A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, APPROVING AN
AGREEMENT A WARDING BID NO. 04-04-01 TO JADE
COMMUNICATIONS INC., FOR CATEGORY 6
STRUCTURED CABLING SYSTEM FOR THE SUNNY ISLES
BEACH GOVERNMENT CENTER, IN AN AMOUNT OF
$111,368.01, ATTACHED HERETO AS EXHIBIT "A";
AUTHORIZING THE MAYOR TO EXECUTE SAID
AGREEMENT; AUTHORIZING THE CITY MANAGER TO
DO ALL THINGS NECESSARY TO EFFECTUATE THE
TERMS OF THE AGREEMENT; PROVIDING FOR AN
EFFECTIVE DATE.
WHEREAS, the City issued Bid No. 04-04-01 for Category 6 Structured Cabling System for
the Sunny Isles Beach Government Center; and
WHEREAS, the City held a Pre-Bid Conference on April 26, 2004, resulting in twelve (12)
bids being received; and
WHEREAS, the City has determined that Jade Communications, Inc., was the most
responsive, responsible bidder; and
WHEREAS, the City wishes to contract with Jade Communications, Inc. for the Category 6
Structured Cabling System for the Sunny Isles Beach Government Center, in an amount of
$111,368.01, attached hereto as Attachment "A".
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Approval of Agreement. The Agreement by and between the City of Sunny Isles
Beach and Jade Communications, Inc. for the Category 6 Structured Cabling System for the Sunny
Isles Beach Government Center, in the amount of $111 ,368.01, attached hereto as Attachment "A",
be, and the same, is hereby approved.
Section 2.
Authorization of Mayor. The Mayor is hereby authorized to execute said Agreement.
Section 3. Authorization of City Manager. The City Manager is hereby authorized to do all
things necessary to effectuate the terms of the Agreement.
Section 4.
Effective Date. This Resolution will become effective upon adoption.

CONSULTANT AGREEMENT
CONTRACT NO. 04/04/01
Ji THIS CONSULTANT AGREEMENT made and entered into this "2>vJ.. day of
lMta.-- 2004, by and between the CITY OF SUNNY ISLES BEACH, FLORIDA, a
municipal corporation of the State of Florida, (hereinafter referred as to "City") and JADE
COMMUNICATIONS, INC., a Florida corporation, authorized to do business in the State of
Florida, (hereinafter referred to as "Consultant"), whose Federal LD.# is ESnnnnAAJ2/. 65-0002108

WHEREAS, JADE COMMUNICATIONS, INC, is a qualified, engineering consultant
firm; and
WHEREAS, the City wishes to employ Consultant to install Local Area Network for the
Sunny Isles Beach Government Center Project as well as other tasks as more specifically
described in Exhibit "A," attached hereto and made a part hereof; and,
WHEREAS, the Consultant is qualified, willing and able to provide the desired services
on the terms and conditions set forth herein;
NOW THERFORE, in consideration of the premises and the mutual covenants herein
named, the parties hereto agree as set forth below:
1. RECIT ALS. The Recitals set forth above are hereby incorporated into this Agreement
and made a part of hereof for reference.
2. SERVICES TO BE PERFORMED. Consultant shall perform the following services:
a. Category 6 structured cabling system, for the City of Sunny Isles Beach, Government
Center, as well as those services more particularly described in Exhibit "A" attached
hereto and made a part hereof.
b. Consultant will prepare and present oral/written reports as needed or as requested on a
periodic basis while engaged in special projects. Consultant shall provide, on a monthly
basis, to the City Manager, a detailed narrative report delineating all services provided by
the Consultant in the previous month. The Consultant shall also provide quarterly,
written reports, detailing all of the Consultant's activities in the previous quarter and the
results of the Consultant's efforts. Each report required herein will be submitted to the
attention of the City Manager.
c. The total price for these services is ONE HUNDRED AND ELEVEN THOUSAND,
THREE HUNDRED SIXTY EIGHT, AND 011100 DOLLARS ($111,368.01), payable
as invoiced.
3. TERM. Subject to the provisions relating to the termination of this Agreement as set
forth in Paragraph 10 hereunder, this agreement shall commence upon execution of this
Agreement and shall expire upon the completion of services rendered in Exhibit "A". Any
change in fee, terms or conditions shall be accomplished by written amendment to this contract.

4. COMPENSATION. Payment to Consultant for all charges and tasks under this
Agreement shall be in accordance with this Agreement and the schedule of charges reflected in
Exhibit "A", which fee shall be disbursed on a monthly basis and under the following conditions:
a. Disbursements. There are no reimbursable expenses associated with this contract.
b. Payment Schedule. Invoices received from the Consultant pursuant to this
Agreement will be reviewed by the initiating City Department. If services have
been rendered in conformity with the Agreement, the invoice will be sent to the
Finance Department for payment. Invoices must reference the contract number
assigned hereto. Invoices will be paid in accordance with the State of Florida
Prompt Payment Act.
c. Availability of Funds. The City's performance and obligation to pay under this
Agreement is contingent upon an annual appropriation for its purpose by the City
Commission.
d. Final Invoice. In order for both parties herein to close their books and records,
the Consultant will clearly state "final invoice" on the Consultant's final/last
billing to the City. This certifies that all services have been properly performed
and all charges and costs have been invoiced to the City. Since this account will
thereupon be closed, any and other additional charges, if not properly included on
this final invoice are waived by the Consultant.
Consultant shall make no other charges to the City for supplies, labor, taxes, licenses,
permits, overhead or any other expenses or costs unless any such expense or cost is incurred by
Consultant with the prior written approval of the City. If the City disputes any charges on the
invoices, it may make payment of the uncontested amounts and withhold payment on the
contested amounts until they are resolved by agreement with Consultant.
Consultant shall not pledge the City's credit or make it a guarantor of payment or surety
for any contract, debt, obligation, judgment, lien, or any form of indebtedness. The Consultant
further warrants and represents that it has no obligation or indebtedness that would impair its
ability to fulfill the terms of this Agreement.
5. UNDISCLOSED CONDITIONS. In the event that undisclosed conditions are
discovered during the performance of this Agreement, the City shall have the right to cancel this
Agreement upon thirty (30) days written notice to Consultant. Upon termination, the City may
bid/re-bid the project if the Consultant fails to perform under this Agreement due to the
undisclosed conditions.
6. INDEPENDENT CONSULTANT RELATIONSHIP. The Consultant is an
independent Consultant and shall be treated as such for all purposes. Nothing contained in this
agreement or any action of the parties shall be construed to constitute or to render the Consultant

