the southeastern United States with high-quality
engineering and planning services. The firm
maintains regional and field offices throughout the
State of Florida with a total full time staff of over 80

E&R staff offers extensive government, academic

E&R PROJECT TEAM
Shahin Hekmat, P. E. will be the overall project
manager and the point of contact for the City
of Sunny Isles Beach, Mr. Hekmat has been
involved in projects with the City for more than
three years and helped prepare the City's Capital
Improvement Plan (CIP) and the associated budget.
Mr. Hekmat is experienced in the planning, design,
permitting, bidding, and awarding and all aspects
of construction administration in the South Florida
Area. Mr. Hekmat has been involved with municipal
projects and agencies since 1990. He was the City
Engineer for City of Weston, the District consultant
with South Broward Drainage District and has
been involved as an in-house consultant to many
other cities such as Lauderhill, North Lauderdale,
Miramar, Pembroke Pines, Dania Beach, Sunny Isles
Beach and Westpark. He has also been involved in
preparing and presenting project scope and funding
needs for large public forums as well as one-on-one
with public officials and community leaders,
Scott Gombar, P.E. will be the Principal-in-Charge.
Mr, Gombar has also been involved with the
City for many years, managing capital projects
and coordinating with outside consultants and
contractors.
Quality Assurance and Quality Control (QA/QC)
Procedures will be followed by all team members,
Standardized requirements are outlined in E&R's
QA/QC Plan and distributed by E&R's QA Manager
at the beginning of the project. The QA/QC program

Tony Grau, P.E. Mr. Grau has over 30 years of
construction experience with Florida Department of
Transportation Districts four and six.
Eisman & Russo has always adopted the philosophy
that our sub-consultants are an extension of our
staff, just as we are an extension of the City staff.
The proposed sub-consultants were selected by
E&R because of their extensive experience and
excellent reputations with Miami-Dade County and
FOOT. Mr, Hekmat has a working relationship with

The City of Sunny Isles Beach wishes to improve
the Collins Avenue corridor both aesthetically and
functionally. To achieve this goal the City has
worked with the various developers and property
owners fronting the Collins Avenue right-of-way to
obtain a ten (10) foot easement along the entire
west right-of-way line. East of Collins Avenue the
newer developments have provided a landscape
buffer and a more uniform scheme of planting as
required by the City's land development code.
In the past the City had completed a number of
different projects within the Collins Avenue corridor
to start the beautification process and enhance
safety along AlA. These projects included stamped
colored crosswalks, fencing within the medians
to deter jaywalking and an annual maintenance
program to replant the Collins Avenue medians with
more colorful hedges and flowers.
The City also commissioned a streetscape master
plan for Collins Avenue, as well as other major
streets within the City limit. This study provided a
number of ideas and recommendations, some of
which have been adopted and implemented by the
City, such as the process of under-grounding of
overhead utilities throughout the City, unique and
uniform signage, identifiable cross walks, decorative
street lighting and furnishing.
The study also recognizes the uniqueness of
Collins Avenue as the "main street" for the City of
Sunny Isles Beach while functioning as a major
arterial for the FOOT. However, the study does
not address the difficulties in implementing many
of the recommendations along this stretch of
highway. The City recognizes that a successful plan
must be simple, must be
constructible and, most of
all, must be salable to the

residents, the property owners and FOOT,
Project Objectives
The Collins Avenue Streetscape Project objectives
are to achieve a more aesthetically pleasing main
street for the City of Sunny Isles Beach residents
and businesses without impacting the traffic
circulation and safety that FOOT must maintain on a
major arterial.
To achieve the goals of this project the following
items must be addressed in the planning stage and
included in the contract documents:
· Underground Overhead Utilities (FPL, BellSouth
and Cable),
· Brick Paver Sidewalk,
· Enhanced planting and buffer,
· Uniform Signage,
· Decorative Street Lighting, and
· Decorative street furnishing (benches, trash
receptacles, bus shelters, newspaper stands)
Project Approach
Eisman & Russo's Team approach to this project
is very simple, The City of Sunny Isles Beach has
already started the "Branding" of the City by using
a uniform sign package, identifying plant materials
that survive the harsh coastal environment, working
with FPL on removing all overhead utilities and
placing them underground, replacing standard FPL
street light poles with decorative ones and finally
standardizing the trash receptacles, benches and
bus shelters, We believe that recreating the wheel
at this time would be expensive and not a step in
the right direction.

