The undersigned Bidder has carefully examined the Bidding Documents and the site of the proposed work
and is familiar with the nature and extent of the work and any local conditions that may in any manner
affect the work to be done.
The undersigned Bidder agrees to do all the work and furnish all materials called for by the Bidding
Documents, in the manner prescribed therein and to the standards of quality and performance established
by the Engineer for the unit price amounts stated in the spaces herein provided, for each of the items or
combination of items stipulated. Unit price line items shall include cost of implementing all applicable
safety requirements. Bidder acknowledges that estimated quantities scheduled are approximated for the
sole purpose of obtaining comparative bids for determination of the lowest responsible bidder and actual
quantities required may increase or decrease. Bidder further agrees that payments will be made on the
basis of actual quantities placed and accepted in the construction

BID SECURITY
Accompanying this Bid is a certified or cashier's check or a bid bond in the amount of 5%_ofthe bid made
payable to the Owner which may be forfeited as liquidated damages if, in the event this proposal is
accepted, the undersigned fails to execute the Agreement and furnish and pay for the required
performance and payment bonds for the Owner within 10 calendar days after acceptance of the Bid;
otherwise said certified or cashier's check or bid bond will be returned to the undersigned.
The undersigned Bidder agrees to commence work within ten (10) calendar days after the date of written
Notice to Proceed, and further agrees to substantially complete the work within THREE HUNDRED
SIXTY FIVE (365) consecutive calendar days after the date of written Notice to Proceed subject to such
extensions of time allowed by the Conditions of the Contract.
The undersigned Bidder agrees that this Bid shall be good and may not be withdrawn for a period of sixty
(60) days after the scheduled closing time for receiving bids.
The undersigned agrees to forfeit to the Owner, as liquidated damages for each calendar day elapsing
between the date established as the date of substantial completion and the actual date of such substantial
completion of the contract work, the amount of$500.00 per calendar day.

Mandatory Pre-Bid Conference Summary Minutes
October 31,2007 at 10:00 a.m.

The bid opening date has been rescheduled to Wednesday, November 28th, 2007 at 10:00 am.
Requests for clarifications and questions must be made in writing to Hugo Guevara
(hguevara@bhaengineers.com) with a copy to Jane Hines, City Clerk (ihines@sibfl.net) and
will be accepted until 5:00 pm on Monday, November 19th, 2007.
In Re: Bid No. 07-10-02 (Central Island Drainage and Street Improvements)
City Clerk Jane A. Hines, CMC, opened the Pre-Bid Conference and introduced Hugo Guevara of
Beiswenger, Hoch and Associates who presented an overall description of the project noting that this
project is strictly related to specialized drainage with deep wells. He reminded the contractors that
the deadline for submission of bid documents is \\'ednesday, November 14,2007 at 10:30 a.m. ,
2007 at 10:00 a.m., at 't,'hieh time we will publiely open those sealed bids. Any additional
questions must be submitted in writing up to five business (5) days before the bid opening date.
Participants of today's meeting will be copied on all answers to written questions, as stated in
Section 100.
The following questions and answers, copies of revised plans and Bid Documents constitute
ADDENDUM NO.1.
QUESTIONS AND COMMENTS FROM PROSPECTIVE BIDDERS
1. This bid was bid out once before, why wasn't it awarded? Do you anticipate that it will
happen again?
The bid was advertised but cancelled, before anyone submitted bids, because funding
documents had not been correctly prepared so we missed the grant funding cycle. No,
the City does not anticipate that to happen again because funding is in place.
2. When is the projected start date?
It is anticipated to start the first Monday in .January 2008 (.January 7,2008).
3. Have all permits and easements been obtained?
The City will issue the construction permits. In reference to the drainage, the City has
already obtained a well permit from DERM. The City has not applied for a FDEP
permit yet. The General Contractor must pay for DERM and FDEP well construction
permits. Construction permits will be reimbursed by the Cit), (approximately $750.00
and $200.00 per well for FDEP and DERM respectively).
4. Is there a long lead time to obtain permits?
DERM's permit is available immediately. FDEP's will take approximately 4 to 6
weeks, minimum.

"Under Special Provisions page 4 of 15 paragraph 13 (CSBE), it states a
2% CSBE subcontractor goal has been established for this project. In
regards to the CSBE requirement, are there any special forms that must be
submitted with the bid, ex Schedule to Intent Affidavit. If so, please
forward the form/forms."
Certified Small Business Enterprises (CSBE) is not a requirement, but it
is recommended and encourage from Prospect Bidders.