an employee, partner, agent, shareholder, officer or in any other capacity other than as an
independent Consultant other than those obligations which have been or shall have been
undertaken by the City, Consultant shall be responsible for any and all of its own expenses in
performing its duties as contemplated under this agreement. The City shall not be responsible
for any expense incurred by the Consultant. The City shall have no duty to withhold any Federal
income taxes or pay Social Security services and that such obligations shall be that of the
Consultant, other than those set forth in this agreement. Consultant shall furnish its own
transportation, office and other supplies as it determines necessary in carrying out its duties
under this agreement.
7. INSURANCE. Consultant shall, at its sole cost and expense, during the period of any
work being performed under this Agreement, procure and maintain the following minimum
insurance coverage to protect the City and Consultant against all loss, claims, damage and
liabilities caused by Consultant, its agents, sub-Consultants or employees, as indicated below:
o Comprehensive General liability insurance -- including broad form contractual
liability coverage for all operations, including, but not limited to, contractual,
products, and completed operations, personal injury and property damage liability
with limits of One Million Dollars ($1,000,000) combined single limit
occurrence.
o Workmen's Compensation, with no less than $100,000 for Employer's Liability.
Said coverage shall include a waiver of subrogation in favor of the City, its
agents, employees and officials.
o Errors and Omission -- Consultant shall carry a minimum of $500,000 coverage
for errors and omissions.
o Business Automobile Liability which shall include coverage for all owned, non-
owned and hired vehicles for limits of not less than $500,000 per occurrence,
Combined Single Limit or its equivalent.
Insurance required of the Consultant shall be primary to, and not contribute with, any
insurance or self-insurance maintained by the City.
Such insurance shall not diminish Consultant's indemnification and obligations
hereunder. The insurance policy(ies) shall be issued by companies authorized to do business
under the laws of the State of Florida and acceptable to the City. Before any work under this
Agreement is performed, and at any time upon request, Consultant shall furnish to the City
certificates of' insurance evidencing the minimum required coverage and shall be
appropriately endorsed for contractual liability, with the City named as additional insured.
All policies shall contain a waiver of subrogation endorsement. All policies and certificates shall
be in forms and issued by insurance companies acceptable to the City's Risk Management
Department. All insurance policies and certificates of insurance shall provide that the policies

may not be canceled or altered without thirty (30) days prior written notice to the City. The City
reserves the right from time to time to change the insurance coverage and limits of liability
required to be maintained by Consultant hereunder.
Consultant shall also require and ensure that each of its sub-consultants providing
services hereunder (if any) procures and maintains, until the completion of the services,
insurance of the types and to the limits specified herein.
ANY EXCEPTIONS TO THE INSURANCE REQUIREMENTS IN THIS
SECTION MUST BE APPROVED IN WRITING BY THE CITY.
8. OWNERSHIP OF DOCUMENTS AND EQUIPMENT. All documents prepared by
the Consultant pursuant to this agreement and related services to this agreement are intended and
represented for the ownership of the City only. Any other use by Consultant or other parties,
shall be approved in writing by the City.
Consultant shall deliver to the City for approval and acceptance, and before being eligible
for final payment or any amounts due, all documents and materials prepared by, and for, the City
under this Agreement.
All oral and written information not in the public domain or not previously known, and
all information and data obtained, developed or supplied by the City, or at its expense, will be
kept confidential by the Consultant and will not be disclosed to any other party, directly or
indirectly, without the City's prior written consent, unless required by a lawful order. All
drawings, maps, sketches, programs, data base, reports and other data developed or purchased
under this Agreement for, or at the City's expense, shall be and remain the City's property and
may be reproduced and reused at the discretion of the City.
The City and Consultant shall comply with the prOVISIOns of Chapter 119, Florida
Statutes (Public Records Law).
All covenants, agreements, representations and warranties made herein, or otherwise
made in writing by any party pursuant hereto, including, but not limited to, any representations
made herein relating to disclosure or ownership of documents, shall survive the execution and
delivery of this Agreement and the consummation of the transactions contemplated hereby.
9. INDEMNIFICATION. Consultant agrees to indemnify and hold harmless, the City, its
officers, agents, and employees from, and against any and all claims, actions, liabilities, losses
and expenses including, but not limited to, attorney's fees for personal, economic or bodily
injury, wrongful death, loss of or damage to property, at law or in equity, which may arise or
may be alleged to have risen from the negligent acts, errors, omissions or other wrongful conduct
of the Consultant, agents or other personal entity acting under Consultant's control in connection
with the Consultant's performance of services pursuant to that agreement and to that extent the
Consultant shall pay such claims and losses and shall pay all such costs and judgments which
may issue from any lawsuit arising from such claims and losses including wrongful termination

or allegations of discrimination or harassment, and shall pay all costs and attorneys' fees
expended by the City in defense of such claims and losses including appeals. The parties agree
that ten percent (10%) of the total compensation is a specific consideration from the City to the
Consultant for this indemnity.
10. TERMINATION.
a. If, through any cause within the reasonable control the Consultant shall fail to fulfill in a
timely manner or otherwise violate any of the covenants, agreements or stipulations material to
this agreement, the City shall have the right to terminate the services then remaining to be
performed. Prior to the exercise of its option to terminate for cause, the City shall notify the
Consultant of its violation of the particular terms of the agreement and grant Consultant ten (10)
days to cure such default. If the default remains uncured after ten (10) days the City may
terminate this agreement.
1. In the event of termination, all finished and unfinished documents, data
and other work product prepared by Consultant (and sub-Consultant( s])
shall be delivered to the City and the City shall compensate the Consultant
for all services satisfactorily performed prior to the date of termination, as
provided in Paragraph 4 herein.
11. Notwithstanding the foregoing, the Consultant shall not be relieved of
liability to the City for damages sustained by it by virtue of a breach of the
agreement by Consultant and the City may reasonably withhold payments
to Consultant for the purposes of set-off until such time as the exact
amount of damages due the City from the Consultant is determined.
111. After receipt of a Termination Notice and except as otherwise directed by
the City, the Consultant shall:
1. Stop work on the date and to the extent specified.
2. Terminate and settle all orders and subcontracts relating to the
performance of the terminated work.
3. Transfer all work in process, completed work and other
materials related to the terminated work to the City.
4. Continue and complete all parts of that work that have not been
terminated.
b. Termination for Convenience of City. The City may, for its convenience and
without cause terminate the services then remaining to be performed in the event the Consultant
is placed either in voluntary of involuntary bankruptcy or makes any assignment for the benefit
of creditors.