ideas into the Collins Avenue streetscape design.
To achieve this goal a number of tasks must be
systematically completed.
Task 1 - Route Survey with Improvements
The success of this project is dependent on having
an accurate route survey along the Collins Avenue
corridor. The right-of-way is packed with utilities
both above and under ground, street lights, traffic
signals, traffic signs and a myriad of various size
and shape utility boxes and controllers, The ten
(10) foot easements outside of the right-of-way
lines are also filled with large monument signs,
decorative walls, landscaped buffers and parking
facilities.
Weidener Surveying and Mapping, P.A. prepared
a photogrammetric mapping of the City of Sunny
Isles Beach in 2000 for the Streetscape project.
The E&R Team will use this existing right-of-way
data and will supplement the plans by adding all
easements and buffers dedicated to the City and/or
other agencies. Finally all improvements from the
back of curb out fifteen (15) feet past the east and
west right-of-way lines will be obtained and plotted
to complete the route survey,
Task 2 - Utility Coordination
One of the most important aspects of the Collins
Avenue beautification is to convert all above ground
utilities to underground facilities. The City has
started this process and has allocated funds in
their Capita/Improvement Budget to complete the
utility conversion project; however, less than half of
the Collins Avenue underground project has been
completed, mainly by developers. The segments
still to be converted are:
1. East R/W line, from William Lehman Causeway
south to 183rd Street (:t
3000 feet)
2, East R/W line, 500 feet
.
north of 178th Street (opposite City Hall) south to
170th street (:t 3100 feet),
3. East R/W line, from Atlantic Avenue south to the
City limit at Haulover Beach Park (:t 2100 feet),
4. West R/W line, from 175th Terrace south to
169th Street (:t2100 feet), and
5. West R/W line, from Sunny Isle Beach Boulevard
(SR 826) south to the City limit at Haulover
Beach Park (:t 3100 feet),
This task will require an immediate meeting with
Florida Power and Light, BellSouth and Atlantic
Broadband. The utility companies must be given
the directive to start the design of their overhead
to underground facilities along the Collins Avenue
corridor, The E&R Team will work closely with all
the utility companies to assure all conflicts are
addressed immediately and that the beautification
project is not impacted.
Task 3 - Florida Department of Transportation
Coordination
The 2,25 miles of Collins Avenue traversing the City
of Sunny Isles Beach is major arterial under the
jurisdiction of Florida Department of Transportation
(FOOT). The existing 100' wide right-of-way with six
high traffic asphalt lanes, inadequate crosswalks,
medians with minimal landscape opportunity
and narrow busy sidewalks does not provide for a
pedestrian friendly experience.
As part of their work program, FOOT will be soliciting
a design consultant for Resurfacing, Restoration
and Rehabilitation (RRR) of Collins Avenue/AlA
due the end of this year. The design would typically
take twelve to eighteen months and construction
another two years. Any proposed changes along
this corridor must be closely coordinated with the
District to assure that the FOOT
regulations are met and the work
effort is not duplicated.

The E&R Team will work with FDOT to provide
additional crosswalks on Collins Avenue, replace all
existing crosswalks with a more distinctive pattern
that would be accepted by the FDOT and finally
improve the appearance of the two flyovers.
Task 4 - Report Preparation
The corridor defined by Collins
Avenue and its infrastructure should
be an asset to the people it serves
as well as a source of community
pride. The E&R TEAM shares the
belief expressed by the City that
successful enhancement of the
Collins Avenue corridor will provide
an important stimulus to community
improvement. This project can serve
as an outstanding example of how
a beautiful streetscape contributes
immeasurably to a community's civic
health and an improved life for its
people.

Collins Avenue is the main
thoroughfare in the City of Sunny
Isles Beach and improving the visual
character is of utmost importance.
The E&R TEAM is proposing a
streetscape plan that will capture the
unique identity of "Florida's Riviera."
The following is a brief summary of
the primary streetscape components.
Collins Avenue West: The Pedestrian Promenade
On the west side of Collins Avenue the E&R TEAM
envisions a 10' wide Pedestrian Promenade with
Medjool Date Palms spaced 20 to 30' on center.
The Medjool Date Palms will be planted in at grade
planters and will be highlighted in the evenings
with landscape uplights.

utilized to accentuate the Pedestrian Promenade
and to provide a landscape buffer for the adjacent
Vehicular Use Areas. The Pedestrian Promenade will
also feature decorative Pole mounted Light Fixtures
and Streetscape Furnishings that are consistent
with City Standards.

Isles Beach further develop its own unique identity
as a world class destination. The overall effect of
the Treatments along the Pedestrian Promenade will
be to provide a safe, comfortable, and aesthetically
pleasing environment that will be a great asset for
the City of Sunny Isles Beach.
Collins Avenue East: A Walk along the Beach
The east side of Collins Avenue will consist of a new
5' wide Pedestrian Walkway as well as New Beach
Access Entrance Features. Existing plantings on
adjacent private property will be integrated into
the overall design and will provide a counterpoint
to the Date Palms and other colorful plantings
located along the Western Promenade. Paving
will consist of decorative precast concrete pavers,
and Streetscape furnishings will be utilized to
provide a unified and consistent theme for both
the East and West sides of Collins Avenue, The
new Beach Access Entrance Features will utilize
Medjool Date Palms as well as colorful shrubs, and
native evergreen groundcovers, Additionally, new
plaza areas will be provided with paving patterns
that help to reinforce a "Beach" inspired sense of
place. Finally, Decorative Streetscape furnishings
and Overhead Shade Structures will be utilized to
provide pedestrian comfort, as well as points of
visual interest along the Collins Avenue corridor.
Intersections and Open Space Areas
A primary concern for the City of Sunny Beach is
the creation of a safe environment that functions
for both Pedestrian and Automobile Traffic. New
crosswalks, Bus Shelters, and an integrated
signage program will be provided in a manner
that coordinates with the overall Streetscape
Improvement Program. Additionally, the Existing
Sunny Isles Identity Signs will be retrofitted to
reflect the new City Signage Standards, Finally,
new colorful plantings
and public artwork will be
utilized to increase the
.
aesthetic beauty and cultural identity of the City of
Sunny Isles Beach,
Task 5 - Public Presentation and Participation
Once a preliminary report has been prepared,
reviewed and approved by the City, the next step in
the process is to present the design concepts to the
residents and the adjacent property owners. The
E&R Team will prepare a Powerpoint presentation
and all necessary handout packages for public
meetings as needed or requested by the City. All
public comments will be recorded and minutes
of the meetings prepared. The comments and
suggestions will be provided to the City and
discussed. Any comments that the City and the
E&R Team agrees on should be implemented to
update the report.
The final report will be provided to the City for
review and comments, then presented to the City
commission for approval and adoption.

