A RESOLUTION OF THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA,
AUTHORIZING THE DONATION OF A 18-PASSSENGER
VAN TO THE ANTIOCH COMMUNITY DEVELOPMENT
CORPORA TION TO TRANSPORT ORPHAN CHILDREN
IN HAITI TO SCHOOL; PROVIDING THE CITY
MANAGER WITH THE AUTHORITY TO DO ALL
THINGS NECESSARY TO EFFECTUATE THIS
RESOLUTION; PROVIDING FOR AN EFFECTIVE
DATE.
WHEREAS, the City of Sunny Isles Beach has retired two (2) of their 18
passenger vans as surplus, after several years of service; and
WHEREAS, several orphan children in Haiti have no means of transpOliation
and walk several miles to and from school daily; and
WHEREAS, the City Commission wish to donate one of the vans to the Antioch
Community Development Corporation, a non-profit organization that has partnered with
an organization in Haiti that supports an orphanage and school; and
WHEREAS, specifically, the City Commission proposes the donation of a 1999
Ford Van (VIN# IFDXE40S5XHB7889I); and
WHEREAS, the van will be used to transport children between the orphanage
and school; and
WHEREAS, the City's generosity will make a great impact on the lives of some
orphan children in Haiti and elevate the quality of their lives.
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION
OF THE CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1. Donation of 18- Passenger Van, The City Commission hereby donates an
18-passenger van to the Antioch Carol City Community Development Corporation.
Section 2, Authority of the City Manager. The City Manager or his designee is
hereby authorized to deliver an 18-passenger van to the Antioch Carol City Community
Development Corporation. The City Manager is authorized to do all things necessary to
effectuate this Resolution.
Section 3.
Effective Date. This Resolution shall become effective upon adoption.

On behalf of the Antioch Carol City Community Development Corporation, I hereby give
sincere thanks to Mayor Edelcup, Hans Ottinet, and the City of Sunny Isles for giving us
the opportunity to assist the citizens of Haiti by donating the 18 passenger van to
Ambassadors for Jesus, the organization through which we give monthly support to the
orphanage and school for Haitian children in Port Au Prince Haiti.
I am thankful for the opportunity that this great event has brought about - fOlming a
partnership with you and the City of Smmy Isles, as we endeavor to help the people of
Haiti. I look forward to meeting with you and/or your staff in the near future to further
discuss other avenues to help the people.
As requested, for your reference, I have also attached a copy of our 501 (c )(3)
designation.

you are exempt from federal income tax under section 501(a) of the Internal
Revenue Code as Ian organization described in section 501(c) (3).

Donors mayideduct contributions to you as provided in section 170 of the
Code. Bequests/I legacies, devises, transfers, or gifts to you or for your use
are deductible for federal estate and gift tax purposes if they meet the
applicable provisions of Code sections 2055/ 2106, and 2522.

their contributions are gifts, with no consideration received. Ticket pur-
chases and similar payments in conjunction with fundraising events may not
necessarily quatify as deductible contributions, depending on the circum-
stances. See Revenue Ruling 67-246, published in Cumulative Bulletin 1967-2/
on page 104, which sets forth guidelines regarding the deductibility, as chari-
table contribut~ons, of payments made by taxpayers for admission to or other
participation ip fundraising activities for charity.
In the head~ng of this letter we have indicated whether you must file Form
990, Return of Organization Exempt From Income Tax. If Yes is indicated, you
are required to; file Form 990 only if your gross receipts each year are
normally more than $25,000. However, if you receive a Form 990 package in the
mail, please fiile the return even if you do not exceed the gross receipts test.
If you are not irequired to file, simply attach the label provided, check the
box in the heading to indicate that your annual gross receipts are normally
$25,000 or less, and sign the return.
If a return is required, it must be filed by the 15th day of the fifth
month after the end of your annual accounting period. A penalty of $20 a day
is charged when a return is filed late, unless there is reasonable cause for
the delay. Ho~ever, the maximum penalty charged cannot exceed $10,000 or
5 percent of y6ur gross receipts for the year, whichever is less. For
organizations ~ith gross receipts exceeding $1,000,000 in any year, the penalty
is $100 per dat per return, unless there is reasonable cause for the delay.
The maximum penalty for an organization with gross receipts exceeding
$1,000/000 shall not exceed $50/000. This penalty may also be charged if a
return is not ~omplete, so be sure your return is complete before you file it.

You are required to make your annual information return, Form 990 or
Form 990-EZ, available for public inspection for three years after the later
of the due date of the return or the date the return is filed. You are also
required to make available for public inspection your exemption application,
any supporting! documents, and your exemption letter. Copies of these
documents are ~lso required to be provided to any individual upon written or in
person requesd without charge other than reasonable fees for copying and
postage. You may fulfill this requirement by placing these documents on the
Internet, pedalties may be imposed for failure to comply with these

Approval of Donation of Van to Antioch Community Development Corporation
At its regular meeting of July 20, 2006, the City Commission adopted Resolution No. 2006-961,
which approved the donation of an ]8-passenger van, VIN# 1FDXE40S5XHB78891, to Antioch
Community Development Corporation. Attached is a copy of the approving resolution to assist