c. Termination for Insolvencv. The City also reserves the right to terminate the
remaining services to be performed in the event the Consultant is placed either in voluntary or
involuntary bankruptcy or makes any assignment for the benefit of creditors.
11. ASSIGNMENTS. TRANSFERS. SUBCONTRACTING. The Consultant shall not
subcontract, assign or transfer any work under this agreement without the prior written consent
of the City. Should the Consultant subcontract any services under this agreement, it shall be
done with continued liability for the Consultant. The Consultant shall remain responsible for
services, responsibilities and liabilities of any person or entity acting under Consultant.
12. TIME OF COMPLETION. The services to be rendered by the Consultant shall be
commenced upon execution of this contract and shall be completed within a reasonable time. A
reasonable extension of time shall be granted in the event the work of the Consultant is delayed
or prevented by the City or by circumstances beyond the reasonable control of the Consultant
including weather conditions of acts of God which render the performance of the Consultant's
duty impracticable.
13. AUTHORITY TO PRACTICE. Consultant hereby represents and warrants that it has
and will continue to maintain all licenses and approvals required to conduct its business and that
it will at all times conduct its business activities in a reputable manner. Proof of such licenses
and approvals shall be submitted to the City prior to commencement of work under this
Agreement.
14. MODIFICATIONS OF WORK. The City reserves the right to make changes in the
work, including alterations, reductions or additions thereto. Upon receipt of the City's
notification of a contemplated change, the Consultant shall (1) if requested by the City, provide
an estimate for the increase or decrease in cost due to the contemplated change, (2) notify the
City of any estimated change in completion date, and (3) advise the City in writing if the
contemplated change shall affect the Consultant's ability to meet the completion dates or
schedules of this Agreement.
15. COORDINATION OF SERVICES. The City's representative/liaison during the
performance of this Agreement shall be Derrick Arias, telephone number 305/947-0606.
Consultant shall not respond to requests for services under this Agreement unless the request is
received directly from Derrick Arias or Christopher J. Russo, City Manager, or designated
personnel. Any requests received from other City departments/divisions shall be referred to the
City Manager's Department representative designated above. Services performed without
authorization shall be considered unauthorized and shall not be compensated/paid by the
City.
16. ATTORNEY'S FEES. If this matter is placed in the hands of an attorney for
collection, or in the event suit or action is instituted to enforce any of the terms or conditions of
this agreement, the losing party shall pay to the prevailing party, in such suit or action in both
trial court and appellate court, together with reasonable attorney's fees for the anticipated cost of
collection and judgment enforcement.

17. CONFIDENTIAL INFORMATION. The Consultant shall not, either during the term
of this Agreement or any time for a period of Ten (10) years subsequent to that date upon which
the Consultant shall leave the employment of the City for any reason whatsoever, disclose to any
person or entity, other than in the discharge of the duties of the Consultant under this Agreement,
any information which the City designates in writing as "confidential." As a violation by the
Consultant of the provisions of this Section could cause irreparable injury to the City and there is
no adequate remedy at law for such violation, the City shall have the right, in addition to any
other remedies available to it at law or in equity, to enjoin the Consultant in a court of equity for
violating such provisions.
18. NOTICES. All notices and communications hereunder shall be in writing and shall be
deemed given when sent postage prepaid by registered or certified mail, return receipt requested
and, if intended for City to Christopher J. Russo, City Manager, with a copy to Lynn M.
Dannheisser, City Attorney, City of Sunny Isles Beach, 17070 Collins Avenue, Suite 250,
Sunny Isles Beach, Florjda 33160, and, if intended for Consultant, shall be addressed to Jade
Communications, Inc., 6610 E. Rogers Circle, Boca Raton, FL 33487 Attention: C. Michael
Kelsey.
19. GOVERNING LAW. The validity of this Agreement and the interpretation and
performance of all of its terms shall be construed and enforced in accordance with the laws of the
State of Florida, without regard to principles of conflict of laws thereof. The location of any
action or proceeding commenced under or pursuant to this Agreement shall be in Miami-Dade
County, in the State of Florida.
20. AUDIT. The Consultant shall make available to the City or its representative all required
financial records associated with the Agreement for a period of three (3) years.
21. NON-DISCRIMINATION. The Consultant agrees to comply with all local and state
civil rights ordinances and with Title VI of the Civil Rights Act of 1984 as amended, Title VIII
of the CiviL Rights Act of 1968 as amended, Title I of the Housing and Community Development
Act of 1974 as amended, Section 504 of the Rehabilitation Act of 1973, the Americans with
Disabilities Act of 1990, the Age Discrimination Act of 1975, Executive Order 11063, and with
Executive Order 11248 as amended by Executive Orders 11375 and 12086.
The Consultant will not discriminate against any employee or applicant for employment because
of race, color, creed, religion, ancestry, national origin, sex, disability or other handicap, age,
marital/family status, or status with regard to public assistance. The Consultant will take
affirmative action to insure that all employment practices are free from such discrimination.
Such employment practices include but are not limited to the following: hiring, upgrading,
demotion, transfer, recruitment or recruitment advertising, layoff, termination, rates of payor
other forms of compensation, and selection for training, including apprenticeship. The
Consultant agrees to post in conspicuous places, available to employees and applicants for
employment, notices to be provided by the City setting forth the provisions of this non-
discrimination clause.

The Consultant agrees to comply with any Federal regulations issued pursuant to compliance
with Section 504 of the Rehabilitation Act of 1973 (29 U.S.C. 708), which prohibits
discrimination against the handicapped in any Federally assisted program.
22. CONFLICT OF INTEREST. The Consultant agrees to adhere to and be governed by
the Miami-Dade County Conflict of Interest Ordinance Section 2-11.1, as amended; and by
Sunny Isles Beach Ordinance No. 99-82, which are incorporated by reference herein as if fully
set forth herein, in connection with the Agreement conditions hereunder.
Consultant covenants that it presently has no interest and shall not acquire any interest, direct or
indirectly that should conflict in any manner or degree with the performance of the services.
23. CONFLICTING PROVISIONS. The terms and conditions in this agreement supersede
any other conflicting provisions that are contained in any other document.
24. MISCELLANEOUS.
a. The rights granted to Consultant hereunder are nonexclusive, and the City reserves the
right to enter into agreements with other persons or firms to perform services including those
provided hereunder.
b. Consultant and its employees shall promptly observe and comply with applicable
provisions of all published federal, state and local laws, rules and regulations which govern or
apply to the services rendered by Consultant hereunder, or to the wages paid by Consultant to its
employees.
c. Consultant shall obtain and keep in force during the term of this Agreement all necessary
licenses, registrations, certificates, permits and other authorizations as are required by law in
order for Consultant to render the service(s) required hereunder.
d. Consultant shall not use the name or official seal of the City in any promotional material
without the prior written consent of the City.
e. Except as expressly provided for in this Agreement, Consultant is not authorized to act as
the City's Agent hereunder and shall have no authority, expressed or implied, to act for or bind
the City hereunder, either in Consultant's relations with sub-Consultants, or in any other manner
whatsoever.
25. ENTIRE AGREEMENT. This Agreement and Attachment Exhibit "A," which is
expressly incorporated herein by reference, shall constitute the entire agreement between the
parties with respect to the subject matter hereof, and it shall supersede all previous and
contemporaneous oral and written negotiations, commitments, agreements and understanding
relating hereto. This Agreement may be executed in multiple identical counterparts, each of
which shall be deemed an original for all purposes.

