, foremen and crews will use their expertise i and skills to construct the work in conformance with the project requirements. All deficient items will be documented and corrected and the work procedures will be updated to reflect the revised process. For example, after the first step is taken, our supervisors, o City representatives and craft will meet to review the operation sequence and make any necessary adjustments prior to the next operation. The execution cycle will begin once the initial inspection has been completed and any corrective changes have been made. From this point, the operation repeats, but it will be monitored and coordinated daily through the Play of the Day meetings and process control and verification. Check It. Inspection and testing are the cornerstone to successfully monitoring what is being built in the field. This duty, as per contract, rests in the qualified hands of the design - builder. As with past projects, we will use qualified independent materials testing firms familiar with the City to perform this requirement. Inspection and testing o• requirements for all operations will be identified within the work plan, and these requirements will be communicated to all members of the team at the preactivity and Play of the Day meetings. The Inspection and Testing Plan will serve as the guide to monitor and control the work; if needed, specific procedures and documentation requirements will be developed for unique work. The procedures will outline test requirements, acceptance criteria and test conditions. The procedure will also contain the test and inspection checklist to be used. @Kiewit Construction of the North Bay Road Pedestrian Emergency Bridge I No. 14 -09 -01 Our Schedule and Understanding of the Project Issues and Work Tasks Project Issues Permitting restrictions. The multitude of federal, state and local permitting requirements in southern Florida can lead to a variety of project issues. Our team is very familiar with navigating permit restrictions from the US Coast Guard, the US Army Corps of Engineers, the South Florida Water Management District and the county's Department of Environmental Resources Management. In this case, the US Coast Guard permit requires that the 4th span be maintained for marine navigation. In addition, the SFWMD permit requires barges to maintain 1 ft. of clearance from the bottom of the channel. Due to shallow areas on the south side of the bridge, barge- mounted equipment will have too much draft to maintain this requirement. Our solution is to construct a temporary bridge or trestle. MOT scheme. The creation of a comprehensive MOT scheme in advance is another project component that could create issues for people who live and work nearby. We are highly skilled at creating and implementing MOT plans that take into consideration the needs of the entire community. With an adjacent daycare area on the north side, we are aware of the attention required to protect the public and secure the construction site every night. From previous projects constructed in similar environments, we have learned the key to success is to keep the surrounding residences informed of our activities and to maintain the schedule as discussed. Schedule and Work Task Understanding Once awarded the project, we will begin pursuing the pre- construction activities, including the job- specific QC plan, the MOT plan, temporary structures design, shop drawings and the procurement of materials. We employ a full -time PE to develop and stamp temporary designs as required. As a part of our quality program, we maintain an engineer at the pre -cast yard to perform quality audits on permanent materials before they are shipped to the project. Following NTP, erosion control, MOT installation, and clearing and grubbing, the soil mixing operation will be the first permanent work installed. Since this operation will disrupt entire access to the bridge, it must take place first so that access to the structure will be clear. Soil is excavated to 1 ft. above the water table for wet mixing. Traditionally, a batch plant would be installed on site during construction; but, due to the tight areas, the soil mixing operation will employ ready mix trucks from a local concrete plant. The mixing operation will take approximately two weeks to complete both sides with two additional weeks of cure time. A cone penetrometer will be used to test the shear strength of the soil before it can be loaded. Next, a temporary bridge or trestle will be built to allow for drilled shaft and crane access to the water. The trestle on the south side will be built up to bent 4, and from the north side will be built south to bent 5. The trestle will allow the augers to drill and pour the shafts during construction. Plus, vacuum trucks will be employed during shaft construction to keep shaft slurry from entering the water. L L. L L 7i. MKIewitConstruction of the North Bay Road Pedestrian Emergency Bridge I No. 14 -09 -01 The trestle will maintain the same footprint as the new bridge, but it will be constructed above the bridge's profile. Removable crane mats will allow caps to be built and beams set from the same trestle. Because the weight of the exterior beams is so great, a 200 TN crane must be used on the trestle. Concurrently with the structures work over water, the roadway work will begin on the opposite side of the bridge. The exfiltration trench along 172nd Street will be constructed in the wet so that no dewatering will be required. Work along this road will take place adjacent to live vehicular and pedestrian traffic, requiring the use of flaggers and special attention to eliminate drop off and clear zone hazards after each shift. Roadway and MSE wall construction will take place after drainage. MSE work is common for us, but the tight access on the south side will require special material handling plans to construct. We have constructed several roads in Miami with the similar geogrid typical sections. Furthermore, we have developed excellent relationships with South Florida subcontractors, which will perform the flatwork, lighting, fencing, paint and asphalt work. We treat subcontracted work as our own and will maintain a supervisor over each operation. As