A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, APPROVING A
CHANGE ORDER TO THE AGREEMENT WITH AUDIO
VISUAL INNOVATIONS, INC., TO PROVIDE
AUDIONISUAL EQIDPMENT AND INSTALLATION FOR
THE SUNNY ISLES BEACH GOVERNMENT CENTER, IN
AN AMOUNT OF SEVEN THOUSAND FOUR HUNDRED
FIFTY-FOUR DOLLARS AND SIXTY-FOUR CENTS
($7,454.64), ATTACHED HERETO AS EXHIBIT "A";
AUTHORIZING THE CITY MANAGER TO DO ALL THINGS
NECESSARY TO EFFECTUATE SAID CHANGE ORDER;
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, by Resolution No. 2004-726 the City Commission approved an agreement
between the City and Audio Visual Innovations, Inc. to install A V Cameras and LCD Projector for
the Sunny Isles Beach Government Center Commission Chambers as well as other tasks, in an
amount not to exceed $213,046.03; and
WHEREAS, the City wishes to change the chamber displays from screens to plasmas, and
Audio Visual Innovations, Inc. has submitted a Change Order to the City in an amount of Seven
Thousand Four Hundred Fifty-Four Dollars and Sixty-Four Cents ($7,454.64), with the total amount
of the contract $220,500.68, attached hereto as Exhibit "A"; and
WHEREAS, Audio Visual Innovations, Inc. is qualified, willing, and able to provide the
desired services on the terms and conditions set forth herein.
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Approval of Change Order to Agreement. The Change Order to the Agreement by
and between the City of Sunny Isles Beach and Audio Visual Innovations, Inc., in an amount of
$7,454.64, attached hereto as Exhibit "A" be, and the same, is hereby approved.
Section 2. Authorization of City Manager. The City Manager is hereby authorized to do all
things necessary to effectuate said Change Order.
Section 3.
Effective Date. This Resolution will become effective upon adoption.
PASSED AND ADOPTED this 18th day of November, 2004.

ADDENDUM TO AUDIO VISUAL INNOVATIONS. INC. AGREEMENT
THIS ADDENDUM, E:XE~HIS -1~ DAY OF Nol1f!hv\b"W'" ,2004,
shall be an integral part of the service agreement. In the event that any terms or conditions contained
herein conflict with the basic agreement, the terms and conditions of this addendum shall prevail.
1. The amended price for these services is SEVEN THOUSAND, FOUR HUNDRED FIFTY-
FOUR DOLLARS, AND 64/100 DOLLARS ($7,454.64), payable as invoiced and reflected in
Exhibit "A".

above.

MEMORANDUM

Re: Addendum to Audio/Visual System for Sunny Isles Beach Government Center
Recommendation:
It is recommended that the City Commission adopt the attached resolution.
Reasons:
The original A/V system proposal included two drop-down electric screens for the commission
chambers. However, it has become apparent through discussions with the General Contractor
(Weitz), architect (Larry Kline),and looking at the current conditions of the job site that trying to
install those screens at this point in time would have a significant impact in terms of delay,additional
cost for change orders, and would not look as "good" as another option we had initially decided
against due to cost,which is to use 61" wall mounted plasma displays. Because of the additional
costs that would be required to install the screens into the"almost finished" ceiling of the new
Commission Chambers at this point,we have concluded that this is the better option.
This addendum is to change the original proposal by removing the two electric screens and LCD
projectors, and substituting 61" plasma displays in their place. This increases the amount of the
project by $7454.64, from 5213,046.03 to 5220.500.68.