26. AMENDMENT. Any modification of this Agreement shall be effective only if in
writing and signed by the parties to this Agreement. No waiver of any provision of this
Agreement shall be valid or enforceable unless such waiver is in writing and signed by the
parting granting such waiver.

1 INTRODUCTION
1.1 GENERAL BACKGROUND
This document is issued as a Request for Proposal ("RFP") for the supply and installation of a
structured telecommunications cabling system for the Sunny Isles Beach Government Center,
The structured cabling system will support voice, data, and imaging applications within the facility
located at 18070 Collins Avenue, Sunny Isles Beach, FL 33160.
This document describes the system requirements to be met in the proposals of the
telecommunications cabling vendors to secure under contract all materials, design engineering,
installation, and supervision and training services for the structured cabling system.
1.2 TERMS AND CONDITIONS OF BIDS
This is an invitation to submit a bid based on the materials, systems and equipment described in this
document.
All bids must be submitted in accordance with the specifications and information contained herein, as
well as with any addenda, if required, issued by the purchaser.
It is the intent of the Drawings and Specifications to provide a complete workable telecommunication
cabling system ready for the Owner's use. Any item not specifically shown on the Drawings or called
for in the Specification, but normally required for a complete system, are to be considered a part of
the contract.
Consideration other than cost alone will be used in making the determination of the successful
contractor. These factors will include financial stability, availability, design support, project
management and field supervision.
The Manufacturers and Products specified in the document are to be used. No substitutions of
components specifically referenced will be allowed, without prior written customer consent after
submittal review
The City reserves the right in it's sole and absolute discretion to reject any and all qualifiers, to cancel
and/or withdraw the RFP at any time.
The City reserves the right to award any contract to any respondee which it deems to offer the best
overall service and be the most responsible responsive bidder, thereby the City is not bound to award
any contract based solely on the lowest quoted price and no contract is created until it is signed with
the approval of the City Commission. The City, in it's sole and absolute discretion, also reserves the
right to negotiate separately with competing firms as it deems to be in the best interest of the City and
as permitted by law.
If only one responsive qualifier for commodity or contractual service is received, in response to the
solicitation, an award may be made to the single qualifier, if the City Manager finds the price
submitted is fair and reasonable, and that other prospective bidders had reasonable opportunity to
respond, or there is not adequate time for re-solicitation. Further, the City Manager reserves the
right, if it is in the best interests of the City,to negotiate with the sole bidder for the best terms,
conditions, and price. The City Manager shall document the reasons that such action is in the best
interests of the City. Otherwise, the qualification may be rejected and:
· New qualifiers may be solicited; and/or
· The sole qualifier may be rejected.
The respondee is responsible for paying any and all taxes associated with the agreement.
Section 287,133, Florida Statutes, provides that a person or affiliate, as defined in that section, who
has been placed on the convicted vendor list following the conviction for a public entity crime may not
submit a bid on a contract with a public entity for any contract for construction or repair of a public
building or a public work, may not submit bids on leases of real property to a public entity, may not be
awarded to perform work as a contractor, supplier, subcontractor, or consultant under a contract with
any public entity, and may not transact business with any public entity in excess of the threshold of
the amount provided in section 287,017 for CATEGORY TWO ($25,000) for a period of 36 months
from the date of being placed on the convicted vendor list. Any such person or affiliate convicted of a
public entity crime shall provide written notice of such conviction.

1.3 INSTRUCTIONS TO THE BIDDER
Bids shall be valid for 90 days.
The Bidder shall consider the nature and amount of work to be done as well as the difficulties
involved in its proper execution.
No bid will be accepted by any contractor who did not attend the scheduled mandatory site survey,
unless this requirement is waived by the City.
The bid shall include all costs deemed necessary to cover all contingencies essential to the
installation of the specified system.
Total cost for installation materials, labor project management, permit fees, taxes and other
miscellaneous items must be listed separately.
A complete materials list, including description, manufacturer, part number, quantity, unit price and
total price must also be included.
All products and materials shall be new, clean, free of defects and free of damage and corrosion,
Where discrepancies are found during the bid process, the most stringent requirements must be
included in the bid.
Any cost encountered. which is not specifically itemized in the bid, shall not be incurred unless
specifically agreed upon, in writing.
No additional compensation will be allowed for extra work incurred on the part of the Contractor due
to the contractor's failure to notice any existing condition, which may cause the additional labor.
Bid responses shall be concise following the format and numbering of this specification. Items not
requiring responses shall be acknowledged by the bidder as being understood,
Bidders must notify the Purchaser as soon as detected any omissions or errors in the specification so
corrective addenda may be issued. Such notification must be received by the Purchaser at least (10)
days prior to the date for receipt of bids.
Sealed bids will be accepted by the City Clerk at City Hall (17070 Collins Avenue Suite 250, Sunny
Isles Beach, FL 33160) by the deadline listed in section 1.5 below. Bids received after the deadline
will be returned unopened.
Inquiries and requests for clarifications should be submitted to Derrick Arias, IT Director, in writing at
17070 Collins Avenue, Suite 250, Sunny Isles Beach, FL 33160 by the deadline listed in section 1.5
below.
1.4 RIGHTS OF THE PURCHASER
The Purchaser reserves the right to accept any bid or, at its sole discretion, reject any or all bids for
whatever reason it deems appropriate.
The Purchaser reserves the right to purchase ALL or PART of the cabling materials and hardware
needed for the project.
Receipt of a bid response does not obligate the Purchaser to pay any expenses incurred by the
bidder in preparation of the bid response or obligate the Purchaser in any other respect.
The Purchaser reserves the right to modify the specifications contained in the Request for Quote at
any time during the bidding period,
Only changes issued as an addendum will be binding upon the Purchaser. No verbal instructions or
interpretations of requirements shall be accepted,

The schedule below indicates the critical dates that must be satisfied by the Contractor. The
Contractor must staff its work crews appropriately to meet the required dates of completion, The
Contractor shall also have staff available to provide coverage during the cut over period of ( 2) days,