seen on the construction schedule below, the anticipated award date is on 5/18/15 with the anticipated NTP on 6/22/15. With the given completion time of 390 days, we have scheduled the work to be completed by April 14, 2016 in 297 days. All the work shown on the schedule will be completed with one structures crew and one roadway crew. The critical schedule items, which will be subcontracted, are the soil mixing and drilled shaft work. The drilled shaft work, which encompasses 21 shafts at six different piers, is scheduled for a total of 11 weeks. The initial two weeks will involve installing and testing the first method shaft. The remaining nine weeks will be spent doing 1 pier /wk with moving access 1/2 pier /wk to set up for the drilling. Schedule Management Narrative Open, continuous communication and teamwork will be a priority for everyone involved with this project. The project included the complete replacement of the steel portion of an existing bascule bridge, removing an existing flyover bridge and the complete reconstruction of 1 mile of A1A. The structural scope consisted of the total replacement of four bascule leaves and installation of new machinery. Structural Steel Superintendent, SW 2nd Ave Bascule Bridge, FDOT District 6, Miami, FL, $43.5 million. Luke served as Structural Steel Superintendent for this $43.5 million bascule bridge project. Scope of work consisted of a new single span, double leaf Bascule Bridge. The bridge at 303 ft clear distance from trunnion to trunnion and a nominal width of 72 ft is one of the longest, widest and heaviest bascule leafs in the world. Construction elements include, heel sections at 900 tons each, four trunnions 4 ft dia.,18 ft long at 50 tons each, toe section at 100 tons each. Over 2300 tons of structural steel was fabricated with over 2400 tons of steel in the counterweight. Luke served as General Superintendent for this FDOT project that involved the rehabilitation of an existing bridge over the ICWW. The scope included demolition of one mile of existing roadway and the construction MKiewitConstruction of the North Bay Road Pedestrian Emergency Bridge No. 14 -09 -01 of a new, one mile bridge, and the rehabilitation of a nine -mile segment of existing roadway. The project was built to USACE and FDOT specifications. It also required strict adherence to environmental regulations and included monitoring endangered and protected species. Work was completed within limited right -of- way to minimize environmental impacts. This project was located in the airside operations area (AOA) of the H -JAIA and includes repairs to trench drains, spalls and full depth concrete slabs. Additional scope includes selective grading, striping, wind cone relocation, and additional taxiway guidance signs. Major quantities include 3,000 SF of spall repairs, 400 LF of trench drain repairs, 833 SY of full depth slab replacements, 8 wind cone relocations. Safety Manager, Inter - county Connector (ICC) Contract B, Maryland State Highway Authority, Baltimore, MD $560 Million In his role as Safety Manager, Bert was responsible for ensuring a safe work environment for more than 400 --raft and staff and more than 100 subcontractors constructing this seven - mile -long, six -lane toll road. Manager, LPV -145 Levee Wall Construction United States Army Corps of Engineers, Saint Bernard Parish, LA $360 million As Safety Manager, Bert was responsible for ensuring a safe environment for all craft and staff, including implementing and supporting the safety programs, policies, procedures and training for all staff. The project provides future hurricane surge protection for the city of Chalmette. Construction included a 30,900 LF floodwall designed to meet the 100 -year storm criteria and new elevation standards. The 6.5 miles of floodwall consisted of two rows of 100 -ft long H -pile, a single row of sheet pile, 14 -ft wide foundation with mud mats and a 12 -ft high, 4 -ft thick concrete wall. Along with the 500 -ft long temporary access bridge, quantities consisted of 13,500 each H -pile; 34,000 LF of sheet pile; and more than 100,000 CY of concrete. Quality Control Manager /Project Engineer, SR 90 Tamiami Trail Modified Water Deliveries, USACE, Miami, FL, $81 million As the alternate Construction QC Manager, Russel's duties included being on site during all construction activities if the CQC Manager was not available, communicating with the USACE on a daily basis and ensuring that the project was built in accordance with plans and specifications. He also performed general site inspections. Russel also served as Project Engineer with responsibilities that included all contract administration, maintaining the P -6 schedule, manage all cost reporting, perform financial projections, review all subcontractor change orders, review all submittals and RFIs prior to submission, facilitate subcontractor coordination and payment and run all owner progress meetings. Before these roles, Russel was the Roadway Engineer, which required tracking quantities, performing takeoffs, working with subcontractors, tracking daily cost, analyzing survey data, and producing cost projections. 19 Kiewit Construction of the North Bay Road Pedestrian Emergency Bridge I No. 14 -09 -01 Key Subcontractor Qualifications Finalized pricing from our subcontractors will not be received until the day this submittal is due in order to secure the best pricing possible. As a result, the key personnel, experience and qualifications of our selected subcontractors will be available post -bid. However, our selection process will ensure that all subcontractors will satisfy the criteria contained in the bid documents, and will represent the best value to the City of Sunny Isles Beach. Our process includes a detailed analysis of each prospective subcontractor. The analysis evaluates factors such as relevant experience performing similar work, safety, quality, schedule performance, financial stability and price. Special attention is given to critical or key elements of work, which includes drilled shafts and soil mixing on this project. On these elements, we evaluate the prospective subcontractor's Superintendent to ensure the firm's personnel has the necessary experience and