Judicial Court, in and for Broward County. This case which was filed in 2006 was related to the Palm
Beach County Airport Parking Structure. The case was settled in August 2007.
2. Yvette Lorenzo, personal representative of the estate and survivors of Orestes Lorenzo vs. Florida
• Department of Transportation, City of Pembroke Pines, William J. Russo, Natasha Russo, and Calvin,
• Giordano&Associates, Inc., Case No: 0601924(12) in the 17th Judicial Court, in and for Broward County.
® CGA was a co-defendant in a wrongful death action brought by the deceased's relatives. This case was
filed on or about June 30, 2006, and was settled to the satisfaction of both parties in September 2008.

• 3. Vue Condominium Association, Inc.vs.Vue Fort Lauderdale, L.L.C., ET AL,Case No. CACE09012422 .Suit
was filed March 3, 2009. This is a typical condominium construction defect suit involving the
• developer,contractor, numerous subcontractors and all the design team. The condominium association
• voluntarily dismissed its claim against Calvin, Giordano February 2011.
• 4. Walnut Creek Community Development District vs. Standard Pacific of South Florida, GP, Inc., Et Al,
• Case No. CACE10019353. Suit was filed May 4, 2010. The case concerns the failure of an HDPE
• drainage system and is in the early stages of discovery. The other parties include the developer, the
pipe manufacturer and the contractor which is defunct but which may have insurance coverage. Our
• understanding is that the problems are primarily the result of defective manufacture of the HDPE pipe
although design and construction deficiencies also are alleged.
• 5. Armando Alvarez vs. Keith and Schnars, PA, Calvin, Giordano & Associates, Inc., Community Asphalt
• Corp., Arazoza Brothers Corporation, Arazoza Brothers Corporation f/k/a Aqualawn, Inc., Vila and Son
• Landscaping Corp., a Florida Corporation, Edward R. Wiener, Michael Viscount, Friendly Checker Cab
Co., Inc. d/b/a Friendly Checker Taxi, Case No. 08-042981-04. Suit was filed September 12, 2008 in the
• 17th Judicial Court, in and for Broward County. Motor vehicle accident involving a turning vehicle into
• path of a motorcycle caused by site line obstruction due to bush not properly maintained. CGA was
responsible for QC of roadway plans. This case is pending.
• 6. Calvin, Giordano & Associates and Franklin County, Florida filed lawsuit NO. 4:2012cv00001 in the
• Florida Northern District Court against B.P. Exploration and Production Inc. in January 2012. The suit
was filed in an attempt to collect payment from B.P. Exploration and Production for services performed
• by Calvin,Giordano&Associates on behalf of Franklin County, Florida.

• We began as M.E. Berry & Associates in 1937 and became Berry & Calvin in 1985. Twelve years later, the
transformation was made to Calvin, Giordano &Associates, Inc. (CGA). From the beginning, much of our
• reputation was built on repeat business with clients from the public and private sectors. Our success is
• reflected in the quality of work we consistently provide and also in the personal approach we take with
each client.The winning combination is affirmed by the fact that CGA grew steadily during an economy that
5 was not only challenging but also daunting to many larger firms in Florida these past few years. We focus
• on adding professionals and technology that address the needs of our South Florida customers instead of
• simply adding new branch office addresses.
• Founded in Hollywood, our main office is now in Fort Lauderdale, within Port Everglades. Today, our firm

• CAPITAL IMPROVEMENTS & ENGINEERING PLANNING
• CGA provides many forms of engineering planning services for municipalities infrastructure needs. It
• begins with assessments of the age, condition, and level of service of existing infrastructure, through
projection of the community's needs far into the future. Using the latest technology and accepted design

methodologies, CGA then develops master plans/programs to be implemented by the municipality to
• address the issues revealed in the evaluation process. We then prioritize and price the necessary
• improvements in order to assist the municipality in developing its infrastructure budget and also to assist
in obtaining alternative funding sources for the improvements. These infrastructure improvements vary
• widely in scope and can range from simple sidewalk and ADA improvements,to large complicated roadway
• and drainage projects. CGA currently assists the municipalities of Weston, West Park, Surfside, Sunny Isles
® Beach and other communities with their capital improvement programs. Various projects in the program
include various storm water drainage and pump stations, FP&L utility under-grounding, sea wall
• replacements, City Hall governmental facility, park facilities, roadway reconstruction and traffic calming,
• decorative street lighting, new parking lot construction, landscaping improvements and water and sewer