2 QUALITY ASSURANCE
2.1 CONTRACTOR QUALIFICATIONS
The Contractor shall at a minimum possess the following qualifications:
Be in business a minimum of five (5) years,
Contractor shall demonstrate satisfaction of sound financial condition and can be adequately bonded
and insured if the project deems necessary.
Possess those licenses/permits required to perform telecommunications installations in the specified
jurisdiction.
Personnel knowledgeable in local, state, and national codes and regulations. All work shall comply
with the latest revision of the codes or regulations. When conflict exists between local or national
codes or regulations, the most stringent codes or regulations shall be followed.
Must possess current liability insurance certificates.
It is preferable that contractor be registered with BICSI and have at least one RCDD on staff.
Must have personnel fluent in the use of Computer Aided Design and possess and operate CAD
software using .DWG or .DXF format.
2.2 REQUIRED CONTRACTOR TRAINING
The Contractor shall be fully conversant and capable in the cabling of low voltage applications such
as, but not limited to data, voice and imaging network systems. The Contractor shall at a minimum
possess the following qualifications:
Personnel trained and certified in the design of Category 6 Cabling systems.
Personnel trained and certified to install the Category 6 Cabling systems.
Provide references of the type of installation provide in this specification.
Personnel trained and certified in fiber optic cabling, splicing, termination and testing techniques.
Personnel must have experience using a light meter and OTDR.
Personnel trained in the installation of pathways and support for housing horizontal and backbone
cabling.
2.3 CONTRACTOR RESPONSIBILITY
Contractor shall be obligated to exercise the highest standard of care in performing it's obligations as
defined in this request for proposal.
Contractor acknowledges that The City of Sunny Isles Beach will rely on contractor's expertise, ability
and knowledge of the system being proposed and shall be obligated to exercise the highest of
standard care in performing it's obligation as defined in the following Scope of Work,
2.4 MANUFACTURER QUALITY & PRODUCT SUBSTITUTIONS
All telecommunications connecting hardware and cable must be made by an ISO 9001 Certified
Manufacturer.
All products must meet the technical requirements listed in sections 6-8. Any products not meeting
these requirements will not be considered,

4 GENERAL REQUIREMENTS
Responsive bidders are to provide the following:
· Insurance Certificate
· Contractor must have workman's compensation insurance in the state in which work is
performed.
· The Contractor should be prepared to furnish surety, performance and completion bonds if
required.
· Liability
· Proof of Manufacturers Certification providing City with a minimum 25 years warranty.

The Structured Cabling System Shall consist of anyone or all of the following Elements:
· Work-Area Cabling
· Horizontal Cabling
· Backbone Fiber Optic & Copper Cabling
· System shall be capable of supporting Voice, Data, and Video.
· System shall consist of Unshielded Twisted Pair (UTP) Cable installed in a star topology.
It is the Contractors responsibility to maintain cable lengths under the 295 ft. distance limitation.
All terminations shall be required to be warranted by Manufacturer, Contractor shall issue a letter of
registration prior to start of project to Customer assuring proposed system is in accordance,
Contractor shall have a permanent office within 50 miles of the project & be available for after hours
emergency service & repair.
Contractor must own all equipment, testers & installation tools required for installation of system.
Contractor must perform installation with own manpower, no subcontracting of any portion of this

5 SCOPE OF WORK
The proposed Cabling System is to be a Category 6 fully compliant & operating system.
5.1 COPPER BACKBONE CABLING
Copper backbone cabling shall consist of 100 150 pair Category 5e riser rated copper cable.
Contractor will install a total of (100) pairs of copper cable between MPOE 1-022 (demark) & the
MDF on the 3rd floor, (100) pairs of copper cable between MPOE 1-022 (demark) & the PD
Computer room 2-020 on the 2nd floor, (100) pairs of copper cable between PD Computer room 2-
020 & the MDF on the 3rd floor, (50) pairs of copper cable between TELCO CLOSET 2-009 & the
MDF on the 3'd floor, (50) pairs of copper cable between TELCO CLOSET 3-009 & the MDF on the
3rd floor, (50) pairs of copper cable between TELCO CLOSET 4-009 & the MDF on the 3rd floor, &
(50) pairs of copper cable between TELCO CLOSET 1-037 (council chambers) & the MDF on the
3rd floor. The Backbone cables will be terminated onto 300 pair 110 termination blocks. The
contractor will then install tails from the blocks onto 96 port Cat 5e Patch panels to facilitate patching
of extensions by customer Telecom Personnel. Pair terminations will be 1 pair per port on panels.
All necessary wire management shall be installed for a complete system.
5.2 CATEGORY 6 HORIZONTAL CABLING
All outlets shall consist of Quad (4) Category 6 Plenum cables installed onto RJ-45 category 6
Outlets. All cable is to be bundled neatly in the ceiling with the use of J-hooks. . All cable will be
terminated onto 48 port Patch panels in the appropriate IDF. Yellow patch cables shall be provided
for port (4) of every location, of sufficient length to connect to data switch at proposed location (as
shown in diagrams - see section 14 of the RFP), Orange patch cables shall be provided for port (3)
of every location, of sufficient length to connect to data switch at proposed location (as shown in
diagrams - see section 14 of this RFP),
5.3 CLOSET TERMINATION
AIIIDF closets will be fitted with (1) 19"xT Data Racks. All Horizontal cables will terminate onto 48
port Category 6 patch panels. It is the contractor's responsibility to provide sufficient panels & wire
management hardware for a complete system. Racks will be equipped with horizontal & vertical
cable management as well as ladder rack for a complete system. All racks to be supplied by
contractor Cabinets to be provided by Customer.
5.4 WORK AREA
All data outlets shall be supplied with two (2) 10 foot patch category 6 patch cables, one yellow color
and one gray color.
5.5 FIBER BACKBONE
Fiber backbone cabling shall consist of 12 strands of multimode (62.5/125 um) plus 6 strands of
single mode (9/125 um) fiber. Contractor will install fiber backbone between the PO Computer Room
2-020 and the MDF on the 3rd floor, between the telco closet 2-009 and the MDF on the 3m floor,
between the telco closet 4-009 and the MDF on the 3'd floor, and between the telco closet 1-037
(council chambers) and the MDF on the 3rd floor. In addition, contractor shall supply fiber patch
cables for each endpoint as follows:
· Telco closet 4-009: (2) SC-LC 2 meter patch cables
· Telco closet 2-009: (2) SC-LC 2 meter patch cables
· Telco closet 1-037 : (2) SC-LC 2 meter patch cables