qualifications. The process is designed to help us deliver work that is performed safely, on time, and with the high - quality standards we expect. In addition, we will evaluate the availability of DBE and small business subcontractors in the market and what trades they are certified in to establish DBE set -aside packages (if required) and ensure their participation on the project. Our typical outreach activities to local firms (including DBE and small businesses) involve: ♦ Providing written notification and making follow -up telephone calls to solicit interest in the project. ♦ Sending out written notification (electronic) to community entities. Schedule & Methodology Our Methodology & Approach to Completing the Work As an experienced bridge and roadway constructor, we understand the work involved with completing the North Bay Road Pedestrian Emergency Bridge. One of the main elements is installing 42 -in. drilled shafts. Then, the substructure is placed on bents founded on the drilled shafts. The bridge's superstructure will consist of a cast -in -place reinforced concrete deck supported by continuous Florida 1 -36 prestressed concrete beams. In addition, a barrier wall with handrail and a Class 5 finish will be installed. Another primary element is the bridge deck. In order to accomplish the deck's tricky pour, the bidwell will have to be setup on a skew to match the bents vs. perpendicular to the bridge deck. This will ensure that the beams will all deflect at the same time. Additionally, the bidwell's west side will have to stay loose so that it can adjust to fluctuating span dimensions. Another project aspect is the clear and grub work. On the south side of the roadway, work can start from the east to the west on NE 172nd Street. During this operation, all the organic material will be removed under utility installations. The geosynthetic fabrics (BX1200 and BX 1100) will be used to stabilize the area underneath the roadway. This method was used successfully in a high traffic area on the outside shoulder of US 441 during the Golden Glades project. After clear and grub on the north is completed, the soil mixing subcontractor will mobilize the north side. Due to the weight of the embankment in the MSE walls on the organic layers, there will be short -term and long -term settlement. We concur with the geotechnical report that the best option to mitigate this issue is soil mixing. We have used this exact application of soil mixing at the Port Everglades project. The advantages of soil mixing include low vibrations, mostly clean and quiet work, the use of readily available materials such as cement, and it generates nearly no spoils for disposal. In addition to these technical project aspects, we understand that this project will require effective coordination and communication with local businesses and residents to minimize potential impacts and keep them informed. We work with the City to foster positive perception of the project by the project and other stakeholders. We will also remain diligent about protecting the area's natural resources and way of life by minimizing any potential impacts to the environment. With experience such as building a bridge in the Florida Everglades, we understand the concerns of working in environmental sensitive areas around the state and how to comply with all the extensive requirements. Our Organization of the Project The timely and efficient project delivery is the end goal of all project teams. At this meeting, we put all issues on the table for the entire project, and include them on our 4- Square Quality Matrix (shown here), with the purpose of publicly identifying good and bad trends and formulating ideas for continual improvement. This allows the team to identify roadblocks that could potentially derail the project, and find timely solutions. This meeting also allows our team to celebrate good trends and successes. During construction, Play -of -the -Day Meetings will occur with all project stakeholders. This will facilitate open dialogue between the City, subcontractors, and our team. Our goal is to implement management systems that have been successful on past projects to identify long lead time items, implement design optimization and sequence construction to meet the project goals. These management systems will be combined with existing subcontractor and supplier relationships built upon numerous of projects within the southeastern United States. The relationships have been fostered over many years with local subcontractors and suppliers and will be of use throughout the execution of work. Familiarity with these firms will allow the project to avoid initial confusion and make it easier to organize scopes of work and facilitate jobsite coordination. Our other prime opportunity to facilitate communication between parties will be in our formal partnering sessions. Overall, our approach to partnering will be to establish a true "partnership" with the City based on effective communication, relationships, and proactive team problem solving. It is clear from experience that this approach works and will optimize the ability for each success criteria established for the project to be achieved. An effective partnering program will set the stage for positive team - building, which is based on: ♦ Team Building and Planning: Beginning during pre- construction this step defines the Project Success Criteria. ♦ Organizing the Team: Structuring and staffing the team and integrating the various entities involved. It was a bridge across the FPL Discharge Canal that enabled cargo trucks to pass from Southport to Midport without having to leave the secured areas of the Port. It was a challenging project because all of the in -water work had to be completed within 6 months of the signing of the subcontract. That deadline was critical because Manatee season started at the end of that 6 -month period and no water work would have been allowed to continue until the following year. Kiewit managed to get FAA clearance (we were in the flight path of FLL) for their crane and were able to deliver the in -water work on time. They also finished the bridge well ahead of the project schedule. Moss would gladly team up with them on another project.