® Our water experience includes the design and permitting of new water distribution networks, water
• distribution system replacements and upgrades, and booster pump stations. This includes water main
extension and/or replacement projects for the Cities of Weston, Pembroke Pines, Miramar, North
® Lauderdale, Miami Beach, Dania Beach, Hollywood, Lauderhill, and the Towns of Davie, Surfside, and Bay
Harbor Islands. In addition, we have modeled existing water distribution systems for the Cities of Weston,
• and Lauderhill, as well as the Town of Davie. This involved assembling existing as-built, permit, and atlas
• data, and supplementing it with surveyed field data to build the existing model in WaterCad. CGA then
• proceeded to model improvements into the system for enhanced system performance, to account for future
demands, and to accommodate future anticipated redevelopment. The modeled improvements were
® prioritized, budgeted and included in the Capital Improvement Plans for the municipality.
• Our wastewater experience includes the design, permitting, and construction of collection systems,
• transmission systems, lift stations, master pump stations, in-line booster pumping stations, and force
• mains. CGA has or is currently in the process of modeling the existing wastewater transmission systems
• (lift stations and force mains) for the Cities of Weston, Pembroke Pines, Opa-Locka, Cooper City, Dania
Beach, and the Town of Davie. As with the water distribution system models, we assembled existing as-
built information, permit information, and atlas data, and supplemented it with surveyed field data to build
• the existing computer model. Similar to the water distribution system evaluation and analyses, CGA
• modeled improvements into the system for enhanced system performance, to account for future demands,
and to accommodate future anticipated redevelopment. The necessary improvements were then
• integrated in the municipality's Capital Improvement Plan. In addition, CGA recently performed an l&I
• study on the wastewater collection system of the Town of Surfside.
® Our team has provided designed pump stations for variety of application including storm water, shallow
wells, deep wells, wastewater lift station, wastewater effluent disposal, potable water distribution, and S in-
line booster station. As such, we have worked with and coordinated with all design disciplines including
structural, electrical, and instrumentation and controls. We have experience with a variety of pumps and
• pump station types. As part of the pump station design we also provided hydraulic modeling, solids
® removal facilities (i.e. bar racks, strainers) design and emergency power fuel system design. We have
worked on pumps with capacities ranging from 0.1 MGD to 200 MGD (320 CFS).

® The Surveying and Mapping Division of CGA utilizes the latest in cutting edge technology when preparing
surveys including GPS equipment, automated data collection, Laser Scanners, Underground utility
® detection technologies and a Vacuum/Slurry subsurface excavation system called the VaXcavator that
® exposes existing underground utilities. Operated by one of the field survey crews, it allows us to obtain
40 sub-surface horizontal and vertical locations required for design, while realizing cost and timesavings for
our clients. Additionally, our state-of-the-art Hypack software with Odom sounder allows us to offer
® hydrographic surveying services. By integrating with our Trimble GPS units, the equipment creates a three-
® dimensional model of the water body being surveyed and significant savings for our clients.
® Recognizing the value to our clients, CGA was among the first firms in Florida to acquire the 3D Laser
® ScanStation. Our Surveying and Mapping staff can now scan virtually anything in half the time of
® conventional methods,with a level of detail not available before. The ScanStation can take a virtual, detailed
snapshot of a site including ceilings, undersides of bridges, elevated pipe racks, tall facades, columns and
® towers. Areas that were previously difficult to access. It collects millions of data points of all items in a 360°
® horizontal and 270° vertical field of view, allowing a full field of view. This eliminates an additional visit to
O capture data not covered in the original scope of work or the question as to where a shot was taken of an
object. The ScanStation captures all the items at once in a permanent record that can be filed, including
® hundreds of high quality photos, and graphics or animations, from 3D to virtual walk-through of sites. It is
® ideal for litigations, client presentations or commission meetings. For added convenience, the survey data

The process we employ is equally as important as the equipment we deploy. All survey requests are
• assigned to a licensed surveyor and mapper who acts as the project manager. In addition, fieldwork is
• researched and calculated in the office for the crews prior to going out into the field. The required data is
® collected electronically using total station instruments, and then downloaded at the end of the day into our
computer system for analysis and processing. This data is then provided to our technicians to create the
• final product, all under the watchful eye of the project manager. The final product is delivered to the client