9 SYSTEM DESIGN REQUIREMENTS
9.1 HORIZONTAL CABLING
The Horizontal Subsystem is the portion of the telecommunications cabling system that extends from
the work area telecommunications outleUconnector to the horizontal cross-connect in the
telecommunications room. It consists of the telecommunications outleUconnector, the horizontal
cables, optional consolidation point, and that portion of the cross-connect in the telecommunications
room serving the horizontal cable.
9.2 BACKBONE CABLING
9.2.1 INTRABUILDING CABLING
All cable will be installed via a conduit system that will be in place prior to start of project. This
system will be used to connect the main equipmenUcomputer (MDF) room to the (IDF) closets. Data
Backbone will consist of Multimode Fiber optic cabling, 12-strand 62.5/125um & 6-strand 9/125um
Single Mode fiber optic, manufactured by Commscope or approved equal. Voice riser shall consist of
100 or 50 pair category 5e.
9.2.2 INTERBUILDING CABLING
When a distribution system encompasses more than one building, the components that provide the
link between buildings constitute the Interbuilding Backbone Subsystem. This subsystem includes
the backbone transmission media, associated connecting hardware terminating this media, and
electrical protection devices to mitigate harmful voltages when the media is exposed to lightning
and/or high voltage power surges that pass through the building cable. It is normally a first-level
backbone cable beginning at the main cross-connect in the equipment room of the hub building and
extending to the intermediate cross-connect in the equipment room of a satellite building.

1 0 INSTALLATION
10.1 SITE SURVEY
Prior to placing any cable pathways or cable, the contractor shall survey the site to determine job
conditions will not impose any obstructions that would interfere with the safe and satisfactory
placement of the cables, The arrangements to remove any obstructions with the Project Manager
need to be determined at that time.
10.2 PHYSICAL INSTALLATION
10.2.1 CABLE PATHWAYS
· Pathways shall be designed and installed to meet applicable local and national building and
electrical codes or regulations.
· Grounding / Earthing and bonding of pathways shall comply with applicable codes and
regulations.
· Pathways shall not have exposed sharp edges that may come into contact with
telecommunications cables.
· The number of cables placed in a pathway shall not exceed manufacture specifications, nor,
will the geometric shape of a cable be affected.
· Pathways shall not be located in elevator shafts,
10.2.2 INTRABUILDING CABLE ROUTING
· The backbone subsystem shall include cable installed in a vertical manner between floor
telecommunications rooms and the main or intermediate cross-connect in a multi-story
building and cable installed horizontally between telecommunications rooms and the main or
intermediate cross-connect in a long single story building like a school or factory.
· Unless otherwise recommended by the manufacturer, all fiber cables will be run in inner duct.
· Fibers will be terminated in the telecommunications rooms using SC, or Fe connectors in
wall mounted interconnect centers or rack mounted panels equipped with sufficient ports,
slack storage space and splice trays if required to terminate and secure all fibers.
· Adequate riser sleeve/slot space shall be available with the ability to ingress the area at a
later date in all telecommunications rooms, such that no drilling of additional sleeves/slots is
necessary,
· The backbone cables shall be installed in a star topology, emanating from the main cross-
connect to each telecommunications room. An intermediate cross-connect may be present
between the main cross-connect and the horizontal cross-connect. This is known as a
hierarchical star topology.
· Optical fiber should be run for any backbone segment greater than 90 m (295 ft.). If the
Intrabuilding backbone segment is less than 90 m (295 ft), and fiber is not routed, the
balanced twisted-pair cable shall be category 5e or higher.
· Backbone pathways shall be installed or selected such that the minimum bend radius of
backbone cables is kept within manufacturer specifications both during and after installation.
· In an underground system, adequate underground conduit space shall be available and
accessible at each building. The conduits shall not exceed a fill factor of 40%.
· All underground systems shall be designed to prevent water runoff from entering the building.
· The backbone cables shall be installed in a star topology, emanating from the main cross-
connect to each satellite building telecommunications room. Alllnterbuilding cables shall be
installed to the applicable codes and regulations.
1 0.2.3 HORIZONTAL CABLE ROUTING
· All horizontal cables, regardless of media type, shall not exceed 90 m (295 ft) from the
telecommunications outlets in the work area to the horizontal cross connect.

· The combined length of jumpers, or patch cords and equipment cables in the
telecommunications room and the work area should not exceed 10m (33 ft) unless used in
conjunction with a multi-user telecommunications outlet.
· Two horizontal cables shall be routed to each work area. At least one horizontal cable
connected to an information outlet shall be 4-pair, 100 n balanced twisted-pair.
· It is recommended that a minimum horizontal cable distance of 15m (49 ft.) shall be
maintained between the telecommunications room and the work area.
· Horizontal pathways shall be installed or selected such that the minimum bend radius of
horizontal cables is kept within manufacturer specifications both during and after installation.
· In open ceiling cabling, cable supports shall be provided by means that is structurally
independent of the suspended ceiling, its framework, or supports. These supports shall be
spaced no more than 1.5 m (5 ft) apart.
· Telecommunications pathways, spaces and metallic cables, which run parallel with electric
power or lighting, which is less than or equal to 480 Vrms, shall be installed with a minimum
clearance of 50 mm (2 in).
· The installation of telecommunications cabling shall maintain a minimum clearance of 3 m
(10 ft) from power cables in excess of 480 Vrms.
· No telecommunications cross-connects shall be physically located within 6 m (20 ft) of
electrical distribution panels, step down devices, or transformers, which carry voltages in
excess of 480 Vrms.
· For voice or data applications, 4-pair balanced twisted-pair or fiber optic cables shall be run
using a star topology from the telecommunications room serving that floor to every individual
information outlet. The customer prior to installation of the cabling shall approve all cable
routes.
· The Contractor shall observe the bending radius and pulling strength requirements of the
4-pair balanced twisted-pair and fiber optic cable during handling and installation,
· Each run of balanced twisted-pair cable between horizontal portions of the cross-connect in
the telecommunication closet and the information outlet shall not contain splices.
· In a false ceiling environment, a minimum of 3 inches (75 mm) shall be observed between
the cable supports and the false ceiling.
· Continuous conduit runs installed by the contractor should not exceed 30.5 m (100 ft) or
contain more than two (2) 90 degree bends without utilizing appropriately sized pull boxes.
· All horizontal pathways shall be designed, installed and grounded to meet applicable local
and national building and electrical codes.
· Maximum conduit pathway capacity shall not exceed a 40% fill. However, perimeter and
furniture fill is limited to 60% fill for move and changes.
· Horizontal distribution cables shall not be exposed in the work area or other locations with
public access.
· Cables routed in a suspended ceiling shall not be draped across the ceiling tiles. Cable
supports shall be mounted a minimum of 75 mm (3 in) above the ceiling grid supporting the
tiles. .
10.2.4 WORK AREA TERMINATION
· All balanced twisted-pair cables wired to the telecommunications outleUconnector, shall have
4-pairs terminated in eight-position modular outlets in the work area, All pairs shall be
terminated using a EIAffIA 568-b Wiring Scheme.
· The telecommunications outleUconnector shall be securely mounted at planned locations.
· The height of the telecommunications faceplates shall be to applicable codes and
regulations.
10.2.5 PULLING TENSION
· The maximum cable pulling tensions shall not exceed manufacturer's specifications,