The E&R Team have all completed projects for
various state, county and local agencies in the
South Florida Area. Almost all of the E&R Team has
performed worked for the City of Sunny Isles Beach,
either as a prime or a subconsultant.
Mr. Hekmat has been involved in many of the
City's capital projects in the past few years; most
recently he was involved in the l72nd Avenue
Road Improvement and the Atlantic Avenue Sewer
Collection and Road Improvement projects.
Ms. O'Leary has been providing landscape
Architectural services to the City of Sunny Isles
Beach for many years completing the Collins Avenue
median Landscape design and the median redesign
of the City Hall Complex, ODRA is currently working
on Landscape Design concepts for Collins Avenue
and have completed similar projects for City of
Aventura (Biscayne Boulevard) and Village of Bal
Harbor (Collins Avenue/AlA).
WSM completed the mapping of Collins Avenue in
the City for the original City of Sunny Isles Beach
. Streetscape Master Plan
and Ms. Weidener was
responsible for the QA/QC
of that project.
Nutting has also been involved in many projects in
the City of Sunny Isles Beach, most recently they
completed a phase 1/11 environmental assessment
as well as geotechnical exploration, pile load test
and monitoring, materials testing and roof fastener
uplift test for the Active Park.
Mr. Ross has been involved in Hydraulics and
Hydrology: surface water and ground water
modeling, drainage design, utility design including
sizing pumps, flood control, environmental resource
permitting, stormwater master planning studies
and canal-culvert design, he is very familiar with
permitting through Miami-Dade County Department
of Environmental Regulation and Management
(DERM), South Florida Water Management District
(SFWMD) and FOOT,
BCC Engineering, Inc specializes in providing
planning, design and construction management
services for civil, structural and transportation
related projects. Mr. Munoz has been involved
in FOOT project and is familiar with the District's
requirements and permit process.

d. WSM started working on this 1.5 mile elevated transit system throughout downtown Miami in August 1989. Full design
surveys including HPNC, VPNC, route surveys, topographic surveys at all proposed stations and utility surveys were
provided. During construction WSM was employed by the construction manager to verify foundations, piers, pier caps,
plinths, running pads and guideways. In 2002, WSM has hired to provide boundary surveys of the entire system. Various
land lines were tied via survey ofthe City of Miami monument lines in the vicinity which were part of the original
control for the project. Total fee for the design and construction management was $515,200 and the boundary surveys
were $130,171. Mr. Fernandez was the surveyor in responsible charge of the proiect throughout all its phases.

Collins A venue (S.R. A I A) is the main thoroughfare
in the oceanfront community of Sal Harbour. A
sub-tropical look that portrays a resort community
image was created. Elements included in the
streetscape design are bus shelters, decorative
paving, benches, trash receptacles, lighting, signage
and lush landscaping. The predominate tree used in
the median was the Coconut Palm, a tree that reflects
the tropical image of South Florida. The 'Zahedii'
Date palm lines the swales which gives it a majestic
look. Collins A venue is the signature roadway for
the Village of Sal Harbour, Florida's most luxurious
locale.

The directive from the City of A ventura was to
develop a Landscape Master Plan for the entire City
of A ventura. A part of this plan included the
streetscape design for A ventura Boulevard, a main
gateway into the City. The landscape concept
included new custom street lighting, new custom
street furniture including bus shelters, entry feature,
benches, trash receptacles, landscape lighting,
xeriscape irrigation, illuminated street signs and
landscaping. The landscape palette included
Majestic Date and Coconut Palms in the median,
with Live Oak trees shading the pedestrian
walkways in the swales. The medians were accented
with evergreen groundcover in the center and
flowering seasonal color at the ends. A ventura
Boulevard is the signature roadway for the City of
A ventura.

2. I understand that a "public entity crime" as defined in Paragraph 287.133(1)(g), Florida Statutes,
means a violation of any state or federal law by a person with respect to and directly related to the
transaction of business with any public entity or with an agency or political subdivision of any

services to be provided to any public entity or an agency or political subdivision of any other state
or of the United States and involving antitrust, fraud, theft, bribery, collusion, racketeering,