• CGA designed approximately 2,750 linear feet of 8-inch PVC water main to replace the Construction Management
O existing 2-inch and 6-inch diameter galvanized steel water mains that were near the end
O of their useful service life. The project included transferring more than 60 residential
water services and 6 connections to existing distribution mains. Installation of the new
O water main was completed via horizontal directional drilling with restrained joint C-
0 900 PVC pipe. By using horizontal directional drilling in lieu of traditional open-cut
construction methods, the City was able to install the water main with minimal impact

® CGA designed and permitted the design system improvements for the Hallandale Beach NE
® Quadrant Drainage Basin, which encompasses an area of approximately 170± acres. This tidally Services Provided
influenced basin is bounded by Atlantic Shores Boulevard, Federal Highway (US 1), Hallandale Drainage Study
® Beach Blvd (HBB), and NW 14th Avenue in the City of Hallandale Beach, Florida. Land uses pump Station& Drainage
® within the study area are primarily single family residential and multi-family residential. Well Design and Permitting
• There are a number of drainage facilities within the project area with limited conveyance
capacity and high head loss. Average lowest floor elevations average between 5.5' to 6.5' NGVD, Approx Construction Cost
• and roadway elevations are approximately 3.7' to 3.9' NGVD within the study area. The NE& SW Quadrants:
® conveyance system is subject to the effects of tidal fluctuation; and therefore at times there is S14 million
virtually no head available to drain the basin. The lack of conveyance capacity within this system
® has led to the complete inundation of the roadway system for extended periods during storm Consultant Cost
® events of lesser frequency than that required by SFWMD.
Using ICPR, CGA modeled a proposed system with an improved collection system conveying NE & SW Quadrants:
® water to two duplex pump stations connected to a total of sixteen wells.These sixteen new wells 5700,000
® will be pressurized with 15 feet of head in order to produce the necessary discharge into the G-
III aquifer. In addition, the existing collection system piping will be upsized in order to achieve Principal-in-Charge
® adequate flows into the proposed pump stations. The existing gravity outfalls will be left intact, Karl Kennedy,PE
® with the only modification being the installation of a 'backflow" flap gate in a City structure at
le the downstream end of the gravity system. The flap gate will stop the tidal water from flowing Project Manager
back into the gravity system at higher tides. The project is permitted through FDEP and the
• Broward County Environmental Resource Department. CGA, in conjunction with the City of Jenna Chamberlain, PE
Hallandale Beach, worked with FEMA to get an expansion approved for the existing HMGP grant
® for the NE Quadrant.. The expansion was approved to provide $15 million in funding for this Drainage Engineers

® The Town of Surfside was incorporated on May 18, 1935 and it encompasses a total land 2008-2009
area of approximately 330 acres. The Town is built out with no land for traditional
® treatment BMPs. Stormwater runoff flows down gridded streets from east to west with no Services Provided
® stormwater treatment facilities in the Town. Runoff from State Road AlA eastward
® enters FDOT storm drains and flow out to the Bay via pump station and six injection wells Engineering

area of approximately 330 acres. The Town's utility infrastructure has become dated and Surveying
® in need of replacement/rehabilitation. Surfside took the forward steps to direct CGA to Permitting
® design, permit, manage and inspect the complete rehabilitation of their water, sewer and GIS

® Sanitary Sewer Surfside's aging sanitary sewer collection system was suffering from Construction Inspection
® extensive inflow & Infiltration (I/I). This 1/I was unnecessarily causing Surfside to
expend funds to treat the inflowing ground water in addition to the town's sewage. CGA Principal in Charge
® performed investigative T.V. inspections of all existing sanitary sewer collection lines. Dennis J.Giordano
® These inspections were utilized to produce a final design of point repairs, grouting and
lining developed to eliminate the I/I. In addition to the rehabilitation of the main lines Project Administrator
® all pump stations,laterals, cleanouts and manholes were rehabilitated or replaced. Chris Giordano,SMC
® Storm Sewer CGA developed the Town of Surfside Stormwater Management Master
5 Plan to address water quality issues and reduce flooding within the Town. The Master Project Manager

® CGA was hired to design a new water main to replace the existing main in the residential
® neighborhood of East Dania Beach. CGA was contracted to provide surveying,
engineering, and construction services for this project. The service connections were
® replaced as well as connections to the existing mains on the cross streets and either end
O of the project. Coordination with the City allowed for the inclusion of a stub-out, valve
and plug for a future condominium connection as well. The contract scope entailed
® plans and specifications preparation, permitting, bidding and award, construction