10.2.6 BEND RADIUS
· The maximum cable bend radii shall not exceed manufacturer's specifications.
· In spaces with balanced twisted-pair cable terminations, the maximum bend radius for 4-pair
cable shall not exceed four times the outside diameter of the cable and ten times for multi-
pair cable. This shall be done unless this violates manufacturer specifications,
· During the actual installation, bend radius on 4-pair cable shall not exceed eight times the
outside diameter of the cable and ten times for multi-pair cable. This shall be done unless this
violates manufacturer specifications.
10.2.7 SLACK
· In the work area, a minimum of 300 mm (12 in) should be left for balanced twisted-pair
cables, while 1 m (3 ft) is left for fiber cables.
· In telecommunications rooms a minimum of 3m (10ft) of slack should be left for all cable
types. This slack must be neatly managed on trays or other support types.
10.2.8 CABLE TIE WRAPS
· Tie wraps shall be used at appropriate intervals to secure cable and to provide strain relief at
termination points. These wraps shall not be over tightened to the point of deforming or
crimping the cable sheath.
· Hook and loop cable managers should be used in the closet where reconfiguration of cables
and terminations may be frequent.
10.2.9 GROUNDING
· All grounding / earthing and bonding shall be done to applicable codes and regulations.
10.2.10 FIRE PROTECTION
· Properly installed fire stop systems shall be installed to prevent or retard the spread of fire,
smoke, water, and gases through the building, This requirement applies to openings
designed for telecommunications use that mayor may not be penetrated by cables, wires, or
raceways.
· Fire stops shall be done to applicable code,
10.2.11 WORKMANSHIP
· All work shall be done in a workman like fashion of the highest standards in the
telecommunications industry.
· All equipment and materials are to be installed in a neat and secure manner, while cables are
to be properly dressed.
· Workers must clean any debris and trash at the close of each workday,

11 TESTING
Testing of all newly installed cable channels shall be performed prior to system cutover.

· Category 6 balanced twisted-pair horizontal and backbone cables, whose length does not
exceed 90 m (295 ft) for the basic link, and 100 m (328 ft) for the channel shall be 100
percent tested according to ANSIITIA/EIA-568-B.1. Test parameters include wire map plus
ScTP shield continuity (when present), length, NEXT loss (pair-to-pair), NEXT loss (power
sum), ELFEXT loss (pair-to-pair), ELFEXT loss (power sum), return loss, insertion loss,
propagation delay, and delay skew.
TEST EQUIPMENT CRITERIA
· All balanced twisted-pair field testers shall be factory calibrated each calendar year by the
field test equipment manufacturer as stipulated by the manuals provided with the field test
unit. The calibration certificate shall be provided for review prior to the start of testing.
· Auto test settings provided in the field tester for testing the installed cabling shall be set to the
default parameters
· Test settings selected from options provided in the field testers shall be compatible with the
installed cable under test.
11.2 FIBER OPTIC TESTING
HORIZONTAL FIBER TESTING
· Fiber horizontal cables shall be 100% tested for insertion loss and length.
· Insertion loss shall be tested at 850 nm or 1300 nm for 50/125f,.lm and 62.5/125f,.lm multimode
cabling in at least one direction using the Method B (1-jumper) test procedure as specified in
ANSIITIA/EIA-526-14A.
· Length shall be tested using an OTDR, optical length test measurement device or sequential
cable measurement markings.
BACKBONE FIBER TESTING
· Fiber horizontal cables shall be 100% tested for insertion lo~s and length.
· Insertion loss shall be tested at 850 nm and 1300 nm for 50/1251lm and 62,5/125f,.lm
multimode cabling in at least one direction using the Method B (1-jumper) test procedure as

12 ADMINISTRATION & DOCUMENTATION
12.1 LABELING
· Horizontal and backbone cables shall be labeled at each end. The cable or its label shall be
marked with its identifieL
· A unique identifier shall be marked on each faceplate to identify it as connecting hardware.
· Each port in the faceplate shall be labeled with its identifier.
· A unique identifier shall be marked on each piece of connecting hardware to identify it as
connecting hardware.
· Each port on the connecting hardware shall be labeled with its identifier.
12.2 DRAWINGS
As-built drawings shall be supplied by the contractor showing the locations of and identifiers for all:
· Horizontal cable routing and terminations
· T elecommu nications outlets/connectors
· Backbone cable routing and terminations
12.3 RECORDS
All records shall be created by the installation contractor and turned over at the completion of work
The format shall be computer based and both soft copies and hard copies shall be part of the As-built
package. The minimum requirements include:
· Cable records must contain the identifier, cable type, termination positions at both ends,
splice information as well as any damaged pairs/conductors,
· Connecting hardware and connecting hardware position records must contain the identifier,
type, damaged position numbers, and references to the cable identifier attached to it.
· Test documentation on all cable types shall be included as part of the As-built package.
12.4 REPORTS
All reports shall be generated from the computer based program used to create the records above.
These reports should include but not limited to;
· Cable Reports
· Cross-connect Reports
· Connecting Hardware Reports

13 WARRANTY
Either a basic link or channel model configuration may be applied to the horizontal and/or backbone
sub-systems of the structured cabling system. Applications assurance is only applied to a channel
model configuration. All channels are to be qualified for linear transmission performance up to 300
MHz to ensure that high-frequency voltage phase and magnitude contributions do not prove
cumulative or adversely affect channel performance.
13.1 SYSTEM WARRANTY
A twenty (25) year warranty available for pending category 6 structured cabling system shall be
provided for an end-to-end channel model installation which covers applications assurance, cable,
connecting hardware and the labor cost for the repair or replacement thereof.
Additional features of the warranty shall include:

13.2 PRODUCT WARRANTY
The manufacturer of passive telecommunications equipment used in a manner not associated with
the Systems Warranty must have a minimum five (5) year Component Warranty on all its product.
The Products Warranty covers the components against defects in material or workmanship under
normal and proper use.
13.3 APPLICATIONS SUPPORTED
Existing and future applications supported for a channel model warranty include those approved by
the Institute of Electronic and Electrical Engineers (IEEE), the Asynchronous Transfer Mode (A TM)
Forum, the American National Standards Institute (ANSI) or the International Organization of
Standards (ISO) that specify compatibility with the cable referenced herein,

The Successful contractor shall purchase & maintain for protection from claims under worker's
compensation acts, claims resulting from negligent acts for damages of bodily injury, claims for
damages because of injury to or destruction of personal property including loss of use resulting there
from & claims arising out of the performance of this agreement. All insurance provided under this

