A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF
SUNNY ISLES BEACH, FLORIDA, APPROVING CHANGE ORDER NO.
1 TO AGREEMENT WITH SIL TEK GROUP, INC., IN THE AMOUNT OF
THIRTEEN THOUSAND ONE HUNDRED SIXTY-SIX DOLLARS AND
SIXTY-SEVEN CENTS ($13,166.67) FOR THE CONSTRUCTION OF THE
181sT DRIVE ACTIVE PARK; AUTHORIZING THE CITY MANAGER
TO DO ALL THINGS NECESSARY TO EFFECTUATE THIS
RESOLUTION; PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the City of Sunny Isles Beach wishes to develop the 18 I sl Drive Active Park
located at 181 15 North Bay Road and publicly advertised Bid No. 05-04-0 I for the construction
of the 18 I sl Drive Active Park; and
WHEREAS, on July 14, 2005, pursuant to Resolution No. 2005-814, the City
Commission awarded Bid No. 05-04-01 to Siltek Group, Inc. ("Siltek"), who was determined to
have the lowest and most responsive, responsible bid, in the amount of $7,154,303.05, and
authorized the City Manager to negotiate and enter into an agreement with Siltek; and
WHEREAS, on September 8, 2005, pursuant to Resolution No. 2005-825, the City
Commission ratified the execution of an agreement with Siltek Group, Inc., in the amount of
$7,186,423.05; and
WHEREAS, it has been determined that additional testing is required on production piles
and augercast test piles, and the engineer of record Miller Legg and Calvin Giordano, and
Associates, have reviewed and approved Change Order No. I submitted by Siltek Group, Inc., for
the testing, in the amount of $13,166.67, bringing the total contract amount to $7,199,589.72,
attached hereto as Exhibit "A".
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Incorporation of Recitals. The recitals set forth in this resolution are incorporated
herein by reference as if fully set forth herein.
Section 2. Approval of Change Order No.1. Change Order No. I with Siltek Group, Inc.,
for the construction of the 181 st Drive Active Park, attached hereto as Exhibit "A", in an amount
of Thirteen Thousand One Hundred Sixty-Six Dollars and Sixty-Seven Cents ($13, I 66.67), be
and the same, is hereby approved.
Section 3. Authorization of City Manager. The City Manager is hereby authorized to do all
things necessary to effectuate the terms of this Resolution.
Section 4.
Effective Date. This Resolution will become effective upon adoption.

Dear 1\1r. Zayas:
Nutting Engineers of Florida, Inc. is pleased to present this proposal for performing augercast
pile load test and installation monitoring for the Proposed Gyninasium and Recreational Park
project in Sunny Isles Beach, Florida.
We understand that approximately 205 augercast piles will be installed for this project. We
understand that a pile load test will be performed in accordance Vv1.th the South Florida Building
Code to twice the design load. Nutting Engineers previously perfonned a geotechnical
exploration for this project, and our findings and recommendations are presented in a report
dated Jillle 16,2004.
Based on our understanding of the project, \ve propose the following scope of work and fee
schedule.
SCOPE OF WORK
We ...vill provide full-time monitoring of the installation of the test/reaction piles and production
augercast piles, provide instrumentation and direct the pile load test. Based on the results of the
pile load test, we will provide final installation criteria for the production piles. A log of each
pile will be kept and will include depth, size, grout take, 'steel reinforcement placed, and other
pertinent information. We have assumed approximately 20 piles per day ,",viII be installed during
production pile installation.

The above fee schedules are provided to give an estimate of the costs associated with the
monitoring/reporting of the installation of the production piles. The actual costs will be based on
the time required to install the piles, behavior of the pile load test, and other factors beyond ow:
control.

At its regular meeting of February 16, 2006, the City Commission adopted Resolution No.
2006-885, which approved Change Order No. I to the Active Park Construction Agreement with
Siltek. Attached are two (2) original agreements and two (2) copies of the approving resolution
to assist you in processing this item.
The remaining original has been placed with the original resolution.
