A RESOLUTION OF THE CITY COMMISSION OF THE CITY OF
SUNNY ISLES BEACH, FLORIDA, APPROVING A SECOND
ADDENDUM TO AGREEMENT WITH GRAPH-PLEX SIGNAGE,
FOR THE PURCHASE AND INSTALLATION OF SIGNAGE FOR
PELICAN COMMUNITY PARK, IN AN AMOUNT NOT TO
EXCEED TEN THOUSAND EIGHT HUNDRED DOLLARS
($10,800.00), ATTACHED HERETO AS EXHIBIT "A";
AUTHORIZING THE MAYOR TO EXECUTE SAID SECOND
ADDENDUM TO AGREEMENT; AUTHORIZING THE CITY
MANAGER TO DO ALL THINGS NECESSARY TO
EFFECTUATE THE TERMS OF THIS SECOND ADDENDUM TO
AGREEMENT; PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, on November 16, 2006 via Resolution No. 2006-1007, the City
Commission approved an agreement with Graph-Plex Signage, in an amount not to exceed Two
Hundred Twenty Seven Thousand Three Hundred Sixty-Five Dollars ($227,365.00), (the
"Agreement"), to fabricate and install monument signs at various intersections on Collins A venue
and City Parks; and
WHEREAS, on March 20, 2008, via Resolution No. 2008-1239, the City Commission
approved a First Amendment to the Agreement for installation of street signage in the Golden
Shores community in an amount not to exceed Eighty Seven Thousand Dollars ($87,000.00); and
WHEREAS, the City of Sunny Isles Beach is now desirous of entering into a Second
Addendum to Agreement with Graph-Plex Signage to obtain signage for the Pelican Community
Park; and
WHEREAS, Graph-Plex Signage is qualified, willing and able to provide the desired
services; and
WHEREAS, Graph-Plex Signage has submitted a proposal to the City to fabricate and
install signage for the Pelican Community Park in an amount not to exceed Ten Thousand Eight
Hundred Dollars ($10,800.00), attached hereto as Exhibit "A", bringing the total Agreement
amount not to exceed Three Hundred Twenty Five Thousand One Hundred Sixty Five Dollars
($325,165.00).
NOW THEREFORE, BE IT RESOLVED BY THE CITY COMMISSION OF THE
CITY OF SUNNY ISLES BEACH, FLORIDA, AS FOLLOWS:
Section 1, Approval of Second Addendum to Agreement. The Second Addendum to
Agreement with Graph-Plex Signage, to obtain signage for the Pelican Community Park, in a
total amount not to exceed Ten Thousand Eight Hundred Dollars ($10,800.00), attached hereto
as Exhibit "A", be and the same, is hereby approved,
Section 2, Authorization of Mayor. The Mayor is hereby authorized to execute said Second
Addendum to Agreement.

Section 3. Authorization of City Manager. The City Manager is hereby authorized to do all
things necessary to effectuate the terms of this Second Addendum to Agreement.
Section 4.
Effective Date. This Resolution shall become effective upon adoption.

this _ day of , 2008, is hereby attached to and is made a part of said Graph-
Plex Signage Contract No. C0607-06 by and between the City of Sunny Isles Beach ("City") and
Graph-Plex Signage ("Contractor") a business corporation I icensed in the State of Florida, whose
Federal Identification # is . The City and Contractor hereby agree as follows:
SECOND ADDENDUM TO CITY OF SUNNY ISLES BEACH
AND GRAPH-PLEX SIGNAGE AGREEMENT
CONTRACT NO. 0708-059
1. ADDITIONAL SCOPE OF WORK: The City wishes to add to the original Graph-Plex
Signage Agreement by having Contractor fabricate and install signage at the Pelican Community
Park, as more fully described in Attachment "A".
2. COST: The Contractor agrees to charge a total amount not to exceed Ten Thousand Eight
Hundred Dollars ($] 0,800.00) for said services.
3. TERM: The term of this Second Addendum shall begin upon the execution of this Second
Addendum and shall end thirty (30) days thereof.
4. INCORPORATION OF TERMS AND CONDITIONS: Except as expressly amended or
modified by this Second Addendum to Agreement, all of the terms and conditions or the original
Agreement and First Addendum thereto, shall remain in full force and effect in accordance with their
terms and conditions.

the date mentioned above,

Installation: $450.00
* Permit Acquisition Fee: Signed and sealed engineering drawingsfor all signs, preparation of
all permit documentation, liaison to City ofjicialsfor permits, running of permits, etc.
Terms: 50% deposit for work to commence. Balance due upon completion.
Delivery: Approximately three weeks from City approval.

Prices based on signs presented on 3.10.08
GraphPlex to fabricate and install one custom ADA sign package per the design submitted on
3.10.08. Back plates are .125" thick solid acrylic with reverse spray in teal green. Copy and
symbols are white applique with Grade II Braille. There are an additional two soft waves in
yellow/orange and blue applique. Signs are installed with foam tape and silicone. Signs to
produce:

Building and identification signs for Pelican Community Park
RECOMMENDATION
This resolution is brought forth for your consideration
REASONS
The construction contract for the Pelican Community Park did not include a budget amount for
signage, As part of the Temporary Certificate of Occupancy (TCO) there are several evacuation
signage as well as room location signage meeting AD1\ requirements that need to be installed prior
TCO. In addition to the interior signage, exterior identification signage for the building is also
needed. In order to expedite the construction and installation of the signage, the City has piggy back
to an existing contract that it currently has Graph-Plex. Graf-Plex has provided a proposal for the
interior and exterior signage in the amount not to exceed of $10,800.
Funding available for this project in account #20-600-5611.