This addendum is issued to clarify and correct information in the original bid package, as
a result of the pre-bid conference held Monday, April 26, and other questions received
regarding the RFP.
Corrections
· Diagram 14.10 shows four cables run from the PD Computer Room to each of
eight (8) locations on the first floor Only two (2) cables will be run between
these locations. Also see questions #6, #15, and #28 below

I. Must the system be designed to meet manufacturer's warranty requirements? If
so, can the appropriate manufacturer components be substituted for those
specified in the RFP?
>- Yes, the system must be designed to meet manufacturer warranty
requirements, and appropriate manufacturer components may be
substituted for those specified in the RFP in order to meet the
manufacturer warranty requirements.
2. The RFP specifies 50 or 100 pair Cat5e to be installed as part of the backbone
riser cable. Cat5e is only sold in 25-pair cable. Should multiple 25-pair cables be
used to meet the 50/100 pair requirement?
);- Yes, multiple 25-pair cables should be used to meet the pair quantity
specified in the RFP.
3. Will grounding be provided or must the vendor provide grounding of all racks,
ladders, etc?
.,. All tel co backboards will have grounding provided as specified in the
architectural plans (see section 14.20 of the RFP). However, all sites
requiring rack/ladder installations will not necessarily have grounded
backboards installed. Vendor is responsible for proper grounding of
installation.
4. Is plenum cable required for installation?
);- Yes, all cable installed must be plenum rated.
5. Must all fiber cable be run inner-duct?
.,. As specified in section 10.2.2 of the RFP, all fiber cable must be run
inner-duct unless otherwise recommended by the manufacturer

13. What is the deadline for bid submission?

14. Can Mohawk cable be used for this project?

15 RFP section 6.1 specifies 6 port faceplates, but all locations are be run with 4
cables with the exception of the 1st floor conference room locations which contain
6 cables. Should 4-port faceplates be used for locations with 4 cables?
~ Yes, 4 or 6 port faceplates should be used as appropriate.
16. Will the copper and fiber optic riser cables be run within continuous conduit from
the Computer Room #3-016 to the teleo closets, alleviating the need for plenum

rated cable / innerduct? Or will the cables be run through sleeves through
theplenum, thus requiring plenum rated cable / innerduct?

17. Is there a specific grade of glass you require for the multimode fiber optic cable?
Are you trying to achieve I Gigabit throughput')
,. No, we do not require any specific grade of glass for the multimode or
single mode fiber.
18. Specifically, how many Category 6 patch cords are required in each color & at
what lengths?
,. As specified in section 5.4, two (2) patch cables are to be supplied for each
work area, one yellow and one gray, each ten (10) feet in length. Based on
the location counts in figure 14.2, there are a total of 177 locations in the
building. Therefore, 177 yellow lOft. patch cords and 177 gray lOft.
patch cords must be supplied for the work areas. In addition, section 5.2
of the RFP specifies yellow and orange patch cords to be provided for
each closet where the work area cabling (horizontal cabling) terminates.
Vendor must determine the correct length at installation based on vendor's
design, as specified in section 5.2, but the quantities will be 177 yellow
and 1 77 orange patch cords.
19. Do you require front and rear horizontal wire management panels?
,. Yes, front and rear panels must be provided.
20. For quotation purposes, are we to base the number of outlets according to the
counts listed on the Horizontal Riser diagram (14.2) or shall we come up with
totals based on triangle counts on each power plan?
,. For quotation purposes, base location counts on the Horizontal Riser
diagram (14.2) in the RFP document. Ideally, vendors will provide an
additional "per location" quote in their bid submission should the City
require any additional locations.

>- For quotation purposes, base number of locations and cable counts on the
riser diagram (14.2) rather than the electrical drawing sheets.
22. Who will be responsible for providing and installing the plywood backboards?
,. The plywood backboards will be provided and installed by the General
Contractor prior to work commencement on the structured cabling system.
23. Is it correct to assume that all conduit(s), poke through fittings, and sleeves will
be provided by the electrical contractor?
,. Yes, all conduit( s), poke through fittings, and sleeves (as shown on
architectural drawings [see section 14.20 of the RFP]) will be provided by
the General Contractor prior to work commencement on the stJUctured
cabling system.
24. Can another manufacturer of rack equipment be substituted instead of
Chatsworth?
,. Yes, equivalent equipment may be substituted.
25. Will the bid due date be extended for this project?

>- As depicted in figures 14.4, 14.6, 14.8, 1410, 14.11, and 14.15, four
CAT6 cables, one each of white, black, yellow, and orange color, shall be
run to each location. The specific arrangement of each outlet shall be
determined prior to work commencement and is not expected to have any
bearing on bidders response to the RFP.
28. Should all the CAT6 jacks in a particular faceplate be one color or if not state
which color should be in what position of the particular faceplate?
>> Jacks should be of matching color with the cable connected to each
particular jack. Therefore, each faceplate shall have four jacks, one each
of white, black, yellow, and orange color. The only exception shall be the
additional two jacks located in the first floor conference room (see also
questions #6 and # 15 above), which shall be of different colors (color #5
and Color #6) to be determined.
29. Specification section 14.2 Horizontal cabling - the count of the station drops do
not match actual count on the floor plans.
>> Refer to questions #20 and #26 above.
30. How are we to determine the third floor locations which ones are PD and NON-
PD with the locations terminating in two different closets? The same for the 1st
and 2nd floor?
>> The third floor of the building is physically divided into two sections,
separated by an access-controlled doorway. The area of the second floor
West of the doorway will be part of the Police Department, and all
locations in this area shall be home-run to the PD Computer Room. All
locations in the area East of the doorway shall be home-run to the main
computer room. The entire second floor of the government center shall be
dedicated to the Police Department, and all locations on the second floor
shall be home-run to the PD Computer Room. The only area on the first
floor that will also be home-run to the PD Computer Room is a single
conference room ( eight locations), room number 1-019.
3 1. Are the CA T6 cables from one particular location being separated on patch panels
in the Computer Room(s) / Telco closets?
,. Yes, all 4 cables from each particular location shall be run to different

1. As shown in section 14, there is passive equipment (ie, 48 port cat 6 patch panels)
that the contractor is to provide. However, there is also active equipment (ie,

any part of the RFP. Are we to provide those items for the City or will the City
provide those items as a customer purchase?
" The contractor will only be providing passive equipment as part of this
RFP. All active equipment will be purchased separately by the City, and
is only shown in diagrams for clarification and planning purposes (ie, for
vendor to determine rack layout in order to provide proper length patch
cords). Contractor will be providing only passive equipment, with the
exception of cabinets, which will also be provided by the City.
