A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, APPROVING AN
ADDENDUM TO THE CONSULTANT AGREEMENT
APPROVED UNDER RESOLUTION 2004-680 BETWEEN THE
CITY OF SUNNY ISLES BEACH AND JADE
COMMUNICATIONS INC. FOR SECURITY WIRING FOR
THE SUNNY ISLES BEACH GOVERNMENT CENTER, IN
AN AMOUNT OF FOURTEEN THOUSAND FIVE HUNDRED
FORTY-FOUR DOLLARS ($14,544.00), ATTACHED HERETO
AS EXHIBIT "A"; AUTHORIZING THE MAYOR TO
EXECUTE SAID AGREEMENT; AUTHORIZING THE CITY
MANAGER TO DO ALL THINGS NECESSARY TO
EFFECTUATE THE TERMS OF THE AGREEMENT;
PROVIDING FOR AN EFFECTIVE DATE.

WHEREAS, under Resolution 2004-680 the City Commission approved a consultant
agreement withjade Communication Inc. for wiring for the Sunny Isles Beach Government Center;
and
WHEREAS, the City wishes to amend the prior consultant agreement to permit Jade
Communications, Inc to provide the security wiring for the Sunny Isles Beach Government Center
WHEREAS, the City wishes to contract with Jade Communications, Inc. for the Security
Wiring for the Sunny Isles Beach Government Center, in an amount of $14, 544.00, attached hereto
as Attachment "A".
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Approval of Addendum to Consultant Agreement. The Consultant Agreement is
amended by and between the City of Sunny Isles Beach and Jade Communications, Inc. for the
Security Wiring for the Sunny Isles Beach Government Center, in the amount of $14,544.00,
attached hereto as Attachment "A", be, and the same, is hereby approved.
Section 2.
Authorization of Mavor. The Mayor is hereby authorized to execute said Agreement.
Section 3. Authorization of City Manager. The City Manager is hereby authorized to do all
things necessary to effectuate the terms of the Agreement.
Section 4.
Effective Date. This Resolution will become effective upon adoption.

We hereby propose to furnish labor and materials-complete in accordance with the above

All material is guaranteed to be as specified. All work above to be completed in a skillful manner
according to standard practices. Any alteration or deviation from above specifications involving
extra cost, will be executed only upon written orders, and will become a "T & Mil extra charge
over and above the estimate. All agreements are contingent upon strikes, accidents or delays
beyond our control. No permit fee's or bonding is included in this quotation. This proposal
subject to acceptance within thirty days and is void hereafter at the option of the undersigned.

Security Wiring
Jade Communications Inc. Shall provide and install the following Security Cabling. Scope
is to provide and install the cables only. Terminations of cabling and all associated hardware is
to be provided by others.
Each Card Reader Shall have One CAT 5 cable and a 18/2 for-the electric strike
MAG Locks shall have one 18/2
Request to Exit button shall have one 18/2 TREX shall have one 18/4 cable
Intercom outside Sally Port shall have two 18/2 and two CAT 5 cables
Door Contacts shall have one 22/4 cable
Code Blue shall have one 22/4 cable
All Cameras shall have One RG59 and one 18/2 cables. In addition the PTZ cameras
shall have one Cat 5 cable

ADDENDUM TO JADE COMMUNICATIONS. INC. AGREEMENT
THIS ADDENDUM, EXECUTED THIS ~ DAY OF ~~, 2004,
shall be an integral part of the service agreement. In the event that any terms or conditions contained
herein conflict with the basic agreement, the terms and conditions of this addendum shall prevail.
1. The amended price for these services is FOURTEEN THOUSAND, FIVE HUNDRED
FORTY-FOUR DOLLARS, AND 01/100 DOLLARS ($14,544.00), payable as invoiced.