® The overall project was to reconstruct the two existing storm water pumping stations in
® the Bonaventure Development District in the City of Weston.
Electrical Engineering services for this project consisted of the conversion of FPL
overhead 13 kV, 3-phase, to underground for a distance of 96' to accommodate the

• CGA designed the City of Hollywood 30th Ave Force Main and Gravity Sewer System
® between Hollywood Blvd. and Pembroke Road. The scope of the project included 3,000
® linear feet of new 8-inch PVC gravity sewer, eight sewer manholes, 3,500 linear feet of
4"-inch force main, 200 linear feet of 4-inch encased directional drill under the CSX rail

accommodate the construction of the new wastewater treatment plant on the University
® of Florida site. The existing service area for Lift Station No. 1 consists of residential and

smaller lift stations contribute sanitary flow to the gravity sewer system which flow into

® The new location for LS No. 1 is approximately 50 feet west of the existing LS No. 1. The

GO Calvin Giordano & Associates, Inc. (CGA) designed the new Lift Station No. 16 to replace the
® existing Lift Station No. 16 in the Windmill Ranches Subdivision of Weston. This station serves a Francisco Reina
® small amount of commercial area and also a portion of the Windmill Ranch Subdivision via a low Sabrina Baglieri
pressure vacuum system.The existing station consisted of a four-foot diameter 20-foot deep wet
® well with two 13 HP submersible pumps.This was upgraded to a six-foot diameter wet well with Construction Cost
® two 20 HP submersible pumps, a complete new control panel, new piping, valves, valve box, $277,000
wiring,and conduit.
® CGA designed the lift station and permitted it through Broward County EPD and the State of CGA Fees
® Florida DEP. We also bid the project for the City, administered the contract and performed $56,000

® boundaries that encompass approximately 4,400 acres. The City has reached Bidding
® development maturity and is focused on maintaining the existing infrastructure.Although
the City does not operate a water or wastewater treatment, they do manage the water Principal in Charge

• Sewage Pumping Station Design - Directed the design of new and replacement pumping stations using
• both dry pit pump designs and submersible pump designs from 2.5 Hp to 175 Hp pumps. Designs
• included new concrete valve vaults, discharge piping, new concrete pads, fencing and landscape. The
existing wet wells and receiving manhole were structurally rehabilitated. Stations included electrical
• control panels as well as some received separate emergency back-up generators.
• • Can Pump Station Conversions - Directed the design of many replacement of existing pumps in the
• adjacent dry well with new submersible, non-clog pumps in the wet well. The wet wells are generally
rehabilitated to prevent infiltration unless the wet well is not of sufficient capacity and then the wet
• well is converted to the terminal manhole and a new wet well is designed adjacent to the existing

submersible pumps, emergency electrical connections and site landscaping.

® quickly. We also acknowledge the major stakeholder in any CIP project is the residents. Recently, CGA has
utilized our in house Data Development/Creative Design Staff to create a project specific website. These
O sites allow the residents and other stakeholders to access up to date information in an easy to use graphical
interface 24 hours per day, 7 days per week.
® This step will begin immediately with the City, but will not begin with the residents until all information
® has been submitted to the City for review and approval prior to becoming public information. We are
® aware of the delicate nature of work being performed within neighborhoods and public right of ways and
would not want to jeopardize the relationship between the City and its residents. This step will also
® continue through project completion.
Project Implementation
® The scope of work for this section will vary project to project. What will remain constant is CGA's
® aggressive nature in expediting any portion of the CIP arena. Again, CGA has a vast experience with local
® municipalities and we are aware of the sensitive nature that is CIP budgets & schedules. Many
stakeholders, residents and elected officials are watching these projects from their homes on a daily basis
® and this brings with it extra pressure to perform. CGA's main office is located in South Florida and much of
® our staff is in the Sunny Isles Beach area on a daily basis. This will help us facilitate our aggressive
schedule. Throughout the process weekly progress meetings will be held to ensure that every entity
O involved is up to speed and all loose ends are being followed up.
Record Keeping&Project Close Out
® CGA has served as a representative for numerous South Florida municipalities. Throughout this time we
® have proven our knowledge of record retention laws, a very important issue for audits and public record
0 requests. In fact, the City of Weston felt so highly of our organizational skills and knowledge of record
retention law, we were hired to remove, reorganize and re-file all of their building department files. All
® relevant files will be kept at CGA's main office where our file room is fire suppressed and our servers are
