Hines, MMC-City Clerk 18070 Collins Ave, 4m Floor Sunny Isles Beach, FI 33160 RE: Bid No. 16-08-01 Event Related Equipment Rental Good afternoon: Thank you for providing Elite Tent Company the opportunity to submit pricing and terms for the bid referenced above. This letter is to confirm that we will honor the pricing, terms, and conditions as set forth in Invitation to Bid 16-08-01 to provide tents and accessories for the City's various events on an as needed basis through the contract term. Enclosed please find a copy of a item and pricing spreadsheet, representing Bid Form 3. All pricing indicates the City discount off of the List based upon tents being staked into the ground. In the event staking is not an option, concrete buckets or water barrels have been listed and will be additional to any estimate required. Below please find responses to several of the items referenced in the bid, ie. Responses to certain terms, conditions, or requirements, references etc. Section 1 .23 Permits. When a Building Permit and Miami-Dade Are Permit are required to be filed a $350 Processing Fee, plus actual fees incurred to be reimbursed by the City at Cost shall be added to final invoice or additionally invoiced. When possible Elite to be provided a minimum of 30 days advance notice for the preparation and filing of all permit related paperwork. In the event permit applications must be filed in a period less than 30 days, an expedited fee of $150-$250 may apply. If ONLY a Miami-Dade Fire Plan Review & Inspection are required to be filed a $150 Processing Fee, plus actual fees incurred to be reimbursed by the City at Cost shall be added to final invoice or additionally invoiced. • BID NO. 16-08-01 www.eiitetent.com • Page 2 Section 1 .23 Insurance Please see attached "current" certificate of insurance. Our policy expires/renews 11/1/16. Immediately following renewal, an updated certificate shall be provided. Section 2.5 References Elite has been the provider of tents, tables, chairs, and other related equipment for the City of Aventura's annual Founders Day Event since 2007 to present. In addition to this event, Elite provides similar equipment for any/all City related events. Section 2.10 Background Checks&Fingerprinting Background Checks& Fingerprinting of Full-Time Employees assigned to work at City of Sunny Isles event installations/breakdowns shall be coordinated and conducted by the City's Human Resources Department,through Laura Covill,once bid contract has been awarded. Additionally, Bite shall provide the City with a Certificate of Insurance,listing the City as additional insured,by any Temporary Labor Service,hired by Bite,for work on City events. BID NO. 16-08-01 www.eiitetent.com • Page 4 Special/Unknown Delivery Requirements In the event a "new" site location is to be utilized,where load in/out conditions require additional labor due to limited vehicle access or proximity to work area from delivery vehicle is at issue additional fees will be calculated according to the following. 1. $50 per extra 100' feet from vehical access. 2. $100 per extra personal required to load in/out. 3. $.20 per s/f for structures installed on the beach in sand.
