A RESOLUTION OF THE CITY COMMISSION OF THE CITY
OF SUNNY ISLES BEACH, FLORIDA, ADOPTING AN IRS
SECTION 125 CAFETERIA PLAN AS SET FORTH IN EXHIBIT
"A"; AUTHORIZING THE CITY MANAGER TO TAKE ANY
AND ALL ACTION AS MAY BE NECESSARY TO EFFECTUATE
THE RESOLUTION; PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the City desires to allow its employees to participate in supplemental health
and dependent care flexible spending account benefits as set forth in the attached City of Sunny
Isles Beach Florida Cafeteria Plan (the "Plan"); and
WHEREAS, the City wishes to adopt a flexible benefits plan that complies with Section
125 of the Internal Revenue Code to enable its employees to choose between qualified benefits
under the Plan on a payroll reduction basis; and
WHEREAS, this Plan will not cost the taxpayers of the City of Sunny Isles Beach any
additional City taxes;
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
1. The City hereby adopts the City of Sunny Isles Beach flexible benefits plan in accordance
with Exhibit "A" as may be amended and restated, effective as of January 1, 2000.
2. The City Manager of the City of Sunny Isles Beach Florida is authorized and directed to
take any and all action as may be necessary to effectuate this Resolution.
3. This Resolution shall is effective upon passage.

EXPENSES. THESE EXPENSES MUST BE INCURRED TO ALLOW YOU and YOUR SPOUSE TO WORK, UNLESS
YOUR SPOUSE IS A FULL-TIME STUDENT or INCAPABLE OF SELF-CARE.
